It is well accepted that good muscle coverage of the bones at the end of a below knee amputation (BKA) stump is preferable, for both weight bearing and protection against prosthesis failure. Elderly patients often have atrophy of the leg musculature secondary to age-related physiological changes and decreased use. These patients often have poor coverage and bulk in their stumps after the standard BKA. We propose a selective muscle-sparing approach to these patients, utilizing selective removal of muscle bundles with regard to their blood supply and fascial planes. The surgical method technique along with outcomes of patients undergoing the procedure is presented here.

The results showed that (1) LINC00265 expression was increased in OA chondrocytes, (2) si-LINC00265 inhibited OA chondrocyte apoptosis and inflammation, and (3) LINC00265 overexpression promoted normal and OA chondrocyte apoptosis and inflammation. Furthermore, we predicted and confirmed that miR-101-3p was a target of LINC00265. LINC00265 negatively regulated miR-101-3p in OA chondrocytes and LINC00265 promoted OA and normal chondrocyte apoptosis via miR-101-3p. Overall, lncRNA LINC00265 regulates chondrocyte apoptosis by acting as a sponge of miR-101-3p in OA.In this study, the dynamic contractions and the associated fatigue condition in biceps brachii muscle are analysed using Synchrosqueezed Wavelet Transform (SST) and singular value features of surface Electromyography (sEMG) signals. The lower values of SVEn with the progression of fatigue indicate the randomness in the signal. Thus, it appears that the proposed approach could be used to characterise dynamic muscle contractions under varied neuromuscular conditions.
To evaluate the role of sentinel lymph node (SLN) biopsy in patients with conjuctival melanoma (CjM).

Retrospective observational cohort study and literature review.

Slovenian patients with CjM are included in the study.

Prospectively collected data of CjM patients treated from June 2005 to December 2016 were retrospectively analyzed.

The numbers of SLN biopsy procedures, positive and false positive SLN, and local and regional relapses have been described together with overall survival.

From June 2005 until December 2016, 24 patients with CjM were treated. The median follow-up time was 65.3months. The mean Breslow thickness was 1.5mm (sd = 1.8mm), and ulceration was present in 29% of cases. Altogether, 14/24 (58%) SLN biopsy procedures were performed. SLN was positive in 2/14 (14%) cases. The estimated 5-year overall survival (OS) of the group was 72.5%, with a median survival of 151months (95% CI 77-224). From January 2013 to January 2020, five (5/140, 3%) authors reported results comparable to our study.

Our results confirm that CjM is a rare disease with approximately 14% of positive SLN. At the moment, there are no firm conclusions regarding who would benefit most from SLN biopsy or whether or not CLND should be offered. Data from literature emphasize the need for consistent and uniform staging and future multicentric studies.

Since Prinzmetal first described a 'variant' form of angina pectoris, with predominantly resting episodes of pain and cyclic severity variations, it has gradually become apparent that this clinical presentation is caused by episodes of coronary artery spasm (CAS) involving focal or diffuse changes in large and/or small coronary arteries in the presence or absence of 'fixed' coronary artery stenoses. However, most clinicians have only limited understanding of this group of disorders.

We examine the clinical presentation of CAS, associated pathologies outside the coronary vasculature, impediments to making the diagnosis, provocative diagnostic tests, available and emerging treatments, and the current understanding of pathogenesis.

Many patients presenting with CAS crises have non-diagnostic ECGs and normal serum troponin concentrations, but CAS can be suspected on the basis of history and association with migraine, Raynaud's phenomenon and Kounis syndrome. Treatment still centers around the use of calcium antagonists, but with greater understanding of pathogenesis, new management options are emerging.

The aim of this study is to perform an audiometric assessment of the effectiveness of surgical treatment of otosclerosis depending on the incidence of Carhart's notch. Methods The analysis included 116 patients treated surgically for the first time due to otosclerosis. Patients were divided into 4 groups depending on the occurrence of Carhart's notch, determined by pure-tone audiometry (PTA) before the surgery and 36 months afterward. The mean value of bone conduction thresholds was calculated for 500 Hz, 1000 Hz, 2000 Hz, and 3000 Hz in the groups in which the Cahart's notch was observed. This value of bone conduction (BC) was a reference point for further analysis in patients who had no preoperative or postoperative Carhart's notch. Results The analysis indicated that Cahart's notch in preoperative PTA is a statistically significant improvement factor for average BC.
