Prior opioid use can influence outcomes for patients with musculoskeletal disorders. The purpose of this study was to compare downstream medical utilization-based outcomes (costs, visits, recurrent episodes) after an initial diagnosis of patellofemoral pain based on pre-injury utilization of opioids.
A total of 85,7880 consecutive patients were followed for a full 12months before and 24months after an initial diagnosis of patellofemoral pain (January 2009 to December 2013). Data were sourced from the Military Health System Data Repository, a single-payer closed government system. Opioid prescription fills were identified, and medical visits and costs were calculated for all knee-related medical care, to include recurrence rates in the 2-year surveillance period.
A relatively small number of individuals filled an opioid prescription in the year prior (n = 1746; 2.0%); however, these individuals had almost twice the mean costs of knee-related medical care ($1557 versus %802) and medical visits (8.4 versusithout prior opioid use. • Prior opioid use has predicted higher costs and poor outcomes after surgery, but this is the first study to confirm similar findings in non-surgical patients.This study was aimed to analyze the readability and quality of patient education websites on esophageal cancer. Yahoo!, Google, and Bing search engines were searched using keywords esophageal cancer, esophageal tumor, esophageal tumor, esophageal malignancy, esophageal cancer, esophageal tumor, esophageal tumor, and esophageal malignancy. The first 50 websites resulting in each keyword search were evaluated using validated FRES, LIDA, and DISCERN scores to assess readability, usability, and reliability, and quality of information, respectively. Non-parametric tests were used for statistical analysis. A total of 108 eligible websites were included in the analysis. Thirty (27.8%) out of the 108 eligible web sites had obtained Health on the Net (HON) code certification. The median FRES score of the included websites was 48.25 out of 100 (range 15.6-70.1). The median LIDA usability and reliability scores were 46.5 out of 54 (range 22-54) and 39.0 out of 51 (range 10-51), respectively. The median DISCERN score was 50.5 out of 80 (range 23-79). A low DISCERN score (≤ 50%) was found in 50% (n = 54) of the websites. The DISCERN score was found to be significantly associated with LIDA usability, reliability, LIDA overall scores (p less then 0.001), and HON code certification (p = 0.01). The quality of the websites providing patient-centered information on the Internet ranged between moderate and low with regards to readability, usability, and reliability scores. Better informed decisions on treatment may be facilitated with the access to good quality information online. Therefore, strategies need to be implemented to regulate and standardize websites to provide good quality, accurate information.Exogenous cyanide (CN-) effects on nitrogen (N) uptake, transport, and assimilation in rice seedlings were investigated at the biochemical and molecular levels. Seedlings were treated with either a 2-d or 4-d supply of potassium cyanide (KCN) in the nutrient solution containing nitrate (NO3-) or ammonium (NH4+). Although a KCN-induced increase was recorded in the activity of β-cyanoalanine synthase (β-CAS) in rice tissues of both NH4+-fed and NO3--fed seedlings, the former showed a significantly greater assimilation rate for CN- than the latter. The addition of KCN decreased NO3- uptake and assimilation, whereas a negligible impact was observed in NH4+ treatments. Enzymatic assays showed a marked activities enhancement of glutamine synthetase (GS), glutamate synthase (GOGAT), and glutamate dehydrogenase (GDH) in NH4+-fed seedlings after KCN exposure. see more Similarly, the rises occurring in nitrate reductase (NR) and GDH activity in NO3--fed seedlings were also detected after 2-d exposure to KCN, whereas a significant reduction of GS and GOGAT activities was determined. The results suggest that although exogenous KCN at moderate or high concentrations caused repressively effects on biomass growth of both NH4+-fed and NO3--fed rice seedlings, the nontoxic concentration of KCN supplied can serve as a supplemental N source in plant nutrition and N metabolism.The biobed is a purification system, which reduces soil pollution for receiving pesticide residues from handling and washing machinery in agricultural areas. The aims of this study were (1) to assess ecotoxicity effects over time to soil fauna, posed by Lorsban® 480 BR (Chlorpyrifos) and Dithane® NT (Mancozeb) residues when disposed of in a biobed system compared with two subtropical soils, and (2) to assess ecotoxicity effects over time to soil fauna simulating an accidental spillage with Lorsban® 480 BR at the biobed. A semi-field experiment was conducted for 420 days in southern Brazil, testing continuous disposal of washing pulverization tanks in biobeds, Typic Haploperox or Typic Hapludults. In addition, different biobeds received a single dose (1 L) of Lorsban® 480 BR to simulate an accidental spillage. Chronic ecotoxicity tests were performed using Folsomia candida, Eisenia andrei, and Enchytraeus crypticus in different sampling times for both experiments. F. candida was the most sensitive species. The biobed system was able to eliminate effects from residues of both pesticides over time in all species, which did not happen in both natural soils. In accidental spillage simulation, even 420 days after contamination, F. candida did not show reproduction. The biobeds can be a feasible alternative for the disposal and treatment residues of pesticides, also for handling and washing pesticides activities. The system was efficient in promoting degradation and reducing ecotoxicity effects posed by Lorsban® 480 BR and Dithane® NT for soil fauna. It is a safe alternative to avoid soil contamination.
