01). PMA was also correlated with hand grip strength and gait speeds (p less then .01). PVR was correlated with height-adjusted ASM and BMI-adjusted ASM (p less then .01). A high PVR significantly decreased the odds ratios for hypertension and diabetes in the unadjusted model and the model adjusted for age, smoking, and drinking status. The ratio of the cross-sectional area of thigh muscle to the cross-sectional area of visceral fat and the BMI-adjusted ASM produced results similar to those of PVR in terms of the odds ratios for hypertension and diabetes. CONCLUSION. Single-slice abdominal CT can supply PMA and visceral fat information together. PMA and PVR were found to be reliable indexes of sarcopenia and sarcopenic obesity. A high PVR was associated with low risks for hypertension and diabetes.Purpose Understanding what limits speech development in minimally verbal (MV) children with autism spectrum disorder (ASD) is important for providing highly effective targeted therapies. This preliminary investigation explores the extent to which developmental speech deficits predicted by Directions Into Velocities of Articulators (DIVA), a computational model of speech production, exemplify real phenotypes. selleck inhibitor Method Implementing a motor speech disorder in DIVA predicted that speech would become highly variable within and between tokens, while implementing a motor speech plus an auditory processing disorder predicted that DIVA's speech would become highly centralized (schwa-like). Acoustic analyses of DIVA's output predicted that acoustically measured phoneme distortion would be similar between the two cases, but that in the former case, speech would show more within- and between-token variability than in the latter case. We tested these predictions quantitatively on the speech of children with MV ASD. In Studyvealed that the speech of the child from the high-variability cluster showed significantly more quantitative within- and between-token variability than the speech of the child from the low-variability cluster. The vowel space of the child from the low-variability cluster was more centralized than that of typical children and that of the child from the high-variability cluster. Conclusions Results provide preliminary evidence that subphenotypes of children with MV ASD may exist, characterized by (a) comorbid motor speech disorder and (b) comorbid motor speech plus auditory processing disorder. The results motivate testable predictions about how these comorbidities affect speech. Supplemental Material https//doi.org/10.23641/asha.14384432.Gastric lipomas are rare benign neoplasms of the stomach. link2 These submucosal lesions and located mostly in the antral region of the stomach. Small lipomas are usually asymptomatic and are detected incidentally. When large, they may present with abdominal pain, gastrointestinal (GI) bleeding or gastric outlet obstruction. We hereby present a case of gastric lipoma in a 54-year-old man presenting with massive upper GI bleed and haemodynamic instability. The diagnosis was established with endoscopy and contrast-enhanced computed tomography of the abdomen. After resuscitation, the patient underwent laparoscopic resection of the antral lipoma.
Practice guidelines recommend the prophylactic use of granulocyte colony-stimulating factors (G-CSFs) in patients with high risk of febrile neutropenia, but evidence suggests that G-CSFs are frequently overused. The objectives of this study were (1) to determine the prevalence and prescribing patterns of G-CSF and (2) to evaluate the impact of a program initiative on G-CSF prescribing patterns, adherence to guidelines, and mortality.
In this retrospective cohort study, data were used from the electronic health records of patients with metastatic colorectal cancer who received care at a multicenter oncology practice network during two time periods July 01, 2013, to December 31, 2014, and July 01, 2017, to December 31, 2017. Beginning 2016, a site-wide program initiative that involved educational materials, appropriate nonuse recommendations, and prior authorization was introduced in the oncology practice network with an aim of reducing G-CSF overutilization. Descriptive statistics,
tests, and chi-squarep prescribers adopt a cost-effective approach in patients with metastatic colorectal cancer, leading to enhanced clinical practice and value-based care.Catastrophic antiphospholipid syndrome (CAPS) is a life-threatening condition characterized by multiple thromboembolic events occurring in a short period of time, frequently accompanied by significant systemic inflammation. Aortic involvement is rare in antiphospholipid syndrome and it had been never described in the context of its catastrophic variant. Here, we report an unusual case of aortic occlusion as a debut manifestation of CAPS and discuss its clinical features with an up-to-date review of the literature to identify risk factors and clues for clinical practice.
Recent advances in structured reporting are providing an opportunity to enhance cancer imaging assessment to drive value-based care and improve patient safety.
The computer-assisted reporting and decision support (CAR/DS) framework has been developed to enable systematic ingestion of guidelines as clinical decision structured reporting tools embedded within the radiologist's workflow.
CAR/DS tools can reduce the radiology reporting variability and increase compliance with clinical guidelines. The lung cancer use-case is used to describe various scenarios of a cancer imaging structured reporting pathway, including incidental findings, screening, staging, and restaging or continued care. Various aspects of these tools are also described using cancer-related examples for different imaging modalities and applications such as calculators. Such systems can leverage artificial intelligence (AI) algorithms to assist with the generation of structured reports and there are opportunities for new AI applications to be created using the structured data associated with CAR/DS tools.
These AI-enabled systems are starting to allow information from multiple sources to be integrated and inserted into structured reports to drive improvements in clinical decision support and patient care.

This study determined whether an electronic version of the geriatric assessment is feasible in a multi-institutional, diverse setting.
Ten sites within the Alliance for Clinical Trials in Oncology participated. link3 Patients who had active cancer or a history of cancer and were 65 years of age or older were eligible. The geriatric assessment was completed with an electronic data capture system that had been loaded onto iPads. Feasibility was defined a priori as completion in at least 70% of patients either with or without help. To enhance racial diversity, the original sample size was later changed and augmented by 50% with the intention of increasing enrollment of older minority patients.
A total of one hundred fifty-four patients were registered with a median age of 72 years (range, 65-91 years). Forty-three (28%) identified themselves as African American or Black. One hundred forty-one patients (92%) completed the electronic geriatric assessment. Feasibility was observed across all subgroups, regardless or Black patients.Wrinkles in bilayer systems comprising a thin stiff film attached to a soft substrate can globally transition into folds under sufficiently large compression. This phenomenon has been extensively studied primarily using uniaxially compressed systems. However, inducing the wrinkle-to-fold transition at designated locations on a wrinkled surface under small biaxial compression remains a challenge. In this study, we describe a method for causing randomly oriented wrinkles to locally evolve into folds using water droplets. When a droplet comes into contact with the random wrinkles that have spontaneously formed upon film deposition owing to residual biaxial compressive strains, radially extended folds instantaneously emerge at the droplet boundary. Upon water evaporation, the wrinkles beneath the droplet also undergo a transition, leaving a fold network. By contrast, the surface regions distant from where the droplet was placed retain the wrinkle morphology. The folded areas can be controlled by adjusting the volume and number of droplets. These transitions are enabled by the capillary forces of water that help to increase the local compressive strains. This capillary-induced wrinkle-to-fold transition provides a simple mechanism to develop folds in selected locations on wrinkled surfaces of film-substrate systems subject to small biaxial compression, which is unachievable with conventional approaches.Cross-linking mass spectrometry (XL-MS) has become a powerful technique that enables insights into protein structures and protein interactions. The development of cleavable cross-linkers has further promoted XL-MS through search space reduction, thereby allowing for proteome-wide studies. These new analysis possibilities foster the development of new cross-linkers, which not every search engine can deal with out of the box. In addition, some search engines for XL-MS data also struggle with the validation of identified cross-linked peptides, that is, false discovery rate (FDR) estimation, as FDR calculation is hampered by the fact that not only one but two peptides in a single spectrum have to be correct. We here present our new search engine, MS Annika, which can identify cross-linked peptides in MS2 spectra from a wide variety of cleavable cross-linkers. We show that MS Annika provides realistic estimates of FDRs without the need of arbitrary score cutoffs, being able to provide on average 44% more identifications at a similar or better true FDR than comparable tools. In addition, MS Annika can be used on proteome-wide studies due to fast, parallelized processing and provides a way to visualize the identified cross-links in protein 3D structures.Folpet, a fungicide used on several crops, easily degrades into phthalimide (PAI) at high temperatures and basic pH. The maximum admitted limit for Folpet in foodstuffs as coffee is defined by the sum of its amount and that of PAI. Noteworthy, PAI can also arise from the reaction between ubiquitous phthalate derivatives and NH3. This work aims to demonstrate that the detection of PAI in roasted coffee is not necessarily diagnostic for Folpet as it can also originate from the reaction between phthalic anhydride (PAA), derived from phthalates, and amino acids (AAs), as a NH3 source. Thermal treatment of AAs with PAA confirmed that PAI generation follows a temperature-dependent path. Experiments with diethyl phthalate (DEP) and AAs have shown that maximum PAI generation via heating occurs at 200 °C for 60 min. PAI generation has also been proven for Folpet-free green coffee beans that were heated under laboratory and industrial roasting conditions.Developing highly efficient photocatalysts to utilize solar radiation for converting CO2 into solar fuels is of great importance for energy sustainability and carbon neutralization. Herein, through an alkali-etching-introduced interface reconstruction strategy, a nanowire photocatalyst denoted as V-Bi19Br3S27, with rich Br and S dual-vacancies and surface Bi-O bonding introduced significant near-infrared (NIR) light response, has been developed. The as-obtained V-Bi19Br3S27 nanowires exhibit a highly efficient metallic photocatalytic reduction property for converting CO2 into CH3OH when excited solely under NIR light irradiation. Free of any cocatalyst and sacrificial agent, metallic defective V-Bi19Br3S27 shows 2.3-fold higher CH3OH generation than Bi19Br3S27 nanowires. The detailed interfacial structure evolution and reaction mechanism have been carefully illustrated down to the atomic scale. This work provides a unique interfacial engineering strategy for developing high-performance sulfur-based NIR photocatalysts for photon reducing CO2 into alcohol for achieving high-value solar fuel chemicals, which paves the way for efficiently using the solar radiation energy extending to the NIR range to achieve the carbon neutralization goal.
