Kami atas nama Vladimir Hospital.
Mengumumkan bahwa setiap pergantian Bulan ,hospital akan mengaktifkan sistem booking,sistem ini dilakukan bertujuan agar teratur dan tertibnya pelaksanaan saat melakukan MCU.
Note:
•pembookingan MCU akan dilakukan selama 1 minggu sebelum dimulainya kegiatan ini,lewat dari seminggu pelayanan pembookingan akan menjadi slow respon.
•Hasil MCU akan dibagikan setiap hari minggu.
•Setiap hari akan di update hari apa saja yang masih avail untuk melakukan MCU dan mengingatkan orang yang telah membooking hari itu.
•waktu jeda melakukan Medical Check Up yaitu saat pelajaran berlangsung.
Dengan ini, saya Tiffany Brooklyn Whitmore selaku Manager Vladimir Hospital menyatakan beberapa rules atau ketentuan dalam menjalakan MCU, yakni :
1. Mengisi Form Pendaftaran MCU
Meliputi dari :
• Nama :
• TTL :
• No. ID/KTP/KLP :
• Uname :
• Status :
2. Menentukan Tanggal dan Waktu MCU
Meliputi dari :
--- Senin s/d Jumat ---
•Max 2 Orang MCU[10.00-23.00]
(Jam ditentukan oleh orang yang membooking)
--- Sabtu s/d Minggu ---
•Max 3 Orang MCU[10.00-23.00]
(Jam ditentukan oleh orang yang memboking)
*Note :
•Jika tidak dapat menghadiri MCU saat itu bisa langsung melaporkan ke manager dan memberikan tanggal pasti untuk melakukan MCU .
•Format hanya dikirim melalui account manager.
