Mental Health Nursing - Assessing Mental Health
A physical exam is often included in mental health assessments to rule out physical issues. This may include blood tests, urine tests or brain scans.
Psychologists also use a wide array of tools to measure mental health. They can use tests that are written on paper and pen to deep interviews or self-fill questionnaires sent to you by post.
Affect and mood
The doctor will first observe the patient's mood and affect. They define a patient's general emotional state. It can range from happiness to sorrow or anger and irritability. The degree of affect can be determined by watching facial expressions and attitude as well as reactivity to internal or external stimuli (e.g. You can express your feelings by laughing at a joke, or crying when you lose a loved one.
Affect is a feeling that lasts only a few minutes but mood is a more sustained emotional state. It is often described as being either euthymic (normal) or dysphoric, meaning depressed. Psychiatric professionals should be careful to avoid confusion between the two terms, as they are defined differently and can be affected by various factors.
While the MSE is a useful diagnostic tool, it isn't always easy to interpret based upon subjective patient report. It is therefore helpful to ask patients how they have been feeling over the last few weeks, instead of using only the observation of a physician. A recent study showed that a large proportion of symptoms, even those grouped together under the same disorder category, vary between assessment tools. This could be due to the different emphasis placed on emotional or cognitive symptoms, as well as the different time periods and frequency in symptom reporting.
It is possible that the MSE is based solely on self-reporting, which can be biased. For instance, a person who complains of anxiety and headaches is likely to report a lower score than a patient who reports only a headache. Patients tend to minimize negative emotions while exaggerating positive ones.
Affect and mood have a direct impact on thoughts which influences the way we make decisions, our interpersonal behavior and physical functioning. They can be affected by stress, fatigue social interactions, global events, hormones and even the weather. In fact, a positive mood has been shown to enhance creativity and thinking power. A negative mood can cause confusion and decrease concentration. A thorough and thoughtful mental health assessment should take into account the patient's mood and affect as well as the underlying factors.
Thought content
Assessing how the patient thinks is one of the most important aspects of a mental health exam. Particularly, it is assessing thought process and thoughts. The thought process is the coherence, logic and structure of the patient's thoughts. Normal thought processes are rational and focused on a goal. Thought processing can be altered in a variety of mental health conditions. Examples include hallucinations, delusions, and loose associations.
Insight and judgement are also assessed during a mental state test. Insight is the ability to spot a problem. Judgment is the ability to make a rational decision after receiving information. Both can be impaired in various mental health conditions, such as schizophrenia and depression. Patients with impaired insight or judgment is more prone to harm themselves or others. For mental assesment if they are at risk of burning their home down and they have a good sense of judgement, it would be wise for them to leave the house immediately. A person with impaired judgment might suggest that they remain in the building to try and put out the fire.
Thought content is the topics, subject matter, and beliefs that a patient holds. Examiners will assess the clarity, relatedness and the content of the patient's thoughts. They will be able to identify tangential ideas or those that are merely circumstantial, and if the patient has digressions (digressions away from the topic of the conversation). They will also comment on a person's pace of thinking, for example, if they seem fast or slower.
Other important aspects to consider are whether the patient's thoughts are mood compatible or not in line with their surroundings. For instance, a person who is depressed may hear voices that sound hostile and frightening, whereas those suffering from schizophrenia may experience a hallucination of the sound of a calm and soothing voice. Ask the patient if they have any thoughts of suicide or homicide. These are uncommon, but could be life-threatening and should be taken into account in every mental health assessment.
Watching
A nurse's observation skills play an important role in the mental health assessment. They will conduct a thorough examination of appearance, behaviour, any unusual beliefs and perceptions (such as hallucinations or delusions) mood, and aspects of cognition (including attention to the environment, orientation memory, and orientation). The mental status exam for nurses also includes observing the movement. This is any physical activity, like eating, drinking, or brushing your teeth.
The Observation component of the mental health examination is crucial in determining if the patient is a danger to them or others. This is determined by assessing whether or not the patient appears to be moving in a way that suggests that they are about to harm themselves or anyone else.

It is important to recognize that observing the behavior of a patient may cause anxiety. It is also crucial that the patient is informed about the purpose of the observation and the length of time it will last.
Nursing observation is an essential ability and should be an integral part of the nursing process. However, it must be kept in mind that patients could disagree with the level of observation and if they cannot reach an agreement with staff then they should have access to an advocate service that will represent their needs and help patients in any discussions about their care. This should be a fundamental part of local observation policy and practice, including the involvement of user groups to ensure that patients are included in the creation of written information as well as staff training on this aspect of governance in the clinic.
Questioning
Many people are reluctant to talk about their mental health issues, but their family members are often able to discern when something isn't right. They can also help the person to get an assessment. These assessments cover a wide range of subjects that include mood, appearance and thought process. In addition to these tests, mental health professionals will also ask the person questions about their personal life, such as what's happened lately and what their feelings have been like.
The test also includes a cognitive assessment that is focused on the person's ability to think clearly and retain information. The person could be asked to complete simple tasks such as focussing on a list or objects, remembering lists or solving math questions. The test will also test the person's ability to make informed decisions and make good judgments. The test measures a patient's perception, which is their knowledge of their condition and how it affects them.
The person's attitude toward others, himself and the illness is an important aspect of a mental evaluation. The tone of voice, facial expressions and body language can convey hostility, anger depression or a feeling of powerlessness. It is important to observe whether the patient seems to seek help or accepting their condition.
In more serious cases the evaluation of mental health could include an examination the patient's mental state which could include illusions or hallucinations. mental assesment can be harmful and even fatal.
In general the mental health assessment should take a holistic approach and examine the person's connection to their social life. A health professional might use art, music, photos or a variety of assessment tools that are electronic to help understand the person and their unique perspective on the world around them. They should also talk with the person about the issues that have led them to seek help and reassure them that there is nothing wrong with visiting an expert in mental health.
