I don't have access to my school email anymore and I can't recall, but I believe due to me moving to a different state only a week after I moved home from Normal, I wasn't checking my email to know I had been dismissed from ISU and by the time I did the deadline had passed.
Explain any unusual circumstances which contributed to your academic dismissal. How have those been resolved? (Please be aware that we are mandated reporters with the State of Illinois and may be required to report certain circumstances to Equal Opportunity and Access) Note: Copies of additional documentation (i.e. doctor's letter, etc.) can be attached.
I don't think I have any "unusual" circumstances. However, I was placed in a dorm with somebody who had no interest in making my time in there anywhere near pleasant(I'd prefer to not have to provide too many more details on this because of the fact that your question made me aware of). Also I made attempts to change dorms 1st semster but none were successful. Along with the issues in my dorm, my girlfriend for the entirity of my freshman year was having health complications(from horrible migranes to fainting multiple times a day with doctors having absolutely no solution) so with us being hours away from home, it put a lot of strain to get by day to day having to juggle work, keeping us in as good of health as possiblle, and working. Along with these I was offered my intern oppurntinaty at Google, but that meant that I also had to find a way to afford to move us to Boulder, CO for the summer and prepare for the job duties that would take time away from everything else I had to.(I don't at the moment have my girlfriends medical records from this previous year but I can provide them upon request)
Describe your personal plan for academic success in your next semester at Illinois State University. What things will you do differently to earn “A” and “B” grades? How will you make academics a priority?
Using the money I have and plan to save from my summer internship as a software engineer at Google, next semster I won't have to work as much to pay for school and in turn will let me focus on school and my personal health more.
If you have been reinstated in the past, please explain the circumstances regarding your previous reinstatement--conditions not met, limited progress made towards academic good standing--in other words, why should ISU feel confident that you will now take advantage of being reinstated and as a result be in good academic standing?

Discuss your participation in Project Success and/or Project Rebound. Did you find the program helpful? Why or why not? What aspects of the program did you complete?
I think I only went to one workshop for Project Success but I don't remember what "aspect" that would be considered.
Which major do you wish to pursue at ISU? What are the requirements to gain admission to this major and how do you plan to meet those requirements? (Please be specific about your intended major). NOTE: If you are reinstated, you will be reinstated as an Undeclared student and will need to work with an Academic Advisor in University College on how/when to apply to your desired major.
I wish to pursue Computer Science. Being informed by Laurie Helms, the only requirement is to have a 2.3 gpa and I plan to achieve that by taking more of my gen eds and potientally retaking MAT 145 within the next two semsters to get the F away from my gpa
Lastly:
My passion is to pursue software engineering as a profession and I've finally achieved that by working this internship. However, I know that this isn't all I wanted and to keep knocking out more of my professional goals I need to complete school and learn a lot more along the way career and related to self. Going to school at ISU gives me the oppurnatiy to do both from having professors like Shukri Abotteen and Tal Parmenter who have some much knowledge for me take to living in the same city as my big brother and his soon to be son. So while I apologize to the students who lost oppurnities that I've wasted over the last year, I've declared that I won't go down the same path again twice.
