Disorganized, tangential and circumstantial connections are thought processes that may indicate mental health conditions such as mania, schizophrenia and bipolar disorder.

Psychologists and neuropsychologists also evaluate the client's attentional focus and capacity to hold information in short-term memory. This can be measured by self-reports, examiner observation or short tests such as counting backwards starting at 100 and multiplying it by sevens. They also evaluate the client's coping strategies and their cognitive performance in daily life, which can be assessed through direct questioning and behavioral observation.

During the MSE psychologists observe the client's facial expressions and body language to determine if they appear nervous or shy. They also look at the client's fidgeting and restlessness in order to determine whether they are fearful or anxious. Psychologists frequently use the MSE along with other testing and assessments to determine a diagnosis and formulate an appropriate treatment plan. Psychologists are also trained to detect the signs that a client's behavior may be similar to a particular mental illness or is instead caused by a different reason like alcohol abuse, injury or medication side effect.
