Thought process is the coherence, logic, relevance and flow of the client's thoughts when they respond to the examiner's questions during the MSE interview. The doctor will also determine if the thought process is goal-directed or disorganized and whether it moves rapidly from one topic to another without a apparent connection between them. Disorganized, tangential, and circumstantial connections are thought processes that may be indicators of mental health issues such as schizophrenia, mania and bipolar disorder.

Psychologists and neuropsychologists also evaluate the client's focus on task and capacity to hold information in short-term memory. This can be determined by self-reports, examiner observation or short tests like counting backwards from 100 and multiplying it by sevens. They also assess the client's strategies for coping and their cognitive performance in daily life, which is measured through direct questioning and behavioral observation.

During the MSE, psychologists observe the client's facial expressions and body language to determine whether they appear withdrawn or aggressive. They also look at the client's fidgeting and restlessness to determine if they are anxious or scared.
