ADHD Diagnosis in the UK
In the UK you have a legal right to choose the person who provides your specialist NHS mental health examinations. Not all healthcare professionals have the necessary qualifications to diagnose ADHD.
Panorama has heard from whistleblowers and patients who claim that private clinics diagnose ADHD and prescribe drugs without proper tests. We went undercover to learn more.
What is an assessment?
A psychiatrist will meet with you to discuss the effects of ADHD on your life and whether medication can assist. You will need to bring details of your symptoms and any issues that they may be causing. You will be asked about your personal and professional life. Some patients find it useful to write down symptoms prior to their appointment. This is so they can remember their symptoms. An assessment usually involves a 45 to 90 minutes of discussion with the psychiatrist.
You can have an adult ADHD evaluation through the NHS or privately. Doctors must take any concerns about ADHD seriously and are willing to refer you for an assessment. The system is not without problems. The BBC's Panorama program raised concerns over doctors prescribing powerful medications for prolonged periods of time and not taking into account the medical history of patients.
If you're worried about the attitude of your GP towards ADHD, consider finding another GP. On the Psychiatry-UK site, you will find templates and suggestions to make your request.
Private psychiatric treatment is faster and more professional, particularly in London. The price can range from PS600 up to PS1,200. Private psychiatrists have experience conducting assessments of adults with diverse backgrounds and can offer specific solutions, particularly in situations where you may be extremely knowledgeable or have a successful job. They can also screen for and help with co-morbidities, such as depression and anxiety which are frequently present in conjunction with ADHD.
The Psychiatrist examines your symptoms based on the DSM V criteria and determines if you meet the criteria of an ADHD diagnosis. You will have to demonstrate six traits (symptoms), which are present in your daily life and how they've affected your previous experiences.
The Psychiatrist then informs you of your options and the next steps. You can decide to start treatment with stimulants such as methylphenidate. Stimulants are covered under the Misuse of Drugs Act and are therefore controlled. They can only be prescribed by a psychiatrist or doctor registered with the General Medical Council.
What is a diagnosis?
A diagnosis is a term used in medicine that refers to the diagnosis of a condition, illness or injury. A doctor will provide a diagnosis and often accompany it with an action plan. The diagnosis is an essential part of the curriculum of medical students and is an integral part in the training of doctors across all areas of medicine. The word is frequently used in medical journals as well as in articles on health care.
There are many ways to get an ADHD diagnosis in the UK. One option is to visit your GP and ask for referral to an adult ADHD specialist or clinic. A private psychiatrist can also make the diagnosis. Private psychiatrists usually have a lot of experience in diagnosing adults suffering from ADHD and can offer the most rapid, professional and understanding process. They are also aware of other conditions that can cause similar symptoms to ADHD such as depression and anxiety.
Once you have received a diagnosis and have a treatment plan, you can discuss options with your psychiatrist. This can include medication or cognitive therapy (CBT), and other non-medical treatments like mindfulness or relaxation techniques. You can then decide whether you'd like to choose the treatment that is most suitable for your requirements.
In addition to allowing you to seek treatment for your ADHD The diagnosis can assist you in obtaining reasonable adjustments and Disability Living Allowance (DLA). In the case of DSA this is crucial as it aids in daily activities that those with disabilities or illnesses need to do.
If you're thinking about stimulants such as methylphenidate for treating your ADHD be aware that these drugs are class B drugs and can cause addiction. If you're concerned your psychiatrist may refer you to an GP that can prescribe a treatment without stimulants.
Finding an ADHD diagnosis can be a difficult and frustrating process. There are a variety of things to take into account as well as the NHS system is often over stretched and under resourced, so it can be extremely easy for doctors or other health professionals to dismiss or minimize a possible diagnosis. You can bring a list of symptoms to the assessment and be objective when describing your symptoms.
How do I get an medical diagnosis?
Your child or you may have ADHD. The first step to controlling symptoms that can affect your life is to obtain a diagnosis. It's not an easy procedure, but it's essential to remember that ADHD isn't a sign laziness or a lack of intelligence. It is a neurobiological disorder. It is also important to recognize that the proper treatment can help you control your ADHD symptoms and improve the quality of your life.
To diagnose you an expert will typically evaluate you by the combination of a thorough background and discussion of your current symptoms and also analyze the results from questionnaires and scales developed to detect ADHD in adults. They then apply the criteria outlined by the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) to make an accurate diagnosis.
In addition to interviewing you and those close to you, the person evaluating you may want to carry out medical tests or cognitive tests to rule out any underlying causes for your symptoms. This can include thyroid disorders and seizures, which typically have similar symptoms as ADHD.
During the evaluation, it is important to be open about your symptoms and how they affect you at home, school or at work. This will enable the evaluator to determine if you suffer from ADHD or a co-existing condition which is contributing your symptoms.
If you're thinking about taking medication, your doctor will discuss it after the assessment has been completed. GPs cannot prescribe ADHD medication, but they can provide other types of mental health treatments such as CBT (cognitive behavior therapy). If you decide to get a prescription from your clinician and he or she will draft a document known as a Shared care agreement for your GP which outlines the treatment you will receive through the NHS.
Asking your friends and family members to recommend experts with expertise in ADHD evaluations of adults is a good idea prior to taking a private assessment. You can also search the internet for reviews of providers in your area and learn about the different methods they employ. Alternatively, you can try BetterHelp's easy-to-use service to get connected with a therapist who is experienced within 48 hours.
Can I get a diagnosis through Right to Choose?
If you're an active member of the Armed Forces, you have a right to choose which mental health professional you're directed to for an ADHD assessment. You'll need to make sure that your GP will accept full care or something called a 'shared care agreement' after the chosen provider has discharged you and prescribes treatment (usually medication or CBT therapy). You can determine which providers have these contracts with the NHS by calling them directly. You can find the waiting times of the providers on this site.
You will be asked about your symptoms, including when they began when your GP refers to an ADHD specialist via the Right to Choose path. This will allow your doctor to better understand your symptoms and their impact on your life. It is crucial to answer these questions honestly and clearly.
On the NHS website you can find out more about how to exercise Your Right to Choose. The NHS does not currently offer a titration for medications via the Right to Choose path. Even if you are diagnosed using this route, you might not be able to access the medication you need.
There are many other routes to an ADHD diagnosis, including private clinics. The BBC's Panorama program recently exposed private clinics that offer quick, cheap diagnoses for people with ADHD. Main Page can have serious consequences if you're seeking to claim benefits like Disabled Students Allowance or reasonable adjustments at work, for instance.
It's important to know that stimulants prescribed for ADHD are classified as Class B drugs, and are subject to restrictions under The Misuse of Drugs Act. If you're found guilty of committing an offence of criminal intent while taking these medicines you could be subject to an extensive fine or even jail time.
In addition to the above, if you're diagnosed with ADHD through the NHS or privately, you'll be entitled to disability benefits in the form of Attendance Allowance or Income Support according to the severity of your condition. You'll need an official letter from your doctor that proves this, along with an original copy of your medical records.
