Chronic intestinal pseudo-obstruction is a severe gastrointestinal tract disorder mimicking a bowel obstruction with no mechanical causes. With the opioid epidemic and the increasing number of opioid-related deaths, there is growing awareness in the medical community regarding the dangers of opioid overprescription. As a result, there is a willingness among physicians to abandon old norms and adopt new data-driven prescribing practices.

Included in the study were 168 patients who underwent an ACLR, knee arthroscopy, or arthroscopic rotator cuff repair over a 17-month period. Patients were excluded if they had an opioid allergy, had preexisting opioid use, had an acute postoperative complication requiring further surgery, required hospitalization, exhibited drug-seeking behaviors, or were lost to follow-up. Medical records were reviewed to determi, by respectively prescribing 20, 10, and 20 pills postoperatively, the amount of unused medications would decrease by 60%, 47%, and 64%, respectively. We recommend prescribing no more than 20, 10, and 20 hydrocodone/acetaminophen pills (5/325 mg) for ACLRs, knee arthroscopies, and arthroscopic rotator cuff repairs, respectively.

The influence of patient sex on clinical outcomes after arthroscopic anterior shoulder stabilization is unclear.

To investigate sex-based differences in clinical outcomes after arthroscopic anterior shoulder stabilization.

Cohort study; Level of evidence, 3.

A retrospective study was conducted among 76 patients who underwent arthroscopic anterior shoulder stabilization from February 2010 to December 2017. The patients were grouped by sex. The recurrence rate of instability was compared, as were pre- and postoperative pain scores, functional outcome scores, and active range of motion (ROM). Postoperative magnetic resonance imaging (MRI) was also performed for structural assessment of the glenohumeral joint.

No significant difference was found in the recurrence rate between female and male patients (13.3% vs 14.8%;
≥ .999; risk ratio, 1.107 [95% CI, 0.266-4.597]). Compared with male patients, female patients had a significantly lower preoperative Constant score (94.4 ± 6.4 vs 85.4 ± 11.1;
= .002operative values, more significant improvements in postoperative shoulder function and active ROM were seen in women.
Female patients had a similar recurrence rate as that of male patients after arthroscopic anterior shoulder stabilization. Most postoperative clinical outcome measures showed no significant difference between the sexes. Despite worse preoperative values, more significant improvements in postoperative shoulder function and active ROM were seen in women.
The axillary nerve is at risk for iatrogenic injuries in surgical procedures involving the lateral aspect of the shoulder joint. To date and to our knowledge, there have been no studies that have compared the relevant distances in the common arm abduction positions used in these types of surgery as well as the relative risks of each position.

To evaluate the effect of arm abduction position on the distance from the acromion process to the axillary nerve in the common abducted arm positions.

Descriptive laboratory study.

The shoulders of 10 fresh-frozen, full-body cadaveric specimens were used. A saber incision was made at the anterolateral edge of the acromion, and the anterior branch of the axillary nerve was identified. The distance between the anterolateral edge of the acromion process and the axillary nerve was measured 3 times in each of 4 positions with the arm at the side as well as at 30°, 60°, and 90° of abduction. Then, the same procedure was performed from the midlateral and the posterolatetead of using the mean distance of 5 cm.
To avoid iatrogenic axillary nerve injuries, surgeons should be aware of the safe zone based on the shortest distance from the acromion process to the axillary nerve to ensure no accidental damage to this structure instead of using the mean distance of 5 cm.
Lateral meniscal deficiency increases the pivot shift in the knee, and although meniscal allograft transplantation (MAT) is a well-established procedure with satisfactory clinical results, biomechanical studies have failed to show whether this procedure is able to restore knee kinematics.

To assess, in vivo, the kinematic effect of lateral MAT in anterior cruciate ligament-intact knees, with a particular focus on the pivot-shift maneuver.

Controlled laboratory study.

Enrolled in this study were 10 consecutive patients undergoing lateral MAT. A surgical navigation system was used to acquire and quantify the following variables anterior-posterior displacement at 30° of flexion (AP30) and 90° of flexion (AP90), varus-valgus rotation at 0° of flexion (VV0) and 30° of flexion, and dynamic laxity on the pivot-shift test, which was determined through anterior displacement of the lateral tibial compartment (APlat) and posterior acceleration of the lateral tibial compartment during tibial reduction (ACC). Data from before and after MAT were compared.

From pre- to postoperatively, there was a significant decrease in tibial translation of 2.8 mm (43%;
= .005) for AP30 and 1.9 mm (38%;
= .018) for AP90 as well as a significant difference of 3.6° (64%;
= .001) for VV0. There was also a significant pre- to postoperative reduction in the pivot shift of 7.4 mm (39%;
= .021) for APlat and 302.9 mm/s
(75%;
= .005) for ACC.

Numerous diagnostic imaging measurements related to patellar instability have been evaluated in the literature; however, little has been done to compare these findings across multiple studies.

To review the different imaging measurements used to evaluate patellar instability and to assess the prevalence of each measure and its utility in predicting instability. We focused on reliability across imaging modalities and between patients with and without patellar instability.

We performed a systematic review of the literature using the PubMed, SCOPUS, and Cochrane databases. Each database was searched for variations of the terms "patellar instability," "patellar dislocation," "trochlear dysplasia," "radiographic measures," "computed tomography," and "magnetic resonance imaging." Studies were included if they were published after May 1, 2009, and before May 1, 2019. A meta-analysis using a random effects model was performed on several measurements, comparing instabilproducible measurements for patellar tracking, and the Insall-Salvati ratio had superior reproducibility for assessing patellar height.
Following the outbreak of the COVID-19 pandemic, there was a surge of research activity to find methods/drugs to treat it. There has been drug-repurposing research focusing on traditional medicines. Concomitantly, many researchers tried to find in silico evidence for traditional medicines. There is a great increase in article publication to commensurate the new-found research interests. This situation inspired the authors to have a comprehensive understanding of the multitude of publications related to the COVID-19 pandemic with a wish to get promising drug leads.

This review article has been conceived and made as a hybrid of the review of the selected papers advertised recently and produced in the interest of the COVID-19 situation, and in silico work done by the authors. The outcome of the present review underscores a recommendation for thorough MDS analyses of the promising drug leads. The inclusion of in silico work as an addition to the review was motivated by a recently published article of Toelzer and colleagues. Allergic rhinitis (AR) is a global disease without specific treatment. Glutathione chemical Human mesenchymal stem cell- (HMSC-) derived exosomes (HMSC-exos) have been implicated for the amelioration of allergic inflammation by delivering miR-146a-5p in a mouse asthma model. However, the antiallergic activity and the underlying mechanism of HMSC-exos in AR remain unclear. The present study aimed to investigate the role of HMSC-exos in the pathogenesis of AR.

Blood specimens were collected from AR patients and healthy donators for investigation. HMSC and CD4
T cells were used in the present study. Flow cytometry was used to characterize the population of Type 1 helper T (Th1) and Th2 cells. Specific siRNA and overexpressed plasmids were designed to silence or overexpress the expressions of miR-146a-5p and SERPINB2. Luciferase reporter assay was adopted to explore the binding site of miR-146a-5p and SERPINB2. Quantitative real-time PCR and immunoblots were performed to estimate the expression of target genes.

The population of Th2 cells was significantly elevated in AR patients as compared with that in healthy donators. HMSC-exos could decrease the expression of SERPINB2 and the differentiation of Th2 cells. miR-146a-5p in HMSC-exos exhibited consistent effects and lowered the expression of SERPINB2 by binding on its 3'UTR region. Moreover, the differentiation of Th2 cells was promoted by SERPINB2 that could be reversed by HMSC-exos. Additionally, the miR-146a-5p expression was negatively associated with the SERPINB2 expression in the serum of AR patients.

HMSC-exos could inhibit the differentiation of Th2 cells via the regulation of the miR-146a-5p/SERPINB2 pathway. miR-146a-5p and SERPINB2 could be applied as potential targets for AR treatment.
