We investigated the impact of CXCR4 or CCR5 receptor antagonism on anxiety-related effects of the synthetic cathinone 3,4-methylenedioxypyrovalerone (MDPV) in the elevated zero-maze (EZM) assay. Rats exposed to a 4-day MDPV binge dosing paradigm and tested 24 or 72 h post-treatment spent more time in the open compartment at the 24-h time point but less time at the 72-h post-binge time point. Daily administration of AMD 3100, a CXCR4 antagonist (10 mg/kg), or maraviroc, a CCR5 antagonist (2.5 mg/kg), during MDPV treatment inhibited the MDPV-induced increase in time spent in the open compartment. Neither antagonist affected the MDPV-induced reduction in time spent in the open compartment at the 72-h post-binge time point. Cocaine, administered in the same paradigm as MDPV, did not increase time spent in the open compartment 24-h post-binge, suggesting specificity to MDPV. The present results identify a surprising anxiolytic-like effect of MDPV 24 h after cessation of repeated exposure that is sensitive to chemokine CXCR4 and CCR5 receptor activity.

This study was undertaken to analyze the clinical results and complication rate of patients undergoing outpatient total joint arthroplasty by a single orthopedic group. All surgeries were performed in the practice-owned ambulatory surgery center (ASC).

All patients indicated for outpatient total joint arthroplasty from 2016-2019 with complete pre and post-operative patient reported outcomes were enrolled in the study including hip, knee and partial knee replacements. Patient reported outcomes including HOOS, KOOS and VR-12 were collected at six months. Patient complication and satisfaction data was also collected.

There were 1007 patients enrolled in the study. At six months, THA HOOS and VR-12 scores improved to 82.2 and 54.5/45. TKA KOOS and VR-12 scores improved to an average of 74.3 and 54.0/43.6. At six months, UKA scores improved to an average of 73.6 and 55.1/41.2. All HOOS, KOOS and VR-12 PCS scores improvements were statistically significant (p<0.001) and met MCID thresholds. A separate cohort of 1898 regionally tracked cases with comprehensive global complication data exhibited 111 complications (unplanned post-operative events generating a medical expense) including manipulation 13 (0.68%), DVT/PE 4 (0.2%), medical 45 (2.4%), wound 8 (0.4%), infection 8 (0.4%). These parameters also showed the highest values for the SD of D. The trochlear sulcus angle was found to be the only parameter with high mean deviation (D≥5mm/5°) with D being 5.67±3.23°.

Cross-sectional data were analyzed from 5575 people (15-69 years) who participated in the 2019 Bhutan STEPS survey. The proportion of mild MDD was 12.3% and moderate to severe MDD 2.0%, and the prevalence of mild GAD was 7.4% and moderate to severe GAD 1.7%. In adjusted ordinal logistic regression analysis, alcohol family problem (Adjusted Odds Ratio-AOR 2.23, 95% Confidence Interval-CI 1.35-3.68), suicidal behaviour (AOR 7.69, 95% CI 3.69-6.01), passive smoking (AOR 7.69, 95% CI 3.69-6.01), heart attack, angina or stroke (AOR 3.32, 95% CI 2.13-5.79), and current tobacco use (AOR 1.74, 95% CI 1.38-2.19) were positively, and middle-aged (30-44 years) (AOR 0.77, 95% CI 0.60-1.00) and male sex (AOR 0.47, 95% CI 0.38-0.58) were negatively associated with mild and moderate-severe MDD. Medium wealth status (AOR 1.82, 95% CI 1.18, 2.82), alcohol family problem (AOR 2.36, 95% CI 1.29-4.31), suicidal behaviour (AOR 6.11, 95% CI 2.75-13.59), and heart attack, angina or stroke (AOR 3.18, 95% CI 1.61-6.29), were positively, and male sex (AOR 0.59, 95% CI 0.44-0.758) was negatively associated with mild and moderate-severe GAD. Our patient is a 30-year-old Filipino female admitted due to mutism, motor, and behavioral changes. Laboratory tests, cranial magnetic resonance imaging, electroencephalogram and cerebrospinal fluid analysis were unremarkable. The patient was diagnosed with stuporous catatonia and was given psychiatric medications, without significant improvement. The patient underwent 10 sessions of rTMS and showed marked improvement of symptoms. This highlights rTMS as a treatment option for catatonic patients that do not respond to pharmacotherapy or in situations where ECT is contraindicated or not available.
Caffeine has been purported to have anxiogenic and panicogenic properties, specifically salient in patients with panic disorder (PD). However, compilations of the magnitude of the effect of caffeine on anxiety and panic attacks are lacking and potential dose-response relationships have not been examined.

In the present systematic review and meta-analysis, we aimed to examine the acute effects of placebo-controlled caffeine challenge on occurrence of panic attacks and subjective anxiety in patients with PD and healthy controls (HC), including dose-response relationships.

Systematic searches were performed in six databases. We included blinded placebo-controlled studies of acute caffeine challenge on panic attacks and/or subjective anxiety in adult patients with PD.

Of the 1893 identified articles, ten met our inclusion criteria. The 9 studies investigating panic attacks included 237 patients, of which 51.1% had a panic attack following caffeine, but none after placebo. Six of these studies compared 128informed about the panicogenic and anxiogenic effects of caffeine, with the caveat that little is known regarding smaller doses than 480 mg. Registration.
