Rheumatic heart disease (RHD) remains the leading cause of cardiac-related deaths and disability in children and young adults worldwide. In The Gambia, the RHD burden is thought to be high although no data are available and no control programme is yet implemented. We conducted a pilot study to generate baseline data on the clinical and valvular characteristics of RHD patients at first presentation, adherence to penicillin prophylaxis and the evolution of lesions over time.
All patients registered with acute rheumatic fever (ARF) or RHD at two Gambian referral hospitals were invited for a clinical review that included echocardiography. In addition, patients were interviewed about potential risk factors, disease history, and treatment adherence. All clinical and echocardiography information at first presentation and during follow-up was retrieved from medical records.
Among 255 registered RHD patients, 35 had died, 127 were examined, and 111 confirmed RHD patients were enrolled, 64% of them females. The cthe urgent implementation of an effective national RHD control programto decrease the unacceptably high mortality rate, improve case detection and management, and increase community awareness of this disease.
This study highlights for the first time in The Gambia the devastating consequences of RHD on the health of adolescents and young adults. Our findings suggest a high burden of disease that remains largely undetected and without appropriate secondary prophylaxis. Our previous study proved that Shen Qi Li Xin formula (SQLXF) improved the heart function of chronic heart failure (CHF) patients, while the action mechanism remains unclear.
H&E staining and TUNEL staining were performed to measure myocardial damages. Western blot was used to examine theexpression of proteins. Moreover, CCK-8 assay and flow cytometry were used to measure cell viability and cell apoptosis, respectively. Concentrations of ATP and ROS in cells, and mitochondrial membrane potential (MMP) were detected to estimate oxidative stress.
In vivo, we found that SQLXF improved cardiac hemodynamic parameters, reduced LDH, CK-MB and BNP production, and attenuated myocardial damages in CHF rats. Besides, SQLXF promoted mitochondrial fusion-related proteins expression and inhibited fission-related proteins expression in CHF rats and oxygen glucose deprivation/reoxygenation (OGD/R)-induced cardiac myocytes (CMs). In vitro, our data show that certain dose of SQLXF inhibited OGD/R-induced CMs apoptosis, cell viability decreasing and oxidative stress.
Overall, certain dose of SQLXF could effectively improve the cardiac function of CHF rats through inhibition of CMs apoptosis via balancing mitochondrial fission and fusion. Our data proved a novel action mechanism of SQLXF in CHF improvement, and provided a reference for clinical.

The 10th and 9th revisions of the International Statistical Classification of Diseases and Related Health Problems (ICD10 and ICD9) have been adopted worldwide as a well-recognized norm to share codes for diseases, signs and symptoms, abnormal findings, etc. The international Consortium for Clinical Characterization of COVID-19 by EHR (4CE) website stores diagnosis COVID-19 disease data using ICD10 and ICD9 codes. However, the ICD systems are difficult to decode due to their many shortcomings, which can be addressed using ontology.
An ICD ontology (ICDO) was developed to logically and scientifically represent ICD terms and their relations among different ICD terms. ICDO is also aligned with the Basic Formal Ontology (BFO) and reuses terms from existing ontologies. As a use case, the ICD10 and ICD9 diagnosis data from the 4CE website were extracted, mapped to ICDO, and analyzed using ICDO.
We have developed the ICDO to ontologize the ICD terms and relations. Different from existing disease ontologies, aloped to ontologize ICD10/10 codes and applied to study COVID-19 patient diagnosis data. Our findings showed that ICDO provides a semantic platform for more accurate detection of disease profiles.

To investigate the reproducibility, stability, internal consistency and the ability to grade malnutrition of Subjective Global Nutritional Assessment (SGNA) in outpatient children with cerebral palsy.
This was a part of a larger, cross-sectional study (ChiCTR2000033869) at the outpatient of a tertiary hospital. The recruitment and data collection of children with Cerebral Palsy aged from 1 to 18 years were from August 2020 to March 2021. The concurrent validity, inter-rater reliability, test-retest reliability and internal consistency of SGNA were tested. To analyze data, specificity, sensitivity, Kendall coefficient, Cohen's kappa coefficient, Spearman coefficient and Cronbach's α coefficient were used.
The agreement between SGNA and anthropometric data was moderate to strong (
= 0.540-0.821). The sensitivity (71.70% to 89.74%) and specificity (77.67% to 91.03%) of SGNA to identify participants with
-score ≤-2 were good. The sensitivity of SGNA to identify participants with weight for age
-score ≤-3 was poor (30.00%). The interrater reliability (
= 0.703) and test-retest reliability (
= 0.779) were good. The item of edema was with poor agreement to SGNA nutritional grades (
= 0.072), and after deleting it from SGNA, the Cronbach's α coefficient of SGNA increased from 0.736 to 0.871.

The study explores the protective role of the peripheral serum of limb remote ischemic postconditioning (LRIP) in reducing the reactive oxygen species (ROS) levels and neutrophil activation, which are responsible for the deleterious reperfusion injury.

The purpose of this service evaluation was to determine the emergence of telehealth in orthotic services across the UK in response to COVID-19. A survey exploring telehealth use was distributed online to orthotists approximately 6 months after the first peak of COVID-19 in the UK. It gathered information on telehealth prevalence, allocated appointment length and waiting times, clinician access to technology and clinicians' opinions on the efficacy of telehealth. The survey received 77 responses with over 90% of respondents reporting using telehealth. Most reported that they expected telehealth to remain part of the service, post COVID-19. Thematic analysis produced two main themes the impact of COVID-19 and challenges still to overcome. Findings suggest that the pandemic has resulted in a backlog of patients waiting for an orthotic appointment, with services currently understaffed and lacking resources. For telehealth to be effective orthotists must have access to appropriate technology and training on how to use telehealth platforms, be provided with appropriate guidance on which patients are appropriate for telehealth consultations and given appropriate appointment times to enable safe and effective care.Purpose A single bout of exercise enhances activity within the cortical networks that support executive function. It is, however, unclear whether exercise improves each core component of executive function and for how long a putative benefit might persist. Method In Experiment 1, participants completed 20-min of aerobic exercise (via cycle ergometer) and cognitive flexibility-a core component of executive function-was examined pre-exercise, and at immediate, 30- and 60-min post-exercise assessments. Experiment 2 entailed a non-exercise control (i.e., participants sat on the ergometer without exercising) involving the same timeline of cognitive flexibility assessment. Cognitive flexibility was measured via stimulus-driven (SD) and minimally delayed (MD) saccades arranged in an AABB paradigm. selleck inhibitor SD and MD saccades require a response at target onset and after target offset, respectively, with the latter requiring executive control. Work has shown that reaction times for a SD saccade preceded by a MD saccade are longer than when a SD saccade is preceded by its same task-type, whereas the converse switch does not influence performance (i.e., the unidirectional switch-cost). Results Experiment 1 showed a unidirectional switch-cost at each assessment; however, the switch-cost magnitude was decreased at immediate and 30-min assessments compared to the pre- and 60-min assessments. In contrast, Experiment 2 did not elicit a change in switch-cost magnitude across the different assessments. Discussion/Conclusion Thus, a single-bout of exercise benefitted the cognitive flexibility component of executive function in the immediate and 30-min post-exercise assessments.
