Sixty subjects with β-TM referred to the Thalassemia Clinic of the Iranian Blood Transfusion Organization (IBTO) were included in our study. The blood samples were collected for laboratory measurements. The carotid intima-media thickness (CIMT), was measured by ultrasonography, and ankle-brachial index (ABI) was calculated. The multivariate linear analysis was performed to determine the appropriate indicators of subclinical atherosclerosis in β-TM. There was no significant difference in baseline characteristics between the study groups. In multivariate linear analysis, age and systolic blood pressure (SBP) were negatively associated with inverse-transformed CIMT [unstandardized β coefficient (B) -0.024, 95% confidence interval (95% CI) -0.032- -0.010, p  less then  0.001; B -0.009, 95% CI -0.017- -0.001, p 0.031, respectively]. There was also a significant correlation between the serum level of high-density lipoprotein cholesterol (HDL-C) and insulin with higher ABI, after adjustment for confounding variables (B 0.003; 95% CI 0.000-0.005; p = 0.030, and B 0.004, 95% CI 0.000-0.009, p = 0.037, respectively). Our results show that advancing age and increased SBP, HDL-C and insulin, associated with higher CIMT or ABI, are appropriate indicators of subclinical atherosclerosis in β-TM patients.Background Assessing clinical progression in amyotrophic lateral sclerosis (ALS) remains a challenge. We evaluated the validity and predictive capabilities of the King's and Milano-Torino Staging (MiToS) systems in a cohort of patients with ALS to demonstrate their benefit in clinical practice.Methodology A cohort study was performed by including ALS incident cases in a referral center from 2007 to 2016. The staging systems were determined at time of diagnosis and follow-up. The standardized median times to reach each stage were computed. A multi-state model in the framework of the Cox model evaluated the predictive value of measurements. The survival C-statistic was reported as a measure of prediction ability.Results Overall, 298 incident cases were included. The King's and MiToS systems described a progressive increase in the risk of dying with each elapsed stage. However, a lower resolution for late disease description for the King's system was observed, and late stages overlapped for the MiToS system. Slight variations in the staging systems appeared to improve performance based on validity and prediction abilities (i) in the King's (C-statistic = 0.783), by adding a new stage involving the need for both gastrostomy and NIV (ii) in the MiToS (C-statistic = 0.792), by merging stage 3 and stage 4 into a single stage 3.Conclusion Both King's and MiToS are valid systems but have certain limitations. Variations in the staging systems may provide a more suitable framework for describing progression and survival. Further research is needed to evaluate the variations in the staging systems.
To demonstrate the utilisation of a risk assessment protocol designed to prioritise elective neurosurgical patients against the risks of COVID-19. This tool can be applied to all other surgical specialties.

Prospective case series of 166 patients.

Single-centre tertiary neurosurgical department.

All patients awaiting an elective neurosurgical procedure were included in this study. All emergency or life-threatening neurosurgical pathologies affecting patients were excluded.

The risk assessment tool identified patients with progressive neurology and stratified need for surgery against risk of harm during the COVID-19 pandemic.

Using our risk stratification tool, 6.6% patients required expedited surgery and a further 11.4% patients were removed completely from the waiting list. The majority of patients 47%, required surgery within 3months.

This simple tool encourages surgical departments to establish contact with patients during COVID-19. The clinician acquires up-to-date information regarding patient symptomatology and subsequently determines surgical priority, a timescale required for surgery and overall uses of NHS resources efficiently. We recommend the use of this tool for all neurosurgical departments, with a wider application to other surgical specialties during the ongoing pressures of elective backlogs secondary to the persistent COVID-19 pandemic.

Variants within the stalk domain of KIF5A are relatively rare. We describe a family of three patients with a complex HSP phenotype and a likely pathogenic KIF5A stalk variant. More family members were reported to have walking difficulties. When reviewing the literature on KIF5A stalk variants, we found 22 other cases. The phenotypes varied with most cases having (complex) HSP/CMT2 or ALS. Symptom onset varied from childhood to adulthood and common additional symptoms for HSP are involvement of the upper limbs, sensorimotor polyneuropathy, and foot deformities. We conclude that KIF5A variants lead to a broad clinical spectrum of disease. Phenotype distribution according to variants in specific domains occurs often in the motor and tail domain but are not definite. However, variants in the stalk domain are not bound to a specific phenotype.Salvia sharifii Rech. & Esfand is a folkloric endemic medicinal plant which is found in south of Iran. Phytochemical screening and antimicrobial activities from the aerial parts of S. sharifii were investigated. The essential oil was extracted by hydro-distillation using Clevenger type apparatus and analyzed using GC-MS. The main constituents were, linalool (20.74%), spathulenol (7.98%), caryophyllene oxide (4.77%), isopentyl isovalerate (4.48%), hexyl isovalerate (4.22%), α -gurjunene (4.16%), α -cadinol (3.77%), γ-terpinene (3.52%), terpinen-4-ol (3.37%) and sabinene (3.13). Total phenolic content was 31.39 mg gallic acid equivalents/g dry weight, and IC50 values in the radical scavenging assay was 1423 mg/mL. The in vitro antimicrobial activity of the essential oil of S. sharifii were studied by disc diffusion method. The results showed that the oil exhibited good antimicrobial activity against all medically important pathogens compare to standard antibiotics.Sludge-based activated carbons (SACs) prepared from sewage sludge and corn straw, were modified by ferric nitrate, and the unmodified SAC and modified SAC were used as the adsorbing agent to treat the landfill leachate, the elimination capacity for chemical oxygen demand (COD) and organic matter in leachate were studied. Based on this, the physicochemical properties of SACs and the components changes in leachate were analyzed and characterized by X-ray photoelectron spectroscopy and three-dimensional fluorescence spectroscopy. The results showed that under optimal experimental conditions, the elimination capacities of SAC372 for COD, biological oxygen demand over 5 days, and NH4+-N in the leachate were 81.58%, 54.73%, and 69.08%, respectively; while the adsorption capacities of modified SAC for these three substances were 86.25%, 63.51%, and 79.15%, respectively. The ferric nitrate modification improved the ability of SAC to eliminate COD and organic matter from leachate slightly, and made the adsorption occurred easily. The adsorption process of unmodified SAC was dominated by multi-layer adsorption, while the adsorption process of modified SAC was dominated by monolayer adsorption. The mass fraction of Fe (2p) in modified SAC remarkably increased, from 0.70% to 26.01%, organic functional groups certain phase of Fe oxides with different valence states were generated in SAC, which provided a substrate for iron-carbon micro electrolysis. After adsorbed by unmodified SAC and modified SAC adsorption, the total fluorescence intensity of in the leachate increased by 17.01% and 116.84%, respectively. Both two SACs could decompose the humic acid-like substances into aromatic protein organic compounds, and modified SAC could further decompose the soluble microbial byproduct-like substances.The β-thalassemias and sickle cell disorders pose a considerable health burden in India. Of the more than 10,000 annual births of children with a severe hemoglobinopathy, only around 10.0% are managed optimally. Thus, genetic counseling and prenatal diagnosis (PND) is a valid option for a large and diverse country. Our center was one of the first to initiate PND and we present our experience over 30 years to evaluate the impact of awareness in changing the trends of PND of hemoglobinopathies. Both second and first-trimester diagnoses were undertaken by fetoscopy/cordocentesis and globin biosynthesis/high-performance liquid chromatography (HPLC) analysis of fetal blood and chorionic villus sampling (CVS) and DNA analysis. Over 30 years, 3478 couples (first trimester 2475; second trimester 1003) from all over India were offered PND. The number of couples coming in the first trimester increased significantly over each decade and couples coming prospectively increased from 2.5 to 18.4%. A cost-effective stepwise approach was used for molecular analysis. Eight hundred and one fetuses (23.0%) were affected and all except three couples opted for termination of these pregnancies. Genetic counseling and PND is the only way to reduce the burden of disease. With awareness, there was a shift from second trimester to first trimester PND over each decade, with an increasing number of couples coming during the first pregnancy. There are only 15 to 20 centers in India offering PND. We have compared our study with other reports on PND from different regions in India.Chinese international students (CIS) in American universities encounter serious mental health issues, and they also underutilize campus mental health care services. The current study examined several factors which result in this phenomenon, such as Chinese face concerns, self-stigma of seeking help for mental health services, as well as English proficiency and language discrimination. EGFR tumor A survey with 433 CIS in multiple U.S. universities was conducted to analyze the relationships between these factors above, help-seeking intentions, and mental health problems (depression and anxiety).
