Bronchogenic cysts are congenital lesions found in the mediastinum, particularly the posterior-superior area. The current study aims to report a rare case of a subcutaneous bronchogenic cyst in the chest wall.

A 41-year-old patient presented with a swelling of the chest wall. The mass had been present since birth. On examination, there was a large soft, round mass over the sternum subcutaneously. It was a fixed, non-flatulence, non-pulsatile, and non-tender mass.

Usually, the condition develops between the fifth and sixteenth weeks of gestation, when the primordial intestine separates into two parts dorsal, which gives rise to the esophagus, and ventral, which gives rise to the pulmonary bud and tracheobronchial tree. As a result, the cyst is an ectopic lung bud that may or may not be connected to the tracheobronchial tree but lacks mesenchymal tissue.

It is known that ChRCC represents a heterogeneous group of neoplasms demonstrating variable, yet distinctive morphologic and genetic profiles. In this international multi-institutional study, we aimed to assess the impact of histologic diversity in ChRCC (classic/eosinophilic versus rare subtypes) on survival outcome. This is an international multi-institutional matched case-control study including 14 institutions, examining the impact of histologic subtypes of ChRCC on survival outcome. The study group (cases) included 89 rare subtypes of ChRCC. The control group consisted of 70 cases of ChRCC including classic and eosinophilic features, age- and tumor size-matched. Most of the rare subtypes were adenomatoid cystic/pigmented ChRCC (66/89, 74.2%), followed by multicystic ChRCC (10/89, 11.2%), and papillary ChRCC (9/89, 10.1%). In the control group, there were in determining the clinical behavior of the tumor.Medical image segmentation is fundamental for computer-aided diagnosis or surgery. Various attention modules are proposed to improve segmentation results, which exist some limitations for medical image segmentation, such as large computations, weak framework applicability, etc. To solve the problems, we propose a new attention module named FGAM, short for Feature Guided Attention Module, which is a simple but pluggable and effective module for medical image segmentation. The FGAM tries to dig out the feature representation ability in the encoder and decoder features. Specifically, the decoder shallow layer always contains abundant information, which is taken as a queryable feature dictionary in the FGAM. The module contains a parameter-free activator and can be deleted after various encoder-decoder networks' training. The efficacy of the FGAM is proved on various encoder-decoder models based on five datasets, including four publicly available datasets and one in-house dataset.
Valvular heart disease (VHD) is an important cause of morbidity and mortality. Echocardiography is the reference standard for VHD diagnosis but is not universally accessible. Manual cardiac auscultation is inadequate for screening VHD. Many machine learning models using heart sounds acquired with an electronic stethoscope may improve the accuracy of VHD diagnosis. We aimed to develop an accurate sound classification model for VHD diagnosis.

A new large stethoscope sound dataset containing 10,366 heart sounds divided into ten categories (nine VHD and one healthy) were prospectively collected. We developed a handcrafted learning model that comprised multilevel feature extraction based on a dual symmetric tree pattern (DSTP) and multilevel discrete wavelet transform (DWT), feature selection, and classification. The multilevel DWT was used to create subbands to extract features at both high and low levels. Then, iterative neighborhood component analysis was used to select the most discriminative 512 features from among the extracted features in the generated feature vector. In the classification phase, a support vector machine (SVM) was used with 10-fold cross-validation (CV) and leave-one-subject-out (LOSO) CV.

Our proposed DSTP-based model attained 99.58% and 99.84% classification accuracies using SVM classifier with 10-fold CV and LOSO CV, respectively.

Transient ischemic attack (TIA) serves a precursor for an acute ischemic stroke (AIS); however, not all TIA patients harbor the same risk for subsequent AIS. We aimed to investigate expediting outpatient management of low-risk TIA patients (ABCD Giles and Rothwell, 2007 score≤3) via our "Fast-Track" TIA Protocol (FTTP).

A retrospective analysis was performed on patients who presented to our academic network 04/2020 - 2/2021. Patients who presented with ABCD Giles and Rothwell, 2007 scores≤3 without large vessel occlusion or flow limiting stenosis were eligible for the FTTP. These patients were discharged on dual antiplatelet therapy and statin and received prescriptions for transthoracic echo, holter monitor, LDL, and A1c along with a scheduled follow-up appointment 30 days from presentation.

182 consecutive patients were evaluated during this period, 21 (11%) were excluded from analysis due to NIHSS > 0 and/or infarct present on MRI. 35 (22%) patients qualified for FTTP and were directly discharged from the ED. Median ABCD
score was 2 for the discharge group and 4 for the admitted group. There was a significant difference with respect to age and hypertension. Additionally, the FTTP patient population were more likely to be smokers than the admitted patient population. 3 FTTP patients re-presented to the ED, but none of them suffered a symptomatic stroke.

A FTTP demonstrated feasibility and safety with low rates of re-presentation and ischemic stroke. Further research is warranted to determine an optimal patient population that can be safely managed in an outpatient setting.

The amount of freshwater runoff and the negative relationship with salinity confirmed that the river is the key factor in the transportation of microplastics to the coastal sea.The rise in the use of single-use plastics and personal protective equipment (PPE) has increased plastic waste in the marine environment. In this study, we surveyed the presence of PPE (face masks and gloves) discharged in 6 beaches along the coast of India. A total of 496 PPE were counted with an average density of 1.08 × 10-3 PPE m-2. The PPE density found was comparable to previous studies. Face masks were the most recorded type of PPE (98.39%), with gloves accounting for only 1.61% of the total. However, a significant reduction in the appearance of PPE was recorded on all six beaches, likely due to the increase in vaccination rates. The most contaminated places were the beaches with recreational activities + fishing. It has been noticed that the lack of awareness of environmental pollution and the negligence of the population and the mismanagement of municipal waste are the main causes of beach pollution by PPE. This study confirms the potential threat of PPE to terrestrial and aquatic organisms of multiple taxa in India, but further studies are needed to quantify the impact of this type of waste on marine animals.
