Background The dysregulation of renin-angiotensin-aldosterone system (RAAS) is closely related to the development of essential hypertension (EH). MicroRNAs (miRNAs) are an important regulator of RAAS. The sponge effect of circular RNAs (circRNAs) on miRNAs makes the circRNA-miRNA-mRNA axis in EH possible, however, there is currently a lack of relevant evidence.Material and Methods A circRNA-miRNA network was constructed based on the previous circRNAs microarray results. The expression of RAAS-related miRNAs and circRNAs were verified by qRT-PCR. Peripheral blood samples of 106 EH patients and 106 healthy volunteers were included in this study. GO and KEGG enrichment were performed to predict the role of candidate circRNAs in EH.Results In EH patients, RAAS-related hsa-miR-483-3p and hsa-miR-27a-3p were down-regulated, and hsa_circ_0122153 and hsa_circ_0025088 were up-regulated. The relative expression of RAAS-related circRNAs and target miRNAs showed a negative correlation (hsa_circ_0122153-hsa-miR-483-3p and hsa_circ_0025088-hsa-miR-27a-3p). Hsa_circ_0122153 or hsa_circ_0025088 combined with corresponding miRNAs and environmental factors may support the early diagnosis of EH. Hsa_circ_0122153 and hsa_circ_0025088 may participate in the regulation of aldosterone and the secretion of renin through the circRNA-miRNA-mRNA network, respectively.Conclusion Highly expressed hsa_circ_0122153 and hsa_circ_0025088 increase the risk of EH. The hsa_circ_0122153/hsa-miR-483-3p and hsa_circ_0025088/hsa-miR-27a-3p axis involving RAAS were potential EH pathways.
Little is known on the beliefs, perceptions and practices of hypertension specialists in addressing non-adherence to therapy. Therefore, a survey was undertaken amongst healthcare professionals (HCPs) managing hypertension in the European Society of Hypertension (ESH) Centres of Excellence.
Cross-sectional data were obtained between December 2020 and April 2021 using an online anonymous structured questionnaire including 26 questions/136 items, that was sent to all ESH Excellence centres.
Overall 67 from 187 centres (37.3%) responded and 200 HCPs from 30 countries answered the questionnaire. MEK inhibitor Participants (60% men) were mainly physicians (91%) and nurses (8%) from University hospitals (77%). Among physicians, 83% had >10 years professional experience. Average time dedicated to discuss medications was 1-5 min in 48% and 6-10 min in 29% of cases. Interviews with patients about adherence were the most frequently used assessment method. Chemical detection of medications in urine was available in 36% of centres. One third of physicians involved their patients regularly in treatment decisions. The most frequent methods to improve adherence included simplification of medication therapy, more frequent visits, and home blood pressure monitoring.
The level of implementation of tools to detect and improve adherence in hypertension management by HCPs in ESH excellence centres is low. Structured educational activities focussing on adherence management and access to the newest objective measures to detect non-adherence might improve these deficits.

Structured educational activities focussing on adherence management and access to the newest objective measures to detect non-adherence might improve these deficits.Auxeticity (negative Poisson's ratio) is the unique mechanical property found in an extensive variety of materials, such as metals, graphene, composites, polymers, foams, fibers, ceramics, zeolites, silicates and biological tissues. The enhanced mechanical features of the auxetic materials have motivated scientists to design, engineer and manufacture man-made auxetic materials to fully leverage their capabilities in different fields of research applications, including aeronautics, medical, protective equipments, smart sensors, filter cleaning, and so on. Atomic force microscopy (AFM) indentation is one of the most widely used methods for characterizing the mechanical properties and response of the living cells. In this contribution, we highlight main consequences of auxeticity for biosystems and provide a representative example to quantify the effect of nucleus auxeticity on the force response of the embryonic stem cells. A parametric study has been conducted on a heterogeneous stem cell to evaluate the effect of nucleus diameter, nucleus elasticity, indenter's shape and location on the force-indentation curve. The developed model has also been validated with the recently reported experimental studies available in the literature. Our results suggest that the nucleus auxeticity plays a profound role in cell mechanics especially for large size nucleus. We also report the mechanical stresses induced within the hyperelastic cell model under different loading conditions that would be quite useful in decoding the interrelations between mechanical stimuli and cellular behavior of auxetic biosystems. Finally, current and potential areas of applications of our findings for regenerative therapies, tissue engineering, 3 D/4D bioprinting, and the development of meta-biomaterials are discussed.
To assess the patterns of patient generated aerosol in the context of ophthalmic surgery and ophthalmic examinations. To inform medical teams regarding potential hazards and suggest mitigating measures.
Qualitatively, real-time time videography assessed exhalation patterns from simulated patients under different clinical scenarios using propylene glycol from an e-cigarette. Quantitatively, high-speed Schlieren imaging was performed to enable high resolution recordings analysable by MATLAB technical computing software.
Without a face mask, the standard prior to COVID 19, vapour was observed exiting through the opening in the drape over the surgical field. The amount of vapour increased when a surgical mask was worn. With a taped face mask, the amount of vapour decreased and with inclusion of a continuous suction device, the least amount of vapour was seen. These results were equivocal when the patient was supine or sitting upright. High-speed Schlieren imaging corroborated these findings and in addition showed substantial increase in airflow egress during coughing and with ill-fitting face masks.

Surgeon safety can be maintained with personal protective equipment to mitigate the increased egress of vapour from the surgical drape and taping, with or without suction is advisable, whilst meticulous hygiene around lenses is required at the time of slit lamp examination.

Do computer prompt software programs at the workplace reduce sitting time and/or increase physical activity at work?
Many products are promoted and being used in the workplace; however, their effectiveness and use are unknown or the evidence base that they work to change behavior is lacking.
We searched for relevant papers published between 2005 and 2020. The inclusion criteria were computer prompt software programs installed as behavioral change interventions; interventions implemented during work hours and delivered through a work personal computer or laptop; and measures of sedentary behavior and/or physical activity. To minimize risk of bias, three recommended best-evidence synthesis criteria were used random assignment, sample size, and external validity. Based on these criteria, articles were selected and evaluated.
Six publications met the quality threshold for review. Seven articles did not meet the quality threshold. Four of the six included publications found that computer prompt software programs decreased sedentary behavior and/or increased physical activity. Two publications reported inconsistent results.
The promising results from this systematic review indicate that there is potential for computer prompt software programs to improve the health of desk-based workers. For conclusive findings, more high-quality, scientific studies are needed.
The best-evidence publications in this review can serve as a guide in selecting and implementing computer prompt software programs at the workplace to decrease sedentary behavior and increase physical activity.

The combined anteversion theory to prevent impingement in total hip arthroplasty (THA) has been proposed. However, because stem-anteversion is influenced by the native femoral anteversion and the stem flexion/extension angle, it is often difficult to adjust stem anteversion during surgery. Therefore, the stem-first (combined anteversion) technique may be useful to adjust and achieve appropriate cup anteversion during surgery with respect to the implanted stem anteversion angle. However, the technique may adversely affect cup or stem angle accuracy and result in intra-operative bleeding, post-operative adverse events, and prolonged operative time. It is inconclusive whether either the stem-first or cup-first technique is safe or accurate. Therefore, this study assessed the accuracy and safety of stem-first THA compared to those of cup-first THA.
This prospective randomised controlled trial analysed 114 patients who were randomly divided into 2 groups (stem-first group
57, cup-first group (control group)
57). Primary outcomes included cup and stem angle, the discrepancies from the targeted angle and combined anteversion (evaluated via CT at 3 months postoperatively). Secondary outcomes included intraoperative blood loss, operative time, WOMAC, and adverse events.
There were no significant differences in age, gender, BMI or in the primary and secondary outcomes between the 2 groups.
Performing stem-first in THA did not adversely affect cup and stem angle accuracy, or result in intraoperative bleeding, prolongation of operative time, or postoperative adverse events. Thus, performing stem-first may be advantageous for achieving combined anteversion theory.
