Given that there is no safe tobacco product, youth and adults who do not use tobacco products should not start using e-cigarettes. As of April 1st, 2024, 1,061 municipalities, plus 26 states, commonwealths, and territories restrict e-cigarette use in 100% smokefree venues. Nicotine use can have an impact on learning, memory and attention and increase the likelihood of addiction to other drugs in the future. But one vape can contain the equivalent of 50 cigarettes of nicotine in the one product. They also contain a complex combination of chemicals including nicotine, nickel, tin, arsenic, chlorine and lead. These substances can be harmful to the body, with different flavourings helping to mask these poisons when they are taken into the body. Months after the release of that advisory, Yale researchers found that byproducts, called acetals, form when flavoring agents mix with solvents in the liquid.
Not only is there evidence of mislabelling of nicotine content among refills labelled as nicotine-free, but there also seems to be a history of poor labelling accuracy in nicotine-containing e-liquids [37, 38]. You’re hitting on the exact controversy, and I think it’s super important to talk about it in this way. First of all, it’s partly a socio-cultural question about nicotine. There are all kinds of appealing aspects to nicotine, I should point out, and this is also a very important point, it’s not the nicotine in cigarettes that kills people. There are even some studies showing that nicotine can help in the treatment of serious diseases, including Parkinson’s. Early [e-cigarettes], what they did was they essentially just took nicotine and they added some flavors to it. But the problem with just using nicotine is, it’s extremely harsh.
The survey also found that Minnesota youth are ill-informed about the health risks of e-cigarettes with 76.1% of 11th graders saying there is either no, slight, or a moderate risk to using e-cigarettes. 11th grade students responding to the student survey said that the top ways they got e-cigarettes included getting products from friends (72.3%), getting products at a vape shop (14.0%), and on the Internet (9.6%). Among 8th grade students, e-cigarette use nearly doubled from 2016 to 2019, and one in four 11th graders now use e-cigarettes.

The new study included a diverse group of adolescents, indicating how the text message program could be helpful for marginalized communities, said Dr. Suchitra Krishnan-Sarin, a professor of psychiatry at the Yale School of Medicine. “Text messaging is a scalable and cost-efficient approach to delivering vaping cessation treatment on a population basis,” the researchers wrote in the study. The 2023 National Youth Tobacco Survey, released in November by the US Centers for Disease Control and Prevention and the US Food and Drug Administration, found that about 2.1 million middle and high school students were e-cigarette users.
This does not necessarily mean, however, that e-cigarettes are free from hazardous effects. Indeed, studies investigating their long-term effects on human health are urgently required. In this regard, the main additional studies needed in this field are summarized in Table 3.

Whilst long-term health effects are not fully known, we do know that they generate toxic substances, some of which are known to cause cancer and some that increase the risk of heart and lung disorders.

They sent kind of traveling troupes of these nicotine Juul marketers that handed out free samples at parties, on yachts, in the Hamptons, at raves, and it just became extremely popular.
The viscosity and density of the e-liquid determine its mobility, capillary action, and delivery to the wick and coil, influencing the likelihood of a dry puff15. Sub-ohm vapes (sometimes called cloud kits) are more advanced vaping devices that are capable of outputting huge amounts of power; if you’ve ever noticed someone exhaling gigantic clouds from their kit, they were probably using one of these! Using a sub-ohm device requires a working knowledge of battery safety and Ohm’s Law, so it’s best to do some reading before investing in one. “India should be taking lessons from the West where vaping has become an epidemic. Claims that e-cigarettes aid cessation is a big lie, rather they are gateway devices. Moreover, we just don’t know the long-term effects of vaping,” said Dr. Bhavna Barmi, a senior clinical psychologist.
Thirty-two states, the District of Columbia, and two territories have imposed a tax on e-cigarettes as of June 15, 2023. Cessation strategies should be based on the best available evidence of efficacy, to go with other tobacco control measures and subject to monitoring and evaluation. Based on the current evidence, it is not recommended that governments permit sale of e-cigarettes as consumer products in pursuit of a cessation objective. While they may help some people stop smoking, vaping products do not have FDA approval as a tool for quitting. E-cigarettes may even keep people from trying proven methods of quitting smoking.
This legislation, which applies to all states, raises the minimum age of sale for all tobacco products— including e-cigarette—to 21. The American Lung Association is very concerned that we are at risk of losing another generation to tobacco-caused diseases as the result of e-cigarettes. The Lung Association remains extremely troubled about the rapid increase of youth using these products and has repeatedly called upon the Food and Drug Administration (FDA) to increase their oversight and scrutiny of these products to protect kids. Susceptibility to smoking or non-combustible nicotine use is also influenced by national tobacco control policies. In Europe, Finland has been among the countries with most advanced tobacco control regulations [31]. Still, there is a large difference in smoking and snus use between students in academically and non-academically oriented education [7].
And that ran right into some very strange regulatory moments that you unpack in the book. I’m curious if you can tell that story and kind of help me understand how they got through it. I’m really interested in that arc and sort of pulling apart the bits and pieces of that, but what struck me about your book is that you don’t start with Juul at all. You start with Altria, which is the company that was known as Philip Morris, that made Marlboro, and you start with them in sort of the very late ’90s, early 2000s. And it seems like what the entire cigarette industry was struggling with was, we’re going to be a regulated industry. So they did all kinds of just really organic research and ended up settling on benzoic acid as the organic acid that was the most satisfying.
Separate studies have shown that newer e-cigarettes may deliver higher doses of nicotine more efficiently, which could explain the recent findings. The investigators acknowledged that more frequent use of e-cigarettes may also reflect greater determination to quit smoking combustible cigarettes. They noted that their study did not evaluate the risks of youth e-cigarette use. “Our findings are consistent with earlier evidence that sufficient nicotine replacement can be helpful for quitting smoking. Recent guidance from the Center for Tobacco Products indicates the importance of switching completely away from combustible cigarettes for those who are also using e-cigarettes,” she added. The investigators revealed that daily vs nondaily use of e-cigarettes was linked to higher overall rates of quitting combustible cigarettes (12.8% vs 6.1%).

E-cigarettes next evolved into “pod mods” with e-liquid sold in disposable pods. In 2015, the emergence of the brand JUUL took pod mods to the next level with high tech design, high-nicotine delivery, and youth- appealing marketing that hooked a new generation to nicotine. Vaping is the term often used to describe the act of using an electronic cigarette. E-cigarettes are battery-operated devices that heat a liquid solution — usually, but not always, containing nicotine — turning it into a vapor that can be inhaled. If the base nicotine mixture is not palatable, many flavors, such as mint, apple and others, can make vaping attractive, especially to adolescents.
The most important points to know are that the long-term health effects of e-cigarettes are still unknown, and all tobacco products, including e-cigarettes, can pose health risks to the user. The tobacco industry has co-opted the term harm reduction to further its own interests of reduced tobacco product regulation and expand its user base. Short-term studies haven't found evidence that exposure to secondhand aerosol from e-cigarettes hurts lung function, with one notable exception. Researchers found that people who were around vaping aerosol showed increases in serum cotinine, which is a marker that someone was exposed to nicotine (an ingredient often found in e-cigarettes).
Half of high school seniors reported having tried e-cigarettes at least once. The growing popularity of e-cigarettes among Minnesota’s youth caused the first rise in overall tobacco use in over two decades. In that same vein, 5.9 million people began vaping, bringing the total use of tobacco products among those 12 and older to 64.4 million. More people began vaping in 2023 than initiated alcohol or illicit substances. Pediatricians are particularly concerned about the threats to child and adolescent health posed by e-cigarettes, including heightened risk of nicotine-dependence and other health concerns related to vaping. The American Academy of Pediatrics has joined with other organizations to urge the U.S.
The CDC advises people who have asthma to avoid tobacco smoke, yet Minnesota teens who have asthma are more likely to breathe someone else’s tobacco smoke or e-cigarette aerosol than teens who do not have asthma. Minnesota’s teens who have asthma are less likely to be protected by rules against smoking in the family’s homes or vehicles. “Even with the current relatively low use of e-cigarettes among adults – 3.7 percent – health care costs are already substantial, and likely to increase in the future if youth continue to use this product,” said Max.

Interestingly, the battery output voltage also seems to have an impact on the cytotoxicity of the aerosol vapours, with e-liquids from a higher battery output voltage showing more toxicity to A549 cells [30]. All this evidence would suggest that e-cigarettes are potentially less harmful than conventional cigarettes (Fig. 2) [11, 14, 22, 24, 27,28,29]. Importantly, however, most of these studies have investigated only short-term effects [10, 14, 15, 22, 27,28,29, 31, 32], and the long-term effects of e-cigarette consumption on human health are still unclear and require further study.
Each e-cigarette was inserted into a sealed chamber attached to a lab-built device that drew air through the e-cigarette for eight seconds at a time with a resting period of 15 or 30 second between each draw. I think that there was another way that they could have done it that would have involved less flashy marketing and that would have been tailored toward adult smokers, which they claimed that it was. And now whether or not there was the intent there, I think that’s a pretty big remaining question that is going to be litigated in courtrooms across the country, is whether or not they intended to market to kids. But at the time, the management was very circumspect about the long-term viability of the e-cigarette market. They thought that regulators were not going to tolerate another nicotine product on the market without going through lots and lots of regulatory hurdles. They believed that this market would not last for long and that they didn’t really need to pour tons and tons of money into it in order to innovate.
Enrollees in the program receive interactive daily text messages tailored to their sign-up date or their target quit date if they set one. Those without a quit date receive messages for at least one month. Program users who set a quit date (which they can change) receive messages for at least one week prior to the quit date and for at least two months following the quit date. Friday’s action is part of a sweeping FDA review intended to bring scientific scrutiny to the multibillion-dollar vaping market after years of regulatory delays. Currently the U.S. market includes thousands of fruit- and candy-flavored vapes that are technically illegal but are widely available in convenience stores, gas stations and vape shops.

There's a lot that's still unknown about the harms of secondhand vaping, but research suggests that bystanders who breathe in the aerosol might be exposed to toxins. It's unclear how exposure to these toxins might impact health, but it's possible that repeated exposure could impact lung function in the long term. The U.S. government has taken steps to restrict most flavored vaping products, effective in 2020.
E-cigarettes are rapidly increasing in popularity, especially among youth. However, products like cigarettes and smokeless tobacco continue to have a stronger hold in rural areas due to decades of targeted marketing by the tobacco industry, which integrated tobacco use into rural culture. In the first study, researchers looked at the impact of e-cigarettes on lipids and glucose in the blood. They recruited 476 healthy human participants without cardiovascular disease who were either nonsmokers, e-cigarette-only smokers, smokers of e-cigarettes and tobacco cigarettes and those who smoked tobacco cigarettes only.
They're also sometimes called JUULs, "vapes" and "vape pens." E-cigarettes are the most commonly used tobacco products among kids—and it's become an epidemic. While much remains to be determined about the lasting health consequences of e-cigarettes, there’s evolving evidence about the health risks of e-cigarettes on the lungs—including irreversible lung damage and lung disease. The prevalence of cigarette smoking in the United States has been declining for over 50 years among all racial/ethnic groups. However, this progress could be stalled or even reversed by the increasing popularity of electronic cigarettes (e-cigarettes). Hispanics have experienced tobacco-related health disparities resulting from early age of cigarette smoking initiation and less success in quitting smoking. The effects of e-cigarettes on tobacco-related health disparities among Hispanics have not been well described.
From a public health standpoint, that’s fantastic, because then they wouldn’t be smoking cigarettes. For the tobacco industry, that’s a complete disaster, because that means if they do not have the next generation of users, they do not have an industry that has a long life. So for me, the controversial issue is less about whether adult smokers should be using e-cigarettes to quit smoking, and more about the continuation of nicotine addiction that continues to addict new generations of users. It became popular among 20-year-olds, and among teenagers as well, middle school and high school students.
Power levels that produce aerosol beyond the ability of the wick to resupply the liquid to the coil may result in overheating of the atomizer coil and consequently overheating of the e-liquid10,11. Different types of wicking material, varying in size and shape, have been used in e-cigarettes3,13. Silica was commonly the first material to be used as a wick, followed by cotton and ceramic3,13,14,15.
Such policies are critical in the absence of strong federal regulation of e-cigarettes. As of March 31, 2023, seven states and an estimated 388 counties, cities, towns, and tribes have restricted the sale of flavored tobacco products. However, a single e-cigarette can be harmful to the body’s blood vessels — even when the vapor is entirely nicotine-free — according to a new study by researchers in the Perelman School of Medicine at the University of Pennsylvania. The cytotoxic and pro-inflammatory effects of different e-cigarette flavouring chemicals were also tested on two human monocytic cell lines—mono mac 6 (MM6) and U937 [86]. Also, all the tested flavours produced significant levels of ROS in a cell-free ROS production assay. Finally, diacetyl, pentanedione, O-vanillin, maltol, coumarin, and CAD induced significant IL-8 secretion from MM6 and U937 monocytes [86].
