Aponeurotic ptosis, the most common cause of acquired ptosis in elderly adults, results from disinsertion or dehiscence of the levator aponeurosis from the tarsal plate. Müller's Muscle is located below the levator aponeurosis, and its main function is to contribute to a few millimeter in upper eyelid elevation. In recent years, studies have shown that Müller's Muscle plays a more important role in the eye-opening process than previously reported. There are few reports on the changes of smooth muscle component with age in Müller's Muscle. As far as we know, it has not been written up previously. Through histological study on Müller's Muscle of Chinese cadaver specimens, we found that Müller's Muscle contained obvious smooth muscle components above the upper edge of the tarsus in the specimen of a child. However, in elderly specimens, we observed that the smooth muscle components decreased gradually even faded away from originated inferior branch of levator superioris muscle to upper margin of tarsus. We hypothesized that the number of smooth muscle components in Müller's Muscle gradually decreases, with the increase of age, which further affects the function of Müller's Muscle in assisting levator muscle, and further causes or aggravates ptosis. Our hypothesis needs to be further verified by more autopsy histological studies of different ages and genders, so as to provide a new idea for the clinical diagnosis and treatment of ptosis.
To assess the cerebral hemodynamic data associated with transient cerebral hemodynamic disturbance (TCHD), including cerebral hyperperfusion syndrome (CHS) and watershed shift ischemia (WSI), by whole-brain (WB) computed tomography perfusion (CTP) before and after revascularization for ischemic moyamoya disease.

A total of 115 consecutive patients with ischemic moyamoya disease underwent revascularization. All patients underwent WB-CTP 24 hours before operation and on the day of onset of TCHD and 6 months after revascularization. The volumes of delay time (DT) >3 seconds and mismatch and relative cerebral blood flow <30% were calculated in 3 time points.

Of the 115 patients, 18 115 had TCHD, comprising 10 with CHS and 8 with WSI. selleckchem Compared with the brain volume of DT >3 seconds before revascularization, the volume decreased significantly (P < 0.05) on the day of CHS. The volume of mismatch in 3 time points indicated no significant differences (P > 0.05). The volume of relative cerebral blood flow <30% showed obvious differences of significance among 10 patients with CHS (P < 0.05) at 3 time points. In the WSI group, the volume of DT >3 seconds, mismatch, and DT >3 seconds showed significant differences, relatively (P < 0.05), at 3 time points. The mean MP abundance (33 particles L-1) detected herein was higher than that in the previously conducted studies. Fragment particles (37.95%) were determined to be the dominant MP type, and the predominant MP color was blue (75.28%). Different pathogenic variants can lead to Pitt-Hopkins syndrome. The most common are large deletions at 18q21 encompassing the TCF4 gene and frameshifting/nonsense single nucleotide variants. However, variants in noncoding regions can also lead to Pitt-Hopkins syndrome by disrupting the normal pre-mRNA splicing machinery. Here we describe three patients with Pitt-Hopkins syndrome caused by a large deletion in chromosome 18, a nonsense variant, and a novel variant located in intron 11 of TCF4 c.922+5G > A. Using RT-PCR analysis and minigene splicing assay we showed that this intronic variant leads to exon 11 skipping resulting in a formation of a premature stop codon. To our knowledge, this is the first functional annotation of a splicing variant in Pitt-Hopkins syndrome.
A long diagnostic delay in multiple sclerosis (MS) reduces the chance of early access to disease modifying therapies with potential to impact on disease trajectory. link3 The aims of this study were to determine the delay in diagnosis of MS and its associated factors in Iran.

Our study was conducted on 536 MS patients who were selected from Multiple sclerosis registry database and neurology clinics in southern part of Iran. Data were collected via a face-to-face interview administered questionnaire. A multiple linear regression model was used to control for the effect of potential confounding factors on the delay of MS diagnosis.

The mean of diagnostic delay was 7.0 months. Single patients were diagnosed on average 1.33 months later compared with those who were married (P = 0.007). Those who were registered with a family physician reported an average of 1.42 months longer delay in diagnosis than others (P = 0.004). For each year of older age at diagnosis, the delay was 1.04 months longer (P < 0.001). On the other hand, those who had migraine were diagnosed 1.58 months earlier than others (P < 0.001). Participants with a vision problem as the initial symptom were diagnosed 1.48 months earlier than those with sensory symptoms (P < 0.001), and 1.28 months earlier than those with headache/dizziness (P = 0.093). It was also suggested that the diagnostic delay was shortened over the study period (on average 1.06 months per year, P < 0.001).

This study aims to investigate the genetic predisposition of haptoglobin (Hp) genotype as a predictor for cerebral vasospasm (CV) after acute subarachnoid hemorrhage (aSAH) in the Egyptian population. This permits CV risk factors stratification of patients with aSAH. Hence, it will guide the treatment plan and intensive monitoring for those patients.

The study was carried out at El Matareya Teaching Hospital, Cairo, Egypt. We studied 50 patients with aSAH who were prospectively recruited and followed up by transcranial Doppler (TCD) examination for 14 days following aneurysmal rupture to early detect hemodynamic changes associated with CV and also the occurrence of delayed cerebral ischemia (DCI) as a secondary outcome. In this study, we attempted to analyze Hp genotyping as a potential predictor of CV and DCI during the acute phase of aneurysmal SAH.
