This article presents narrative medicine as a new field of medical humanities. Narrative medicine has become influential over the world as well as in Denmark in recent years. The injuries were remarkably similar with jagged soft tissue lacerations and soft tissue parts of questionable vitality. These cases and their treatment are presented to show examples of the handling of soft tissue injuries of the face in an emergency department setting and to show, that very conservative debridement of these injuries is the recommended approach.Hereditary pancreatitis (HP) is an autosomal dominant disease with 80% penetrance. HP is characterised by the debut of recurrent acute pancreatitis episodes during childhood with gradual progression to chronic pancreatitis. Patients with phenotypic HP have a significantly increased lifetime risk of developing pancreatic ductal adenocarcinoma (PDAC). Patients with HP represent a rare but important group of high-risk individuals in need of early diagnosis and screening for potential PDAC. The aim of this review is to provide an overview of the epidemiology, genetics and clinical aspects of HP.The treatment of malignant bone tumours in children has improved significantly over the past 50 years. Previously, the only curative treatment was amputation, but today the surgical treatment has changed towards limb salvage surgery. However, there is not consensus regarding choice of surgical procedure, and it is mainly based upon the surgeon's previous experience. This review describes four reconstructive methods rotationplasty, free vascularised fibula graft, bone transport and expandable endoprosthesis. All four methods are suitable surgical options with satisfactory functional outcome, though they vary in complications and revision rates.Drought is one of the main limiting factors affecting tea plant yield and quality. Previous studies have reported that K+ (potassium) application significantly alleviated drought-induced damage in tea plants. However, the intrinsic mechanisms underlying K+-alleviated drought stress are still obscure. In our study, two contrasting varieties, Taicha12 (drought tolerant) and Fuyun6 (drought sensitive), were used to investigate the intrinsic mechanisms behind K+-alleviated drought stress in tea plants. In the present study, we compared with the case of tea plants under drought higher water and chlorophyll contents were found in drought-stressed tea plants with an external K+ supply, confirming the role of externally supplied K+ in mitigating drought stress. We also found that an adequate K+ supply promoted Cl- accumulation in the mesophyll of Taicha12 (drought tolerant) over that of in Fuyun6 (drought sensitive). Moreover, Gly, Cys, Lys and Arg were not detected in Fuyun6 under 'Drought' or 'Drought + K+' conditions. Results showed that an exogenous supply of Arg and Val significantly alleviated drought-induced damage in Fuyun6, suggesting their role in K+-alleviated drought stress in tea plants. Collectively, our results show that chloride and amino acids are important components associated with K+-alleviated drought stress in tea plants.BACKGROUND New surgical approaches for apical prolapse have gradually been introduced, with few prospective randomised controlled trial data to evaluate their safety and efficacy compared with traditional methods. OBJECTIVE To compare surgical uterine preservation with vaginal hysterectomy in women with uterine prolapse and abdominal procedures with vaginal procedures in women with vault prolapse in terms of clinical effectiveness, adverse events, quality of life and cost-effectiveness. DESIGN Two parallel randomised controlled trials (i.e. Uterine and Vault). Allocation was by remote web-based randomisation (1  1 ratio), minimised on the need for concomitant anterior and/or posterior procedure, concomitant incontinence procedure, age and surgeon. SETTING UK hospitals. PARTICIPANTS Uterine trial - 563 out of 565 randomised women had uterine prolapse surgery. Vault trial - 208 out of 209 randomised women had vault prolapse surgery. INTERVENTIONS Uterine trial - uterine preservation or vaginal hysterectomy. Vaun the different procedures. It was found that all the surgical procedures were successful within the 12-month review period. Abdominal surgery in the Vault trial as well as any that was required in the Uterine trial, was, however, slightly less cost-effective. Serious complications and the need for further prolapse surgery were similar in all groups. A small number of women did require additional surgery for prolapse recurrence or for small mesh exposure when additional or prolapse procedures had involved mesh. Women in both trials will be followed up for at least 6 years to determine longer-term costs and consequences.The optimal choice of euthanasia method for laboratory rodents depends on a number of factors, including the scientific goals of the study, the need to minimize animal pain and/or distress, applicable guidelines and laws, the training and proficiency of personnel, and the safety and emotional needs of the personnel performing the euthanasia. This manuscript aims to provide guidance to researchers so they may select the method of euthanasia that results in minimal experimental confounds, such as the creation of artifact and alteration of tissues and analytes. Specific situations addressed include euthanasia of large numbers of rodents and euthanasia of neonates. Eukaryotic translation initiation factor 5A2 (EIF5A2) has shown to be upregulated inmany malignant tumors and proposed to be a critical gene involved in tumor metastasis. In thisstudy, we aim to investigate the expression status of EIF5A2 in human ATC tissues, and to study therole and mechanisms of EIF5A2 in ATC tumorigenesis in vitro and in vivo. Expression of EIF5A2protein were analyzed in paraffin-embedded human ATC tissues and adjacent nontumorous tissues(ANCT) (n=24) by immunochemistry. Expression of EIF5A2 mRNA and protein were analyzed infresh-matched ATC and ANCT (n=23) and ATC cell lines by real-time PCR and western blotting. Theeffect of targeting EIF5A2 with short hairpin RNA (shRNA) or EIF5A2 overexpression on the ATCtumorigenesis and TGF-β/Smad2/3 signals in vitro and in vivo was investigated. Expression ofEIF5A2 was significantly upregulated in ATC tissues and cell lines compared with ANCT and normalfollicular epithelial cell line. Functional studies found that targeting EIF5A2 induced SW1736 celldeath in vitro and in vivo, followed by significantly downregulated phosphorylation of Smad2/3(p-Smad2/3) in SW1736 cells at the protein level. Ectopic expression of EIF5A2 could promoted8505C cell growth in vitro and in vivo, followed by significantly upregulated p-Smad3 at the proteinlevel. Recombinant human TGF-β1 (hTGF-β1) treatment decreased the anti-proliferative activity ofthe EIF5A2 downexpressing 8505C cells through reversing p-Smad2/3. Using the specific inhibitorSB431542 to block TGF-β pathway or Smad3 siRNA to knockdown of Smad3 increased theanti-proliferative activity of the EIF5A2 overexpressing 8505C cells through inhibiting p-Smad2/3.Our findings indicated that EIF5A2 controled cell growth in ATC cells, and EIF5A/TGF-β/Smad2/3signal may be as a potential therapeutic target for ATC treatment.Marine n-3 fatty acids improve most of the biochemical alterations associated with insulin resistance (IR). Experimental models of dietary-induced IR in rodents have shown their ability (often at a very high dose) to prevent IR, but with sometimes a tissue specific effect. However, in a high sucrose diet-induced IR rat model, they are unable to reverse IR once installed; in other rodent models (dexamethasone, Zucker rats), they are inefficacious perhaps because of the severity of IR. The very low incidence of type-2 diabetes (T2D) in Inuits in the 1960s, which largely increased over the following decades in parallel to the replacement of their traditional marine food for a western diet strongly suggests a protective effect of marine n-3 towards the risk of T2D; this was confirmed by reversal of its incidence in intervention studies reintroducing their traditional food. In healthy subjects and insulin-resistant non-diabetic patients, most trials and meta-analyses conclude to an insulin-sensitising effect and to a very probable preventive or alleviating effect towards IR. Concerning the risk of T2D, concordant data allow us to conclude the protective effect of marine n-3 in Asians while suspicion exists of an aggravation of risk in Westerners, but with the possibility that it could be explained by a high heterogeneity of studies performed in this population. Some longitudinal cohorts in US/European people showed no association or a decreased risk. Further studies using more homogeneous doses, sources of n-3 and assessment of insulin sensitivity methods are required to better delineate their effects in Westerners.Knowing the biological signals associated with appetite control is crucial for understanding the regulation of food intake. Biomarkers of appetite have been defined as physiological measures that relate to subjective appetite ratings, measured food intake, or both. Several metabolites including amino acids, lipids and glucose were proposed as key molecules associated with appetite control over 60 years ago, and along with bile acids are all among possible appetite biomarker candidates. Additional metabolites that have been associated with appetite include endocannabinoids, lactate, cortisol and β-hydroxybutyrate. However, although appetite is a complex integrative process, studies often investigated a limited number of markers in isolation. Metabolomics involves the study of small molecules or metabolites present in biological samples such as urine or blood, and may present a powerful approach to further the understanding of appetite control. Using multiple analytical techniques allows the characterisation of molecules, such as carbohydrates, lipids, amino acids, bile acids and fatty acids.
