In the paper, after clarifying terms such as 'identity', 'self' and 'personhood', I propose an empirical account of identity based on the notion of "whole phenotype". This move allows one to claim the persistence of the individuals before and after their being affected by dementia. Furthermore, I show how this account permits us to address significant questions related to demented individuals' loss of the capacity of moral decisions.I apply Gloria Anzaldúa's "borderlands theory" to Jamie Cortez's Sexile, an HIV/AIDS prevention publication created as a first-person narrative of the journey of queer, trans activist Adela Vasquez who fled to the US from Cuba in 1980. I argue that Sexile is a borderlands text and operationalizes Anzaldúa's mestiza consciousness at various levels- ranging from the essence of the text and what its existence represents to the literary techniques used in the telling of Adela's narrative. In the first half of this paper, I explore Anzaldúa's borderlands theory and the ethos of Sexile as a text, including Vázquez's story as inherently a borderland narrative; as a product of democratized knowledge; the praxis of the text's creation; and the technology of the graphic novel. Throughout the second half, I look more closely at the content of the text using close reading to explore notions of the "borderland" represented throughout Adela's narrative. Through both its content and the praxis of creation, Sexile represents the power of graphic medicine in illustrating the nuances of narratives at the interstices of marginalized identities and captures the multitudes and ambiguities of trauma, loss, rebirth, kinship, and joy at the margins.Steven Soderbergh's Contagion (2011) positions the vaccine as the end point of the arc of pandemic, marking both the containment of an elusive virus and the resumption of a life not fundamentally different from before the disease outbreak. The film reinforces the assumption that a pandemic will awaken all of us to the urgency of vaccination, persuading us to put aside our reservations and anxieties and the idea that compliance is the inevitable outcome of quarantine. This article explores how pro-vaccination cultural products such as Contagion might in fact undermine public health efforts by promoting a false narrative, which simplifies the kind of vaccination campaign necessary for herd immunity to develop. An ethic of sacrifice and selflessness drives the public health messaging of the film but leaves intact certain individualistic tropes and plague narrative scapegoating tendencies, while the framing of the vaccine as "gift" takes it out of the realm of medical science altogether. Most studies of severe/fatal COVID-19 risk have used routine/hospitalisation data without detailed pre-morbid characterisation. Using the community-based UK Biobank cohort, we investigate risk factors for COVID-19 mortality in comparison with non-COVID-19 mortality. We investigated demographic, social (education, income, housing, employment), lifestyle (smoking, drinking, body mass index), biological (lipids, cystatin C, vitamin D), medical (comorbidities, medications) and environmental (air pollution) data from UK Biobank (N = 473,550) in relation to 459 COVID-19 and 2626 non-COVID-19 deaths to 21 September 2020. We used univariate, multivariable and penalised regression models. Age (OR = 2.76 [2.18-3.49] per S.D. [8.1 years], p = 2.6 × 10-17), male sex (OR = 1.47 [1.26-1.73], p = 1.3 × 10-6) and Black versus White ethnicity (OR = 1.21 [1.12-1.29], p = 3.0 × 10-7) were independently associated with and jointly explanatory of (area under receiver operating characteristic curve, AUC = 0.79) increased risk of COVID-19 mortality. In multivariable regression, alongside demographic covariates, being a healthcare worker, current smoker, having cardiovascular disease, hypertension, diabetes, autoimmune disease, and oral steroid use at enrolment were independently associated with COVID-19 mortality. Penalised regression models selected income, cardiovascular disease, hypertension, diabetes, cystatin C, and oral steroid use as jointly contributing to COVID-19 mortality risk; Black ethnicity, hypertension and oral steroid use contributed to COVID-19 but not non-COVID-19 mortality. Age, male sex and Black ethnicity, as well as comorbidities and oral steroid use at enrolment were associated with increased risk of COVID-19 death. Our results suggest that previously reported associations of COVID-19 mortality with body mass index, low vitamin D, air pollutants, renin-angiotensin-aldosterone system inhibitors may be explained by the aforementioned factors.Ruptured intracranial aneurysms, as the leading cause of spontaneous subarachnoid hemorrhage (aSAH), represents an emergency with high morbi-mortality. The comprehension of the underlying pathology that involves inflammatory and immune responses, through the neutrophil-to-lymphocyte ratio (NLR), could help to predict complications such as delayed cerebral ischemia (DCI) or rebleeding and the functional outcome. Systematic review of English-based literature through PubMed and Biblioteca Vitural em Saúde (BVS) to find papers discussing the use of NLR in the aSAH setting. Area-under-curve (AUC) of receiver operating characteristics (ROC), cutoff value, sensitivity, and specificity were retrieved. From 53 articles included, 4 papers were evaluated after exclusion criteria. Rebleeding could be predicted with a NLR cutoff value of 9.88 (sensitivity 72.3%, specificity 63.3%). The mean cutoff value for DCI was 12.85, with sensitivity 66.3% and specificity 75.8%. Finally, a worse 3-month functional outcome could be predicted with a mean sensitivity of 73.3% and a mean specificity of 54%. NLR is a new issue in scientific community, especially neurosurgery. The current understanding points to a multifactorial process after aSAH that emerges as alterations on the NLR. As a measurement readily available and cost-effect after admission of the patient, its use signals that patients that need expedite surgical treatment or more aggressive treatment for vasospasm. As other medical subspecialties already use this ratio to predict outcomes, the literature reviewed by this paper constitute the earliest clues that higher NLR predicts re-bleeding, DCI, and functional outcome.Breast tuberculosis is a rare form of extrapulmonary tuberculosis which tends to affect females of reproductive age, and is much rarer in males, postmenopausal women, and pre-pubescent girls. It is difficult to diagnose because it can mimic benign conditions such as a fibroadenoma, as well as malignant diseases like a carcinoma. It is also particularly difficult to distinguish breast tuberculosis from granulomatous mastitis, so other possible diagnoses should be ruled out first. The diagnosis of breast tuberculosis has three essential pillars clinical examination, radiological evaluations, and histopathological sampling. Radiological evaluations are not only critical in diagnosis but are also important in the planning of the treatment and during the follow-up. This paper aims to review the imaging findings and the differential diagnosis of breast tuberculosis.Open injuries bear the risk of foreign body contamination. Commonly encountered materials include gravel debris, glass fragments, wooden splinters or metal particles. While foreign body incorporation is obvious in some injury patterns, other injuries may not display hints of being contaminated with foreign body materials. Foreign objects that have not been detected and removed bear the risk of leading to severe wound infections and chronic wound healing disorders. Besides these severe health issues, medicolegal consequences should be considered. While an accurate clinical examination is the first step for the detection of foreign body materials, choosing the appropriate radiological imaging is decisive for the detection or non-detection of the foreign material. Especially in cases of impaired wound healing over time, the existence of an undetected foreign object needs to be considered. Here, we would like to give a practical radiological guide for the assessment of foreign objects in head and neck injuries by a special selection of patients with different injury patterns and various foreign body materials with regard to the present literature.Severe acute respiratory syndrome coronavirus 2 (SARS-CoV2) is the novel pathogen responsible for the coronavirus disease 19 (COVID-19) outbreak. Researchers and clinicians are exploring the pathogenetic mechanisms of the viral-induced damage and growing interest is focusing on the short-term and long-term immune-mediated consequences triggered by the infection. We will focus on post-SARS-CoV2 infection arthritis which may arise as a new pathological condition associated with COVID-19. find more In this article, we describe a case of acute oligoarthritis occurring 13 days after a SARS-CoV2 severe pneumonia in a middle-aged Caucasian man and we go over a brief review of the current available literature. We hypothesize that molecular mimicry might be the basic immunological mechanism responsible for the onset of COVID-19-related arthritis based on the current knowledge of SARS-CoV2 and on the known pathogenetic mechanism of viral-induced arthritis.
Pathological tibial torsion is known to negatively influence the functionality of the lower extremity, and therefore, its assessment might play an important role. While 3D imaging is used for many examinations of the musculoskeletal system, for the determination of tibial torsion no 3D measurement technique has been available so far. We developed a 3D measurement method and assess its interobserver reliability as well as its correlation with standard 2D measurement methods.

CT scans of 82 tibiae in 79 patients with a mean age of 41years were included. A novel 3D measurement technique was developed and applied. Measurements were compared with two frequently used 2D measurement methods. ICC (intraclass correlation coefficient) for the new technique was determined and compared to the 2D measurement method. Furthermore, differences between left and right legs as well as between males and females were assessed.

The ICC for the 2D methods was 0.917 and 0.938, respectively. For the 3D measurements, ICCs were calculated to be 0.954 and 0.950. Agreement between 2 and 3D methods was moderate to good with ICCs between 0.715 and 0.795. Torsion values for left and right legs did not differ significantly in 2D and in 3D (26.2 vs 28.5° and 27.2 vs. 25.9°). The same is true for the differences between male and female in 2D and 3D (26.2 vs. 29.6° and 25.0 vs. 31.2°).

The newly developed 3D measurement technique shows a high intraclass agreement and offers an applicable opportunity to assess the tibial torsion three-dimensionally.
The newly developed 3D measurement technique shows a high intraclass agreement and offers an applicable opportunity to assess the tibial torsion three-dimensionally.Ona River is one of the three major rivers draining the city of Ibadan. Groundwater is the major source of drinking water in the metropolis; however, data on quality of shallow aquifers bordering Ona River is relatively scarce. This study aimed to evaluate bacteriological status, heavy metal content, and associated human and ecological health risks in hand-dug wells nearby Ona River. A total of 24 water samples from 12 sampling points were collected for chemical and microbial analyses. Heavy metals and microbial pathogens were analyzed using atomic absorption spectrometry and total plate count methods, respectively. Analyses of microbial and heavy metal (HMs) data showed that shallow hand-dug wells within the vicinity of Ona River were bacteriologically contaminated while most of analyzed heavy metals (except manganese) exceeded the drinking water quality standards. Interpretation of microbial and heavy metal (HMs) data identified predominance of anthropogenic activities as the major source of contamination in drinking water.
