D. acutus envenomation can result in coagulopathy and lead to death if not treated correctly. A 20-year-old man was discovered with a severely swollen left thigh with overlying dark purple, discolored skin. He was immediately transported to hospital. Laboratory examinations revealed dysfunctional coagulation and fluid-electrolyte imbalances. He died 2 h later despite resuscitation efforts. Surveillance footage revealed that he had walked through a grass field while returning home that night. Autopsy and pathological examination findings revealed a large area of muscle necrosis of the left thigh, renal tubular necrosis, and hepatocyte necrosis. Potential fang marks were found on the decedent's jeans. Due to our suspicions, we performed specific enzyme-linked immunosorbent assays (ELISA) and detected D. acutus venom in the kidneys, left thigh muscle, liver, lung, spleen, and heart tissues of the decedent. In conclusion, the clinical manifestations, autopsy, histopathological examination, ELISA, and investigation results confirmed D. acutus envenomation.The surfaces of thirty-one gunpowders (GPs) from Winchester, Federal, Hercules, DuPont, and Remington were examined by scanning electron microscopy/energy dispersive X-ray spectroscopy. Particles and surface coatings on 22 of the GPs had compositions of sulfur/potassium/barium or sulfur/potassium detected. Primer-like particles composed of lead/antimony/barium were found on surfaces of twelve GPs. Gunpowders from fifteen different lots of Winchester 9 mm Silvertip® hollow point cartridges were found to vary in GP type (flake and flattened ball) and associated inorganic compounds of sulfur/potassium/barium or sulfur/potassium or one with only potassium. No inorganic particles or coatings were detected on two of these Winchester GPs. The elements from the inorganic compounds on GPs likely contribute to gunshot residue generated by the discharge of a firearm.Prophylactic oxytocin administration at the third stage of labour reduces blood loss and the need for additional uterotonic drugs. Obesity is known to be associated with an increased risk of uterine atony and postpartum haemorrhage. It is unknown whether women with obesity require higher doses of oxytocin in order to achieve adequate uterine tone after delivery. The purpose of this study was to establish the bolus dose of oxytocin required to initiate effective uterine contraction in 90% of women with obesity (the ED90 ) at elective caesarean delivery. We conducted a double-blind dose-finding study using the biased coin up-down design method. Term pregnant women with a BMI ≥ 40 kg.m-2 undergoing elective caesarean delivery under regional anaesthesia were included. Those with conditions predisposing to postpartum haemorrhage were not included. Oxytocin was administered as an intravenous bolus over 1 minute upon delivery of the fetus. With the first woman receiving 0.5 IU, oxytocin doses were administered according to a sequential allocation scheme. The primary outcome measure was satisfactory uterine tone, as assessed by the operating obstetrician 2 minutes after administration of the oxytocin bolus. Secondary outcomes included the need for rescue uterotonic drugs, adverse effects and estimated blood loss. We studied 30 women with a mean (SD) BMI of 52.3 (7.6) kg.m-2 . The ED90 for oxytocin was 0.75 IU (95%CI 0.5-0.93 IU) by isotonic regression and 0.78 IU (95%CI 0.68-0.88 IU) by the Dixon and Mood method. Our results suggest that women with a BMI ≥ 40 kg.m-2 require approximately twice as much oxytocin as those with a BMI less then 40 kg.m-2 , in whom an ED90 of 0.35 IU (95%CI 0.15-0.52 IU) has previously been demonstrated.

The congruent matching cells (CMC) method was invented for accurate breech face impression comparison, in which a reference impression is divided into correlation cells. Each cell is registered to a cell-sized area of the comparison impression that has maximum similarity in surface topography. Four parameters are used to quantify the congruent matching pattern of the registration position and orientation. This paper aims to further develop the cell-division-matching method based on a convergence feature and to develop practical convergence-improved algorithms for firing pin impression comparison. The convergence feature refers to the tendency of the x-y registration positions of correlated cell pairs to converge at the correct registration angle when comparing same-source samples at different orientations. The areal Gaussian filter is employed to extract high-frequency micro-features; the least-squares matching method is used to improve each cross-correlation precision and reach convergence in the registration positions of correlated cell pairs; and a density-based clustering algorithm is introduced to collect the registration positions of dense cell pairs relative to a virtual common center and to remove outliers. Improvements are achieved in the reliability and accuracy of the number of congruent matching cell pairs (CMCs) collected, which represents the quantification of the degree of pairwise impression similarity. Experiments in this report used 40 firing pin impression samples on cartridge cases fired from 10 pistols. The current study provides frequencies of presentation of ten cut surface characteristics and examines intra- and interindividual variation between three hand-powered reciprocating saws. Pig radii were used as a proxy for human remains. 42 cut surfaces were created with three hand-powered reciprocating saws by the same researcher. Cut surface characteristics were assessed macroscopically, microscopically and with Scanning Electron Microscope (SEM), as well as photographed with Reflectance Transformation Imaging (RTI). Cut surface polish is the only characteristic that demonstrates consistently significant difference between saws. Five characteristics demonstrate some variation between blades entrance shaving, breakaway spurs, breakaway notches, pull-out striae, and tooth hop. Four characteristics demonstrate no difference between blades cut surface striation shape, cut surface striation regularity, harmonics, and exit chipping. Tooth hop is the most reliable characteristic for differentiating between blades in this study, while pull-out striae demonstrates high intra-individual variability and a low frequency of presentation making it unreliable for differentiating between saw blades. Cut surface striations, exit chipping, and breakaway spurs occurred with the highest frequencies and were reliable for determining direction of blade progress and blade stroke, but were not reliable for differentiating between saw blades. Harmonics were absent. Further research is necessary to create large databases of known saw mark examples with known intra- and interindividual variability rates and error rates.Both posture and loading rate are key factors in the herniation process and can determine the failure mechanism of the disc. The influence of disc structure on the herniation process has yet to be directly observed, thus the aim of this study was to test the hypothesis that discs containing greater levels of pre-existing disruption would be more vulnerable to herniation when subjected to severe levels of posture and loading. 30 ovine lumbar motion segments were subjected to combinations of 4 loading conditions (0 - 12° flexion,0 - 9° lateral bending, 0 - 4° axial rotation, 0-1500 N axial compression) for 1000 loading cycles at 2 Hz in a dynamic disc loading simulator. The discs were scanned in an ultra-high field MRI (magnetic resonance imaging, 11.7 T) prior to and following testing. 4 discs herniated and 7 discs suffered nucleus displacement. These discs contained pre-existing defects in the central dorsal annulus. Generally, following testing, discs contained more dorsal annulus disruption, including 7 discs which developed similar characteristic defects although these did not herniate. Overall, more severe complex postures produced more disruption. While more severe postures such as twisting and bending increased disc damage, these results are probably the first directly showing that naturally occurring defects in the disc can act as initiation sites for herniation. The clinical significance of these findings is that, in principle at least, MRI based techniques could be capable of identifying vulnerable discs, with the obvious caveat that further correlation with clinical techniques is required.In 2014, the Indonesian government passed the Mental Health Act (MHA) to address the country's complex mental health situation. The implementation of the MHA has been slow, and little is known about how the MHA is perceived by mental healthcare providers within local settings. This study aimed to obtain insight into psychiatrists' views on the MHA, including on how it affected their clinical practice and on challenges of translating the MHA into practice. The study was conducted in Bali, and 27 psychiatrists (15 men and 12 women) participated in a semi-structured interview. Thematic analysis indicated four overarching themes raising the profile of mental health, developing a shared understanding of mental illness, integrating psychiatric practice with other services and views on implementation of the MHA into practice. Overall, the psychiatrists viewed the MHA as a step in the right direction to improve mental health services and to create awareness at local and national levels. However, there was consensus that the meaning of the MHA's concepts of mental problems and disorders were not compatible with psychiatric everyday practice or their patients' understandings.
