This study aimed to screen the entire genome for genetic markers associated with risk for Achilles tendon injury.

A genome-wide association analysis was performed using data from the Kaiser Permanente Research Board and the UK Biobank. Achilles tendon injury cases were identified based on electronic health records from the Kaiser Permanente Research Board databank and the UK Biobank from individuals of European ancestry. Genome-wide association analyses from both cohorts were tested for Achilles tendon injury using a logistic regression model adjusting for sex, height, weight, and race/ethnicity using allele counts for single nucleotide polymorphisms (SNP). Previously identified genes within the literature were also tested for association with Achilles tendon injury.

There were a total of 12,354 cases of Achilles tendon injury and 483,080 controls within the two combined cohorts, with 67 SNP in three chromosomal loci demonstrating a genome-wide significant association with Achilles tendon injury. The first locus contains a single SNP (rs183364169) near the CDCP1 and TMEM158 genes on chromosome 3. The second locus contains 65 SNP in three independently segregating sets near the MPP7 gene on chromosome 10. The last locus contains a single SNP (rs4454832) near the SOX21 and GPR180 genes on chromosome 13. The current data were used to test 14 candidate genes previously reported to show an association with Achilles tendon injury, but none showed a significant association (all P > 0.05).

Traditionally, midshaft clavicular fractures in adolescents are treated nonoperatively. In later years, a trend toward operative treatment can be observed. Documentation of the benefit of surgery in this group is scarce. The purpose of this study is to evaluate the long-term patient reported functional outcomes and complications for patients treated operatively and nonoperatively for displaced midshaft clavicular fractures. Using the same outcomes we also compared the operative methods.

One hundred nine adolescents aged 12 to 18 years sustaining displaced midshaft clavicular fractures in the period 2010 to 2016 were identified in our computerized files. Sixty-one were treated nonoperatively, 48 operatively (22 plate and 26 intramedullary nail). Their radiographs and patient journals were examined for fracture classification, wound infection, sensory affection, surgery duration, hardware removal, and nonunion (n=109). Long-term function, pain, and satisfaction were measured with Quick Disability of Arm, Shullary nail, and nonoperative treatment. No additional benefit is demonstrated for surgery in our material. Nonoperatively treated patients are more satisfied with the cosmetic results. Little difference is seen between the operative methods in our study. We conclude that surgery should rarely be the choice of treatment for displaced midshaft clavicular fractures in adolescents.

Dendritic cells (DCs) are considered a multifunctional cell population that links the innate and adaptive immune systems. Dendritic cells have a capacity for antigen capture and presentation to T cells, which initiates a cascade of inflammatory reactions. On contrary to its importance in immunology, DCs have not been known well in peri-implantitis.A scanning electron microscopy coupled with energy-dispersive X-ray spectroscopy examination was used to examine a fixture that failed due to peri-implantitis, and a transmission electron microscopy was used to examine the peri-implant inflamed soft tissue. The presence of a DC was suggested in both scanning electron microscopy and transmission electron microscopy images. Titanium elements were also detected in the fixture-attached bone with energy-dispersive X-ray spectroscopy analysis. These findings suggested a link between Ti particles and DCs activation. The correlation between the presence of Ti particles and DCs will help to elucidate the detailed mechanisme detailed mechanism of peri-implantitis.
The frontalis suspension procedure is a common surgical method for patients with severe blepharoptosis. While frontalis suspension is a very effective method, the transplant material may be visualized after surgery in patients with a deep sulcus and severe blepharoptosis. To prevent this complication, we performed a combination of dermal fat grafting and frontalis suspension using a polytetrafluoroethylene sheet in 5 patients (6 eyelids). We followed-up the patients for at least 6 months postoperatively (mean 6.8 months) and observed no transplant material visualization or occurrence of infection. The mean pre- and postoperative margin reflex distance-1 was -3.75 (-5 to -2) and 2.10 (1-3), respectively. Bulky upper eyelids were observed 6 months postoperatively in 1 patient (2 eyelids). None of the patients underwent reoperation. In conclusion, the combination of frontalis suspension using a polytetrafluoroethylene sheet and dermal fat grafting for severe blepharoptosis and a deep upper eyelid sulcus was efor severe blepharoptosis and a deep upper eyelid sulcus was effective in preventing visualization of the transplant material.
Hyrtl [Wien Med Wochenschr. 1862;19291-292] reported firstly the ophtalmopetrosal sinus (OPS) running from the superior orbital fissure and emptying either in the transverse sinus, or in its junction with the superior petrosal sinus. Since then, this anatomic variant was mostly referred and poorly studied. Neither Hyrtl, nor other authors brought any evidence of such OPS. It is hereby reported and proofed such an OPT which was found at dissection. It drained ophthalmic veins and coursed posteriorly, first on the greater wing (alar segment), then on the temporal bone (temporal segment, on the petrosquamous fissure), emptying in the transverse sinus. A lateral type of the superior petrosal sinus connected the OPS and the transverse sinus. The OPS courses on the middle fossa floor, lateral to the Meckel's cavum, foramen ovale, foramen spinosum, and fallopian hiatus. It is a rare thus unexpected finding during specific approaches of the middle fossa floor.
