However, a growing number of clinical observations have shown that LVADs are associated with thromboembolic events, which are potentially related to the changes in intraventricular flow. RNA Synthesis inhibitor In this study, a fluid structure interaction (FSI) simulation was conducted to evaluate the hemodynamics of a patient specific left ventricle (LV) with varying LVAD IC orientations. The LV model was obtained from computed tomography scans and modeled to have contraction and relaxation during cardiac cycles following available experimental data of LV volume changes. The LV of the patient was assumed to have an end systolic volume of 223.7 mL and a stroke volume of 46.4 mL. Four different IC positions were considered towards the (1) septum; (2) aortic valve (AV); (3) mitral valve (MV) and (4) inferior wall (IW). The potential thrombus growth around the IC was assumed to be caused by blood stagnation regions with low velocity ( less then 5 mm/s) and low shear rate ( less then 60/s) flow. Mean velocity magnitudes and low blood velocity regions around the IC were numerically obtained. To quantitatively compare the thrombosis risks of the four simulation cases, the time-averaged volumes of the low-velocity regions and the low shear rate regions were calculated. The intraventricular volumes of low velocity zones based on IC orientation are 1.42 mL toward the septum, 1.14 mL toward the AV, 0.93 mL toward the MV, and 1.24 mL toward the IW. The intraventricular volumes of low shear regions based on IC orientation are 11.54 mL toward the septum, 11.15 mL toward the AV, 9.24 mL toward the MV, and 10.7 mL toward the IW. IC orientation toward the MV results in lower volumetric regions of low flow and low shear within the ventricle, which consequently may lead to a reduced risk of thrombus formation.One new clerodane-type diterpenoid, together with one known, was isolated from the leaves and twigs of C. krabas. The roles of gut microbiota in the activities of anti-fatigue foods can also be explained in the causes and the effects of fatigue. Among the causes of fatigue, the accumulation of lactic acid, decrease of energy, and reduction of central nervous system function were related to gut microbiota metabolism. Among the harmful effects of fatigue, oxidative stress, inflammation, and intestinal barrier dysfunction were related to gut microbiota dysbiosis. Furthermore, gut microbiota, together with anti-fatigue foods, can inhibit pathogen growth, convert foods into highly anti-oxidative or anti-inflammatory products, produce short-chain fatty acids, maintain intestinal barrier integrity, inhibit intestinal inflammation, and stimulate the production of neurotransmitters that regulate the central nervous system. Therefore, it is believed that gut microbiota play important roles in the activities of anti-fatigue foods and may provide new insights on the development of anti-fatigue foods.The balance of cisplatin uptake and efflux, mediated mainly by organic cation transporter 2 (OCT2) and multidrug and toxin extrusion 1 (MATE1), respectively, determines the renal accumulation and nephrotoxicity of cisplatin. Using transporter-mediated cellular uptake assay, we identified wedelolactone (WEL), a medicinal plant-derived natural compound, is a competitive inhibitor of OCT2 and a noncompetitive inhibitor of MATE1. Wedelolactone showed a selectivity to inhibit OCT2 rather than MATE1. Cytotoxicity studies revealed that wedelolactone alleviated cisplatin-induced cytotoxicity in OCT2-overexpressing HEK293 cells, whereas it did not alter the cytotoxicity of cisplatin in various cancer cell lines. Additionally, wedelolactone altered cisplatin pharmacokinetics, reduced kidney accumulation of cisplatin, and ameliorated cisplatin-induced acute kidney injury in the Institute of Cancer Research mice. In conclusion, these findings suggest a translational potential of WEL as a natural therapy for preventing cisplatin-induced nephrotoxicity and highlight the need for drug-drug interaction investigations of WEL with other treatments which are substrates of OCT2 and/or MATE1.
To analyze the visual outcomes following chemotherapy for optic pathway glioma in children with neurofibromatosis type 1.

This was a retrospective cohort study of 58 children with optic pathway glioma between 2003 and 2019. Best corrected visual acuity (BCVA) at diagnosis and at the last follow-up visit were analyzed. Correlation tests were performed to determine whether the final BCVA was predicted by the initial BCVA, initial Dodge stage, or retinal nerve fiber layer (RNFL) thickness stability. The first-line chemotherapy regimen included vincristine plus carboplatin (2003 to 2016) and vinblastine (since 2017).

Twenty-four children (41%) received chemotherapy. The median age at diagnosis was 3 years, and 61% of the children were female. From the first visit to the last follow-up visit, there was a statistically significant difference in mean BCVA in eyes in the no treatment group (
= .034) but not in eyes in the chemo-therapy group (
= .824). A moderate and weak positive correlation was found (
= 0.58 and
= 0.29, respectively). At the last follow-up visit, BCVA remained stable in 73% and improved in 27% of eyes in the no treatment group, and BCVA worsened in 25%, remained stable in 62%, and improved in 13% of eyes in the chemotherapy group. At the last follow-up visit, BCVA and Dodge stage had a weak negative correlation (
= -0.06 and
= -0.17, respectively). A negative moderate correlation was identified between RNFL thickness and BCVA at the last follow-up visit (
= -0.48 in the no treatment group and
= -0.46 in the chemotherapy group).

To determine the best treatment for dissociated vertical deviation (DVD).

The authors investigated the postoperative results of unilateral surgery for DVD in 14 patients when the visual acuity difference between the two eyes did not result in a postoperative dominance switch. All patients underwent bilateral medial rectus recession for congenital esotropia.

With this unilateral surgical procedure, all patients showed a significant improvement of the DVD in the primary position. None of the patients developed hypotropia, anti-elevation syndrome, or inferior oblique overaction in the contralateral (dominant) eye, and no changes were observed in the contralateral eye.

To evaluate the heterogeneity of both the clinical features and genetics of familial exudative vitreoretinopathy (FEVR) in a Northern Irish population.

A retrospective trawl of a secure pediatric database was completed, as well as communication with all Northern Ireland ophthalmologists to identify adult cases. Cases were cross-referenced with a regional genetics database. Data on patient demographics, clinical findings, genetic testing, and patient treatment were collected.

Sixteen patients were identified. Average age at presentation was 11.8 years (range 4 months to 38 years). Earlier age at presentation was associated with more advanced disease and those presenting later had more subtle signs such as retinal tear or vitreous hemorrhage. Four types of gene mutations were identified in 7 patients (

). Thirteen patients had complications associated with FEVR and associated systemic conditions were found in 5 patients. Twelve eyes received active treatment to control disease.

FEVR is a sight-threatening disease affecting prenatal retinal angiogenesis with a spectrum of disease and diverse genetic basis. Clinicians should look for signs of systemic and other ophthalmic sequelae in patients with FEVR because this could point to a genetic cause. Vigilance should also be exercised in older patients with unexplained vitreous hemorrhage or retinal tear with consideration of widefield angiography if FEVR is suspected.

To review and analyze abstracts presented at American Association for Pediatric Ophthalmology and Strabismus (AAPOS) annual meetings, and to identify publication rates and potential predictive factors for better outcomes.

All abstracts accepted for presentation at AAPOS annual meetings from 2013 to 2017 were examined. A comprehensive literature search via PubMed and Scopus was conducted to identify whether the abstracts had been published as full text. A multivariate analysis was achieved to assess the factors related to successful publication, and a Kaplan-Meier analysis was performed to evaluate the publication time course of abstracts.

Of 819 analyzed abstracts, 437 (53%) were published in peer-reviewed journals with a median impact factor of 2.7. The overall mean time to publication was 3 ± 2.2 years. The multivariate analysis revealed that abstracts were more likely to be published if they were accepted at an AAPOS annual meeting as an oral paper (
