We report a case of atypical type A thymoma variant manifesting polymyalgia rheumatica. A 68-year-old man underwent extended thymectomy with concomitant resection of the pericardium and right lung for an anterior mediastinal tumor. He was diagnosed with atypical type A thymoma variant with pericardial invasion. He developed pain in his extremities one year and 2 months after surgery. Detailed examinations resulted in a diagnosis of polymyalgia rheumatica and bone metastasis of thymoma. He was treated with oral prednisolone for polymyalgia rheumatica. His symptoms and bone lesion have been stable up to the present time of 3.5 years post-surgery. BACKGROUND Primary graft dysfunction (PGD) is a leading cause of early morbidity and mortality after heart transplantation (HT). Statins are known to have immunomodulatory and anti-inflammatory effects, and perioperative statin therapy has been associated with reduced cardiovascular complications and improved outcomes after cardiac and non-cardiac surgery. Thus, we investigated the influence on PGD of statin therapy administered to recipients before HT. METHODS A retrospective cohort study was conducted on 275 HT recipients assessed from 1997 to 2017; 167 (61%) had received statins during the month prior to and at time of transplantation, whereas 108 (39%) had not. Endpoints included PGD (defined according to the ISHLT consensus statement), in-hospital mortality, and one- and five-year survival. RESULTS PGD incidence was significantly lower for statin-treated patients (21 vs 60%, P less then 0.001). Multivariable analysis demonstrated that pre-HT statin therapy was independently associated with a significant 65% reduced risk for PGD and a 73% reduced risk for in-hospital mortality. One- and five-year mortality, adjusted for age, gender and amiodarone therapy, were significantly lower for recipients treated with statins (HR 0.33 and 0.39, 95% CI 0.17-0.63 and 0.22-0.68, respectively; P = 0.001). CONCLUSIONS Pre-HT statin therapy was independently associated with a reduced risk for PGD and mortality. Our results also suggested that statins have a beneficial prognostic impact on heart failure patients awaiting HT. BACKGROUND - Patients with acute type A aortic dissection (ATAAD) present with heterogeneous involvement of the aortic root complex. Despite this variation, the aortic root can usually be preserved the majority of the time by Teflon inlay patch reconstruction of the dissected sinuses of Valsalva (SOV). In this study, we report the long term anatomic, functional, and clinical outcomes associated with the preserved SOV after surgery for ATAAD. METHODS - From 2002-2017, of 776 emergency ATAAD operations at a single institution, 558 (71.9%) underwent valve resuspension with SOV preservation. Echocardiography reports were reviewed to obtain postoperative SOV dimensions. Cumulative incidence of SOV dilation ≥45mm was calculated using the Fine-Gray method with death as a competing risk. Repeated measures linear mixed effects model was used to determine risk factors for SOV growth over time. RESULTS - During the follow-up period, 62/558 (11.1%) patients developed SOV diameter ≥45mm. Cumulative incidence of SOV dilation ≥45mm at 1, 5, and 10 years was 5.5%, 12.4%, and 18.9% respectively. In a multivariable cox regression model, preoperative SOV diameter ≥ 45mm was associated with a hazard ratio of 14.11 (95% CI 7.03-31.62) for postoperative SOV dilation ≥45. In a repeated measures linear mixed effects model, preoperative and discharge SOV diameter were significant predictors of SOV dilation. Postoperative time course was also identified as significant indicating growth over time. CONCLUSIONS - The preserved sinuses of Valsalva after surgery for ATAAD may be prone to progressive dilatation over time. Closer echocardiographic surveillance may be warranted in these patients. BACKGROUND Anomalous origin of the right coronary artery from the pulmonary artery (ARCAPA) is a rare congenital cardiac lesion that has been diagnosed in both children and adults with symptoms ranging from an asymptomatic murmur to sudden cardiac death. The aim of this review was to characterize published cases of ARCAPA to better understand this rare congenital coronary anomaly. Selleckchem Bavdegalutamide METHODS A systematic review was performed using Pubmed, Embase, and Google Scholar for cases of ARCAPA. Keywords searched included "anomalous origin of the right coronary artery from the pulmonary artery" and "ARCAPA." RESULTS A total of 223 cases of ARCAPA were identified in 193 case reports. There was a slight male predominance (54.5%) and the median age at presentation was 14.0 years. Thirty eight percent of patients were asymptomatic and most commonly identified during evaluation of a murmur. Angina and dyspnea were the most common presenting symptoms (22.4% and 17.0%, respectively). In symptomatic patients, a bimodal distribution of age at presentation was observed with a peak near birth and another between age 40-60. The condition was most commonly diagnosed with coronary angiography (40.4%). Most cases were repaired surgically (72.6%) and re-implantation of the right coronary artery onto the aorta was the most common method of repair (62.3%). CONCLUSIONS ARCAPA represents a rare coronary anomaly with great variability in clinical presentation. An understanding of the pathophysiology associated with the lesion is critical when determining treatment strategies. This review represents the most extensive review of the condition to date. AIM We evaluated risk factors for sexual dysfunction in male cyclists with the primary intention of determining if genital/pelvic pain and numbness are associated with erectile dysfunction (ED). METHODS We surveyed male cyclists using an online anonymous questionnaire. STUDY OBJECTIVE To compare incidence of perioperative complications of total vaginal hysterectomy (TVH) in women with and without prior cesarean section (CS). DESIGN Retrospective cohort. SETTING Tertiary care academic institution. PATIENTS 742 women who underwent TVH over a five-year period. INTERVENTIONS TVH. link2 MEASUREMENTS AND MAIN RESULTS Prior CS did not increase overall rate of Clavien-Dindo grades 2-3 complications (p = 0.20). link3 The incidence of cystotomy (2.2% CS vs. 1.1% no CS, p = 0.29), ureteral injury (1.1% vs. 0.2%, p = 0.23), proctotomy (1.1% vs. 0.2%, p = 0.23), postoperative bleeding (1.1% vs. 0.6%, p = 0.47), or reoperation (0.0% vs. 0.3%, p = 1.00) was not increased from having a prior CS. Prior CS increased blood transfusion (5.6% vs. 0.6%, p less then 0.05), but did not increase conversion to laparotomy (2.2% vs. 0.6%, p = 0.15), length of hospitalization (11.2% vs. 14.1% discharge same-day, 66.3% vs. 63.6% discharge postoperative day one, 22.5% vs. 22.4% discharge on or after postoperative day two, p = 0.76), or 30-day readmission rates (1.1% vs. 3.5%, p = 0.34). link3 CONCLUSION In patients who underwent TVH, prior CS increased postoperative blood transfusion but did not increase risk for overall perioperative complications. The present study was conducted to explore the influence of dietary carbohydrate on antioxidant capacity and non-specific immunity of hybrid grouper, which would contribute to determine the tolerable dietary carbohydrate content. Seven diets with grade levels of carbohydrate (5.27, 8.95, 11.49, 14.37, 17.78, 20.82 and 23.65%) were fed to triplicate groups of fish for 10 weeks. Results showed that the inclusion of carbohydrate above 11.49% produced significant increased content of hydrogen peroxide (H2O2) in liver and malondialdehyde (MDA) in both serum and liver. link3 The specific activity of catalase (CAT), superoxide dismutase (SOD), glutathione peroxidase (Gpx) and total antioxidative capacity (T-AOC) were significantly elevated with the increase of dietary carbohydrate from 8.95 to 23.65%, which may be associated with the reduced hepatic soluble protein content. However, opposite variation was observed in the expression of antioxidant related genes (SOD1 and Gpx), which was partly caused by the activation of NF-E2-related factor 2 (Nrf2) and inhibition of Kelch-like-ECH-associated protein 1 (Keap1) at the transcriptional level. The immunoglobulin M (lgM) content and activity of lysozyme and CCP in serum significantly depressed when dietary carbohydrate was above 11.49%. The expression of pro-inflammatory cytokines (TNF-α, IL-1β and IL-8) was significantly increased with the increase of dietary carbohydrate from 5.27 to 8.95% and thereafter significantly reduced, which was consistent with the changed expression of toll-like receptor 2 (TLR2) and nuclear factor κΒ (NF-κΒ). In above, high dietary carbohydrate significantly impaired the antioxidant capacity and reduced the non-specific immunity of hybrid grouper, and the tolerable dietary carbohydrate content should not exceed 11.49%. The arginine repressor (ArgR) regulates the expression of genes involved in arginine biosynthesis. Upon attaining a threshold concentration of arginine in the cytoplasm, the trimeric C-terminal domain of ArgR binds three arginines in a shallow surface cleft and subsequently hexamerizes forming a dimer of trimers containing six Arg co-repressor molecules which are buried at the subunit interfaces. The N-terminal domains of this complex bind to the DNA promoter thereby interrupting the transcription of the genes related to Arg biosynthesis. The crystal structures of the wild type and mutant Pro115Gln ArgR from Corynebacterium pseudotuberculosis determined at 1.7 Å demonstrate that a single amino acid substitution switches co-repressor specificity from Tyr to Arg. Molecular dynamics simulations indicate that the first step, i.e., the binding of the co-repressor, occurs in the trimeric state and that Pro115Gln ArgR preferentially binds Arg. It was also shown that, in Pro115 ArgR hexamers, the concomitant binding of sodium ions shifts selectivity to Tyr.
