Since e-cigarettes arrived in the U.S. in 2007, they have been investigated by addiction researchers as possible cessation devices for adults trying to quit combustible, or regular, cigarettes. Because e-cigarettes contain fewer chemicals the industry has presented them as a healthier alternative to regular cigarettes. But vape liquids can still contain nicotine, a highly addictive drug. E-Cigarettes, also called personal vaporizers or “vapes”, present another way for smokers to ingest nicotine.

So these were pods that were laced with this product called vitamin E acetate, which is a syrupy, golden liquid that a lot of the bootlegged products were cut with in order to either make it more syrupy or in order to make it last longer, to go further. And so the CDC ultimately said it was most likely related to these products that contained this ingredient. And so as big tobacco tries to innovate, they cannot out-innovate Silicon Valley. So at the end of the day, Altria, the maker of Marlboro cigarettes, decides to invest in Juul.

At that point, levels of formaldehyde exceeded limits set by the California Office of Environmental Health Hazard Assessment (OEHHA), though other compounds didn’t come close. About a quarter of middle and high school students surveyed in 2017 said they had been around someone vaping at least once in the past 30 days. In 2021, only about 11 percent of high school students and 3 percent of middle school students surveyed said they had tried e-cigarettes. Many providers ask their patients about alcohol, drug use and smoking, yet forget to ask about vaping.

For this particular reason, e-cigarettes are often viewed as a smoking cessation tool, given that those with nicotine can prevent smoking craving, yet this idea has not been fully demonstrated [2, 6, 7]. E-cigarettes have become the most commonly used tobacco product among U.S. and Iowa youth, and their popularity has risen dramatically over the past several years. According to the Iowa Youth Survey (IYS), Iowa 11th-graders were far more likely to use e-cigarettes compared to traditional cigarettes. They also reported increased likelihood in trying e-cigarettes and decreased quit success when compared to cigarettes.

E-cigarettes not only pose substantial health risks to youth and young adults, they pose a significant environmental threat (see the Truth Initiative fact sheet on Tobacco and the Environment). "These products appear to be designed to circumvent the laws and regulations in place to protect people—especially children—from the harmful effects of smoking and tobacco use," Jordt said. Similar limitations were found when five of seven flavourings were found to cause cytotoxicity in human bronchial epithelial cells [87]. Beginning August 1, 2019, Minnesota law prohibits the use of these products indoors where cigarette use is prohibited, including bars and restaurants. However, a 2019 study into the long-term health effects of vaping found that people using e-cigarettes had a higher risk of respiratory disease than people who never smoked. Beside the direct harm experienced by users, e-cigarette vapors are potent sources of air pollution such as aldehydes, carbon monoxide, particulate matter (PM), VOCs, heavy metals, and nicotine1,3,13.

Health experts say cancer and heart and lung disease caused by smoking kill more than 480,000 people in the United States annually. No longer do I have to huddle under the awning of a building, trying to avoid the rain, nor do I have to freeze my fingers off trying to rush through a cigarette just so everyone else can be satisfied. At last I can feel like I am a part of society again, and not some evildoer who deserves to be sent out to the village gates to hide my atrocious acts.

These products have undergone rigorous scientific review, including toxicologic assessments, and have been found by FDA to meet the statutory public health standard. Vaping is often thought of as safer than cigarette smoking, but vaping causes health problems, too. Both vaping and smoking are addictive and bring potentially dangerous chemicals into your body.

Susceptibility to smoking or non-combustible nicotine use is also influenced by national tobacco control policies. In Europe, Finland has been among the countries with most advanced tobacco control regulations [31]. Still, there is a large difference in smoking and snus use between students in academically and non-academically oriented education [7].

E-cigarette companies also take advantage of other marketing outlets, including the internet, retail environments, and recreational venues and events. This resource presents an overview of key aspects of e-cigarettes, including health effects, marketing practices, and regulations based on research and evidence available at the time of publication. Both British American Tobacco Plc and Altria are prodding the FDA for heightened enforcement on “illicit” synthetic nicotine electronic cigarettes in the U.S. vaping market. The companies have estimated those synthetic products represent about half of the overall domestic e-cigarette market. Over the next few years, the optimism over e-cigarettes waned as their popularity skyrocketed. Prior to 2024, adults could also import nicotine e‑cigarettes with a prescription via the personal importation scheme. The PACT Act requires online sellers of ENDS products to register with ATF and to verify the age of purchasers both at the point of sale and the point of delivery, as well as to comply with tax collection provisions and state and local laws.

Further, it may influence the students’ descriptive and injunctive norms, which both predict smoking initiation among youth [27]. The most consistent predictors have been descriptive norms of parents’ and close friends’ smoking, indicating that addressing close network members might be the most effective in interventions [27]. In our study, parental smoking reported by adolescents increased all types of susceptibility when analysed independently.

He emphasised that e-cigarettes are not an effective tobacco cessation tool, as they are not approved by food regulatory bodies. Additionally, they often lead to dual usage with people continuing to use traditional cigarettes along e-cigarettes. Given the importance of flavors contributing to use of e-cigarettes among youth, these data briefs build on this previously published article, providing an update on trends in unit sales of e-cigarettes in the U.S. by product and flavor type. These estimates are based on the information available at the time of publication and may be subject to updates as more information becomes available. A Foundation-led study presented at the 2024 Society for Research on Nicotine and Tobacco annual meeting found that monthly e-cigarette sales in California declined by 41% after statewide restrictions on the sale of flavored tobacco products took effect. However, the report also noted that the tobacco industry continues to market products to offset these declines, including non-menthol cigarettes that contain other cooling chemicals.

Dr Sonu Goel, a distinguished public health expert and chairperson of the Tobacco Control Section explained that e-cigarettes, also known as Electronic Nicotine Delivery Systems (ENDS), vaporise a nicotine solution instead of burning tobacco leaves. This solution typically contains propylene glycol or glycerol, nicotine, and flavouring agents. Amid the increasing popularity of e-cigarettes and vapes among young people, health experts from PGIMER have raised concerns. They pointed out that many young people mistakenly think that e-cigarettes are “cool” because of their flavours, sleek designs, and social appeal.

Earlier this year, for instance, his research group found that acute exposure to e-cigarettes causes vascular inflammation. Minnesota teens are using e-cigarettes and vapes at alarming rates, exposing themselves to the harms of nicotine and risking addiction. In this data brief we use the 2017 MYTS to report e-cigarette initiation prevalence, which teens are trying e-cigarettes, and why initiation is a problem. One in four middle school and high school students in Minnesota had tried e-cigarettes (teens call it vaping or juuling).

Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74]. Similar observations were reported for the content ethylene glycol. In this regard, either it was detected at concentrations that did not exceed the authorised limit [73], or it was absent from the aerosols produced [4, 71, 72]. Only one study revealed its presence at high concentration in a very low number of samples [5]. Nevertheless, its presence above 1 mg/g is not allowed by the FDA [73]. Figure 1 lists the main compounds detected in aerosols derived from humectant heating and their potential damaging effects.

Similarly, a 2014 review reported that vaping may trigger gum, mouth, and throat irritation. In particular, a 2015 study examined the effects of flavored e-juices on both human lung cells and lung cells in mice. Here’s what we currently know about the effects of vaping fluids with and without nicotine and vaping cannabis or CBD.

New research led by Roswell Park experts in health behavior and published today in the journal JAMA Network Open suggests that daily e-cigarette use may help some people to quit using combustible cigarettes. Chalmers said some students are realizing the dangers of vaping after seeing their peers struggle with nicotine addictions. She believes that adding the CATCH My Breath education only solidifies it for them. Rates of e-cigarette use among Cumberland students aren’t available, but across North Carolina, 21.4% of high school respondents to the state’s 2023 Youth Risk Behavior Survey said they currently vaped.

A lot of the focus of the controversy and a lot of the focus in your book is that Juul’s marketing to teenagers, to influencers, to social media, created the crisis for itself, that they had an initial marketing plan and they hired a flashier marketing guy. And he was like, “We’re going all in.” New York City-cool, billboards with models, the whole deal. And Juul immediately was like, “We have no interest in selling outright. We want to retain control of the company.” And so there are all these negotiations going back and forth and they ultimately decided to invest $12.8 billion into Juul for a 35 percent share; at the time, the company was valued at $38 billion. So in this bout of desperation, when Altria is realizing that Juul is an actual threat, they actually start scouring the globe for products that they could just simply buy, that had been on the market prior to August of 2016.

The e-cigarette industry maintains that higher nicotine concentrations can help adults who smoke heavily switch from combustible cigarettes to vaping products, which are relatively less harmful to them. The FDA has approved high-nicotine, tobacco-flavored e-cigarettes for that purpose, said April Meyers, CEO of the Smoke-Free Alternatives Trade Association. Results from in vitro studies are in general agreement with the limited number of in vivo studies. For example, in an analysis using primary human umbilical vein endothelial cells (HUVEC) exposed to 11 commercially-available vapours, 5 were found to be acutely cytotoxic, and only 3 of those contained nicotine [24]. In addition, 5 of the 11 vapours tested (including 4 that were cytotoxic) reduced HUVEC proliferation and one of them increased the production of intracellular reactive oxygen species (ROS) [24]. Three of the most cytotoxic vapours—with effects similar to those of conventional high-nicotine CS extracts—also caused comparable morphological changes [24].

Friday’s approval included two disposable e-cigarettes and two pre-filled e-cigarette pods manufactured by NJOY, a subsidiary of tobacco giant Altria Group, which makes Marlboro and some menthol-flavored cigarettes. The decision arrives as the FDA is criticized for failing to rein in illegal but widely available flavored vaping products that appeal to young people. Some e-cigarettes are designed to be completely disposable, while others are rechargeable2,14. Disposable e-cigarettes and vaping pods, spent e-cigarette capsules or replaceable pods, pose the most significant potential environmental burden5,15. Vaping pods are an example of plastic waste because they are not biodegradable and are poorly recyclable15. Also, they contain similar waste components as reusable e-cigarettes but are used for a shorter time before being discarded5.

This legislation, which applies to all states, raises the minimum age of sale for all tobacco products— including e-cigarette—to 21. The American Lung Association is very concerned that we are at risk of losing another generation to tobacco-caused diseases as the result of e-cigarettes. The Lung Association remains extremely troubled about the rapid increase of youth using these products and has repeatedly called upon the Food and Drug Administration (FDA) to increase their oversight and scrutiny of these products to protect kids. Truth Initiative is America's largest nonprofit public health organization committed to preventing youth and young adult nicotine addiction and empowering quitting for all. That's hard to say — the potential health risks of vaping CBD are still hotly debated.

E-cigarettes and vaping devices are not covered by federal law restricting cigarette advertising. E-cigarettes recently surpassed conventional cigarettes as the most commonly used tobacco product among youth.1 It is critical that public health officials and the general public understand the potential risks of using them. We work with community partners to raise awareness of the harms of e-cigarettes and vaping and flavored tobacco products that attract youth, through the Unhyped and CounterBalance campaigns.

Although JUUL is currently the top-selling e-cigarette in the U.S., other e-cigarettes are becoming available that look like USB flash drives. Because of their shape, school teachers, administrators, and parents may not notice students using these devices in school, including in classrooms and bathrooms. Nicotine is a highly addictive substance found in tobacco products, including cigarettes, cigars, chewing tobacco, e-cigarettes, and pouches. Of this sample, 3,242 participants developed heart failure within a median follow-up time of 45 months.

Each e-cigarette was inserted into a sealed chamber attached to a lab-built device that drew air through the e-cigarette for eight seconds at a time with a resting period of 15 or 30 second between each draw. I think that there was another way that they could have done it that would have involved less flashy marketing and that would have been tailored toward adult smokers, which they claimed that it was. And now whether or not there was the intent there, I think that’s a pretty big remaining question that is going to be litigated in courtrooms across the country, is whether or not they intended to market to kids. But at the time, the management was very circumspect about the long-term viability of the e-cigarette market. They thought that regulators were not going to tolerate another nicotine product on the market without going through lots and lots of regulatory hurdles. They believed that this market would not last for long and that they didn’t really need to pour tons and tons of money into it in order to innovate.

These chemicals have been linked to lung inflammation, chronic coughs, shortness of breath and lung disease. E-cigarettes are not risk-free but are believed to cause less harm than smoking. Meanwhile, California was the least popular state for youth vaping, as just six percent of teens had picked up the habit. However, the state's largely rural landscape and small population could be partly to blame for its high youth vaping rate. About 80 percent of Wyoming's residents live in rural areas, as much of the land area is used for farming and national parks.

Critics of e-cigarettes fear that vaping will get kids hooked on nicotine and that they'll "graduate" to cigarettes when they want a bigger kick, Warner says. The 966 respondents who had never vaped more often believed that e-cigarette waste was dangerous to throw in trash compared to the 1,083 respondents who had vaped at least once (81.4% vs. 71%). Fewer of those who had vaped at least once (79.3%) believed that e-cigarettes contained toxic substances compared to those who had never vaped (89.6%).

The report also discusses the changing landscape of tobacco products, including smoked products, smokeless products, and e-cigarettes. The liquid solution used in e-cigarettes, sometimes called vape juice or e-liquid, usually contains nicotine and other chemicals. E-cigarettes can be used to smoke or "vape" marijuana products, herbs, waxes and oils. The liquid they contain also has many monikers—it might be called e-juice, e-liquid, cartridges, pods, or oil.

However, nicotine patches and many other FDA-approved quit aids are available now to help people quit smoking. E-cigarettes are not currently approved by the FDA as aids to help stop smoking. On the other hand, there is a large body of evidence clearly showing that FDA-approved medications are safe and effective ways to help people quit smoking, especially when combined with counseling. Scientists are still learning about how e-cigarettes affect health when they are used for long periods of time.

The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76]. Since 2016, the FDA regulates the flavours used in the e-cigarette market and has recently published an enforcement policy on unauthorised flavours, including fruit and mint flavours, which are more appealing to young users [77].

Vuse, owned by Reynolds American, and Juul control about 60% of the market, while hundreds of disposable brands account for the rest. The liquid and vapour contain some potentially harmful chemicals also found in cigarette smoke, but at a much lower level. Data from the 2019 Minnesota Student Survey shows a steep rise in e-cigarette use among students. This study was supported by funding from the National Institute on Drug Abuse of the NIH and the FDA Center for Tobacco Products. Adjusted analyses revealed that the type of e-cigarette device used played no role in the rates of cigarette discontinuation. Data were taken from the nationally representative Population Assessment of Tobacco and Health Study (PATH), which assessed participants through audio-based, computer-assisted self-interviews in English or Spanish. Interviews were conducted in "waves" -- October 2014 to October 2015, October 2015 to October 2016, December 2016 to January 2018, and December 2018 to November 2019. The last wave analyzed took place during the COVID-19 pandemic in March to November 2021 and was administered via audio computer-assisted self-interviews and telephone interviews.

Vaping isn’t safe and can cause health problems, including life-threatening lung injuries. You may have heard the benefits of vaping are that it’s safe or that it helps you quit smoking. Some people vape because they think it’ll help them quit smoking if they’ve already tried other methods and haven’t been able to quit. Unfortunately, you may end up addicted to vaping instead, so the benefits of vaping for smoking cessation may not be as good as they seem.

E-cigarettes are battery-operated devices that can look like a real cigarette or pen. There are hundreds of brands, and they're sometimes marketed as a way to get your nicotine fix without the danger of cigarettes. “This research has provided greater understanding in the court systems nationwide as to the nature of drug usage, abuse, and overdose cases in which electronic cigarettes were used to deliver an illicit drug,” the researchers concluded. Many of those who vape were likely to report issues that kept them from being able to safely dispose of their used and empty e-cigarette products. More than half (57.8%) of those who had used e-cigarettes in the past month found it inconvenient to dispose of e-cigarette waste responsibly.

Despite the absence of supporting data or environmental impact studies, eco-friendly claims have been used by manufacturers as a marketing strategy to promote e-cigarettes to consumers3,11. ESD aerosol is a new source of pollution and toxins being emitted into the environment. We do not know the long-term health effects of ESD use and although the industry marketing of the product implies that these products are harmless, the aerosol that ESD emit is not purely water vapor. These figures coincide with additional research that indicates an overall increase in e-cigarette use. Since the early 2000s TTCs have developed interests in e-cigarettes (also known as electronic delivery systems, or ENDS), heated tobacco products (HTPs), snus and nicotine pouches.

The vapor produced by e-cigarettes usually contains nicotine, heavy metals, volatile organic compounds, carcinogens (cancer causing chemicals), and in some cases, flavoring linked to serious lung disease. These and other harmful substances present in e-cigarette vapor can negatively affect the respiratory health of users. In January 2020, the US Food and Drug Administration (FDA) ordered the removal of flavored cartridge-based e-cigarettes from the market.
