Higher complication rates have been reported for total hip arthroplasty (THA) after osteosynthesis of proximal femur fractures (PFF). This study evaluated the infection risk for conversion of internal fixation of PFF to THA by a single-staged procedure in the absence of clear infection signs.

Patients undergoing a one-staged conversion to THA (2013-2018) after prior internal fixation of the proximal femur were included. Preoperative diagnostics with laboratory results, hip aspirations as well as intraoperative microbiology and sonication were assessed. Postoperative complications were recorded as well as patient demographics, duration between initial and conversion to THA, explanted osteosynthesis and implanted THA.

Fifty-eight patients (24 male/34 female, 62.8 ± 14.5years) were included with a mean time of 3.8 ± 7.5years between internal fixation and conversion to THA (45 cementless, 3 cemented, 3 hybrid and 7 hybrid inverse THAs). Preoperative mean blood level CRP was 8.36 ± 14mg/l (reference value &ltage conversion of PFF fixation to THA. Moreover, we found a high infection rate (5.8%) for early postoperative periprosthetic joint infection. Interestingly, CRP has not been proven to be an adequate parameter for low-grade infections or occult colonized implants. Therefore, we recommend a comprehensive pre- and intraoperative diagnostic including hip aspiration, swabs and sonication when considering one-staged revision.
To evaluate associations of omega-3 fatty acid (O3-FA) blood levels with cardiometabolic risk markers, functional capacity and cardiac function/morphology in patients with heart failure with preserved ejection fraction (HFpEF).

O3-FA have been linked to reduced risk for HF and associated phenotypic traits in experimental/clinical studies.

This is a cross-sectional analysis of data from the Aldo-DHF-RCT. From 422 patients, the omega-3-index (O3I = EPA + DHA) was analyzed at baseline in n = 404 using the HS-Omega-3-Index
methodology. Patient characteristics were; 67 ± 8years, 53% female, NYHA II/III (87/13%), ejection fraction ≥ 50%, E/e' 7.1 ± 1.5; median NT-proBNP 158ng/L (IQR 82-298). Pearson's correlation coefficient and multiple linear regression analyses, using sex and age as covariates, were used to describe associations of the O3I with metabolic phenotype, functional capacity, echocardiographic markers for LVDF, and neurohumoral activation at baseline/12months.

The O3I was below (< 8%), witnts. Higher O3I was associated with a more favorable cardiometabolic risk profile and aerobic capacity (left) but did not correlate with echocardiographic markers for left ventricular diastolic function or neurohumoral activation (right). An O3I-driven intervention trial might be warranted to answer the question whether O3-FA in therapeutic doses (with the target O3I 8-11%) impact on echocardiographic markers for left ventricular diastolic function and neurohumoral activation in patients with HFpEF. Prostate-specific membrane antigen (PSMA) is a promising target for prostate cancer imaging and therapy. The most commonly used scaffold incorporates a glutamate-urea (Glu-Urea) function. We recently developed oxalyldiaminopropionic acid-urea (ODAP-Urea) PSMA ligands in an attempt to improve upon the pharmacokinetic properties of existing agents. Here, we report the synthesis of an optimized
Ga-labeled ODAP-Urea-based ligand, [
Ga]Ga-P137, and first-in-human results.

Twelve ODAP-Urea-based ligands were synthesized and radiolabeled with
Ga in high radiochemical yield and purity. Their PSMA inhibitory capacities were determined using the NAALADase assay. Radioligands were evaluated in mice-bearing 22Rv1 prostate tumors by microPET. Lead compound [
Ga]Ga-P137 was evaluated for stability, cell uptake, and biodistribution. PET imaging of [
Ga]Ga-P137 was performed in three patients head-to-head compared to [
Ga]Ga-PSMA-617.

Ligands were synthesized in 11.1-44.4% yield and > 95% purity. They have high affinity to PSMA (K
of 0.13 to 5.47nM). [
Ga]Ga-P137 was stable and hydrophilic. [
Ga]Ga-P137 showed higher uptake than [
Ga]Ga-PSMA-617 in tumor-bearing mice at 6.43 ± 0.98%IA/g vs 3.41 ± 1.31%IA/g at 60-min post-injection. In human studies, the normal organ biodistribution of [
Ga]Ga-P137 was grossly equivalent to that of [
Ga]Ga-PSMA-617 except for within the urinary tract, in which [
Ga]Ga-P137 demonstrated lower uptake.

The optimized ODAP-Urea-based ligand [
Ga]Ga-P137 can image PSMA in xenograft models and humans, with lower bladder accumulation to the Glu-Urea-based agent, [
Ga]Ga-PSMA-617, in a preliminary, first-in-human study.

The last one involves removing the anti-association factor eIF6 from the 60S ribosomal surface by the joint action of the Elongation Factor-like 1 (EFL1) GTPase and the SBDS protein. Herein, we studied the evolutionary relationship of the EFL1 and EF-2 protein families and the functional conservation within EFL1 orthologues. Phylogenetic analysis demonstrated that the EFL1 proteins are exclusive of eukaryotes and share an evolutionary origin with the EF-2 and EF-G protein families. EFL1 proteins originated by gene duplication from the EF-2 proteins and specialized in ribosome maturation while the latter retained their function in translation. Some organisms have more than one EFL1 protein resulting from alternative splicing, while others are encoded in different genes originated by gene duplication. However, the function of these alternative EFL1 proteins is still unknown. We performed GTPase activity and complementation assays to study the functional conservation of EFL1 homologs alone and together with their SBDS counterparts. None of the orthologues or cross-species combinations could replace the function of the corresponding yeast EFL1•SBDS binomial. The complementation of SBDS interspecies chimeras indicates that domain 2 is vital for its function together with EFL1 and the 60S subunit. The results suggest a functional species-specificity and possible co-evolution between EFL1, SBDS, and the 60S ribosomal subunit. These findings set the basis for further studies directed to understand the molecular evolution of these proteins and their impact on ribosome biogenesis and disease.Photoautotrophic microalgae have become intriguing hosts for recombinant protein production because they offer important advantages of both prokaryotic and eukaryotic expression systems. Advanced molecular tools have recently been established for the biotechnologically relevant group of stramenopile microalgae, particularly for several Nannochloropsis species and diatoms. Strategies for the selection of powerful genetic elements and for optimization of protein production have been reported. Much needed high-throughput techniques required for straight-forward identification and selection of the best expression constructs and transformants have become available and are discussed. The first recombinant proteins have already been produced successfully in stramenopile microalgae and include not only several subunit vaccines but also one antimicrobial peptide, a fish growth hormone, and an antibody. These research results offer interesting future applications in aquaculture and as biopharmaceuticals. In this review we highlight recent progress in genetic technology development for recombinant protein production in the most relevant Nannochloropsis species and diatoms. Diverse realistic biotechnological applications of these proteins are emphasized that have the potential to establish stramenopile algae as sustainable green factories for an economically competitive production of high-value biomolecules.This study determined the indoor air levels of polychlorinated biphenyls (PCBs) in some selected public bars in Southern part of Nigeria. The result obtained was used to carry out health risk assessment of exposure to PCBs in these indoor environment.s. Air samples were obtained from the selected popular bars using polyurethane (PUF) passive samplers. PCB quantification was done using GC-MS operated in selected ionization mode (SIM). Health risk assessment from exposure of these pollutants was determining using toxicity equivalence quotient (TEQ), inhalation risk assessment (IRA), incremental life cancer risk (ILCR), and hazard quotient (HQ). The results showed the average concentration range of 0.15-0.17 µg/m3. The mean TEQ at various locations was 0.00050 ng WHO-TEQ/m3, while the calculated daily inhalation exposure values were 0.0004 ng TEQ kg-1 day-1 for adults and 0.0002 ng TEQ kg-1 day-1 for children. Twelve human head and neck specimens (24 sides) have been studied using microdissection and histological serial sections of the nerves. The surgical anatomy of the dual reinnervation procedure according to JP Marie was investigated notably the branching pattern of the phrenic nerve (PN), the Ansa cervicalis (AC) and the recurrent laryngeal nerve (RLN).

Despite variations of the AC, a prominent inferior common trunk for sterno-hyoid and sterno-thyroid muscles can be used in more than 90% of the specimens. If the AC is missing because of previous surgery, the tiny nerve of the thyro-hyoid muscle can be used preferred. The PN display a double roots pattern from C3 to C4 cervical plexus in 50% of the specimens. A single root pattern can be found and an end-to-lateral neurorraphy can be used. Intra-laryngeal nerves pattern of the RLN display tiny collaterals which cannot be selected for abduction-adduction activity. Direct implantation of the Y-shape great auricular nerve within the posterior crico-arytenoid muscles can be a reliable method leading to challenging mechanical and functional conditions.

Several anatomical pitfalls, including intra-operative choices and variants of the donor nerves, but also the challenging intra-laryngeal dissection of the inferior laryngeal nerve need to be solved. A successful laryngeal reinnervation still needs further studies for a simplified procedure.
