Their successful colonization of extreme niches requires corresponding adaptations at proteogenomic level in order to maintain stable cellular structures and active physiological functions. Although some studies have already investigated the extremophilic lifestyles of archaeal species based on genomic features and protein structures, there is a lack of comparative proteogenomic analysis in a large scale. In this study, we explored 686 high-quality archaeal genomes (proteomes) sourced from the Pathosystems Resource Integration Center (PATRIC) database. General patterns of genomic features such as genome size, coding capacity (coding genes and non-coding regions), and G + C contents were re-confirmed. Shortly after drainage of pericardial effusion he developed a low cardiac output syndrome which had to be treated with extracorporeal membrane oxygenation (ECMO). This 9-day ECMO support helped the patient to recover his cardiac function, and thereafter a remission of his primary disease was successfully achieved with chemotherapy. It is a matter of discussion whether a large pericardial effusion with moderate symptoms in patients with HL should be evacuated or just observed since the effusion should ameliorate with chemotherapy. But based upon our experience in this case of hemodynamic instability due to a large effusion requiring evacuation, we propose that pericardiocentesis procedure should be performed with caution at a slow drainage rate of 0.5-1 ml/kg/hour with a maximum rate of 50 ml/hour, to help avoid the low cardiac output syndrome in patients with similar disease conditions.Background People living with dementia and multimorbidity are frequent uses of health, amplifying risk of fragmented care when moving between care settings. Objectives This Systematic review aims to identify interventions to support transitional care for older people living with dementia and multimorbidity, and their caregivers. Methods A systematic search from January 2000-2018 of academic databases for studies which implemented a transitional care intervention for older people living with dementia and multimorbidity, and their caregivers. Results Out of 6053 identified citations, 11 studies and 13 papers were included. These studies included 1861 people living with dementia, mean age 80 years and 1503 caregivers, mean age of 69 years. Narrative synthesis identifed six elements of care that optimise outcomes; unmet needs; depression; education and support; physical decline; poor quality of life and; access and knowledge of community services. Conclusion This review demonstrates the paucity of interventions available to reduce impact and experiences of transitions for this vulnerable population. The need is increasing for further research and development in transitional care.Yellow leaf disease (YLD) is an economically important disease affecting betel palm in several countries, the cause of which remains unclear despite associations with putative agents, including phytoplasmas. In this study, we screened the potential casual agents associated with YLD in Hainan, China using next-generation sequencing and revealed the association of areca palm velarivirus 1 (APV1) with the YLD-affected palm. The complete genome of the APV1-WNY isolate was determined to be 17,546 nucleotides in length, approximately 1.5 kb longer than the previously reported APV1_HN genome. Transmission electron microscopy showed that APV1 particles are flexuous and filamentous, a typical morphology of species in the Closteroviridae family. Comparison of symptomatic and symptomless tree populations showed a strong association between APV1 and YLD. APV1 was detected in Pseudococcus sp. mealybugs sampled from YLD-affected trees in many locations, suggesting that mealybugs are a potential transmission vector for APV1. Although further studies are needed to confirm a causal relationship, these results provide timely information for the prevention and management of YLD associated with APV1.Background Empathy is a key skill for nurses for providing high-quality care. Results The definitive model composed of 15 items grouped into three factors presented good psychometric properties. Levels of empathy in the sample can be considered high. Conclusion The study provides a reliable and valid instrument to measure levels of empathy in nurses in the Spanish context. Impact Statement The instrument is suitable for assessing educational needs, and evaluating the effectiveness of training programmes.Ultrasound (US) is a noninvasive and versatile technology that in recent years found acceptance in almost all the medical specialties, with diagnostic and interventional applications. In the diabetic foot syndrome (DFS), US found specific indications mainly in the screening, quantification, and follow-up of the vascular component of the pathology, but also in the study of the deformities and structural modifications induced by neuropathy and in the diagnosis and surgical management of infections, especially those that induce anatomical changes, like abscesses and fasciitis. This review will summarize all these application of US, giving special attention to the vascular aspects, and on the predominant role that US gained in recent times to guide the indication to revascularization, on the new standardized approach to the study of the arterial tree of the limb and the foot, the so-called duplex ultrasound arterial mapping, which significantly increased the utilization of US to plan the revascularizations in this complex pathology. Outside the vascular fields, the diagnosis of neuropathy and infection and the intraoperative use of US in the surgical management of abscesses and fasciitis will be discussed, leaving the last part to the new and interesting applications of US in the management of DFU, a field that is still in evolution, offering new possibilities to the health care professionals involved in the management of these chronic wounds. The variety of applications both in diagnostic and operative fields makes US a rather versatile technology-a toolkit-that should have a special place among those at reach of the specialists of DFS care.
No evidence has shown the efficacy of Sodium Risedronate (Risedronate) for glucocorticoid-induced osteoporosis (GIO) in patients with Rheumatoid arthritis (RA). The aim of this study was to explore the effectiveness and safety of Risedronate for GIO complicated with RA.

This was a six-month randomized, double-blind, placebo-controlled trial of 95 patients with GIO complicated with RA from 19 centers. The primary endpoint was the change from baseline in lumbar spine bone mineral density (L-BMD). Secondary endpoints included changes in femoral neck and total hip BMD and bone turnover markers, as well as rheumatoid arthritis Disease Activity Score with 28-joint counts. Incident of non-traumatic spine fractures and adverse events were tracked as safety endpoints.

Increase in L-BMD was significantly greater in the Risedronate group compared to the Placebo group (Risedronate 3.49% [95% CI 1.92-5.05] vs Placebo 0.12% [95% CI -2.07 to 2.30],
 < .0001). No significant difference was found in the femoral neck and total hip BMD. Although adverse events were observed in 28 patients, none were considered serious. Non-traumatic vertebral fractures were identified in 10 patients.

This study measured the performance of normal-hearing listeners on the Repeat-Recall Test (RRT) in two noise types (2-talker babble [2TBN] and continuous speech-shaped noise [SSN]) by two noise azimuths (0° and 180°) configurations at signal-to-noise ratios (SNRs) of 0, 5, 10, and 15 dB and quiet.

Twenty-one listeners with normal hearing who also passed cognitive screening were tested in the sound-field with the speech stimulus presented from 0° at 75 dB SPL in 4 noise configurations. The order of SNRs, noise configurations, and RRT topic conditions was counterbalanced across listeners.

Analysis revealed that repeat scores were significantly better for 2TBN, for noise at 180°, and for high context (HC) sentences. Recall performance was significantly better for SSN and HC sentences. Listening effort ratings were higher for SSN and for noise front condition at SNR ≤ 10 dB. The 2TBN noise was tolerated longer than SSN. Performance on all measures improved with SNRs.

These data showed performance differences among noise configurations and provided a preliminary basis for comparison with hearing-impaired listeners' performance on the RRT.
