Higher staffing levels in long-term care have been associated with better outcomes for residents in several landmark studies. However previous systematic reviews found mixed results, calling into question the effectiveness of higher levels of staff. With persistent concerns about quality, rising resident acuity, and a growing demographic of seniors requiring more services, understanding the relationship between quality and long-term care staffing is a growing concern.

This review considered the following question What is the influence of nursing and personal care staffing levels (registered nurse, licensed practical nurse, and nursing assistant) and / or skill mix on long-term care residents, measured by quality of care indicators?

Preferred Reporting Items for Systematic Review and Meta-analysis Protocols guided the report of this systematic review.

Published articles focused on quality and nursing and personal care staffing in long-term care in peer-reviewed databases (MEDLINE, CINAHL, and AGELINE) last decade. Although research continues to standardize units of measurement, and longitudinal and instrumental variable analyses are increasingly being used, very few studies controlled for endogeneity, conducted adequate risk-adjustment, and used resident-level data. Additional strides must still be made to improve the rigor of long-term care staffing research.The dynamic framework of mind wandering (Christoff, Irving, Fox, Spreng, & Andrews-Hanna, 2016) is reviewed and modified through integrating the construct of mindful meta-awareness. The dynamic framework maintains that mind wandering belongs to a family of spontaneous thought phenomena. The key defining feature of mind wandering is 'spontaneity' which characterizes the dynamic nature of thoughts in the framework. The argument is made that incorporating the mindful meta-awareness construct modifies the dynamic framework as follows (1) the framework's criteria for mind wandering do not hold anymore as meta-awareness changes the relationship between thoughts and constraints, and (2) lucid dreaming can be categorized as unguided thought while at the same time being dependent on deliberate constraints. Finally, the application of this modified framework will be discussed in terms of the treatment of mental disorders related to spontaneous thought alterations, in particular depression and nightmares.Electroplating sludge had a high content of heavy metals and usually lacked high-value-added utilization. In this work, Cu-containing sludge was used to synthesize a spinel catalyst, which was applied in catalytic oxidization of toluene. As a result, the sludge-derived spinel removed 50% of toluene (1000 ppm, 9600 h-1) at 280 °C. In comparison, a reagent-synthesized spinel with a similar component removed 50% of pollutant at 294 °C. The sludge-derived spinel also showed a stable performance for over 50 h at 370 °C. Even when the initial concentration was increased to 5000 ppm, or the gas hourly space velocity was increased to 40,000 h-1, the temperature for 50% removal was only increased to 303 °C. According to characterizations, surface oxygens of the sludge-derived spinel were more active than those in the reagent-synthesized one. Besides, the former had more active surface oxygens (207.9 μmol/g) than the latter (183.1 μmol/g). Furthermore, the sludge-derived spinel was coated on a monolithic honeycomb, which were also effective in catalytic oxidization of toluene. Whether under planned or market allocation, urban land is an important part of China's market-oriented economic reform. However, direct empirical research on the impact of industrial land transfer on environmental pollution is still lacking. Based on matched data, this study investigates the impact of land resource mismatch and land marketization on the pollution emissions of Chinese enterprises. Pollution emissions refer to the emission intensity of industrial enterprises' major pollutants, such as industrial water, industrial waste gas and sulfur dioxide, and it is calculated by the comprehensive index method. The impact of land resource mismatch and land marketization on the pollution emissions of Chinese enterprises is investigated using a panel fixed effect model, subsample regression, the instrumental variable method and a mediating effect model. The benchmark analysis shows that land resource mismatch increases pollution emissions. The influence mechanism analysis shows that land resource mismatch may increase the pollution emissions of enterprises by causing overinvestment and low productivity. In addition, the expanded analysis shows that China's land marketization can reduce the intensity of and have a long-term impact on the pollution emissions of enterprises. This study describes U.S. population-based recurrence rates among older, resected women with HR+/HER2- early BC.

We conducted a retrospective cohort study of older women diagnosed with incident, invasive stages I-III HR+/HER2- BC who underwent surgery to remove the primary tumor using the Surveillance, Epidemiology, and End Results (SEER)-Medicare Linked Database (2007-2015). SEER records and administrative health claims data were used to ascertain patient and tumor-specific characteristics, treatment, and frailty status. Cumulative incidences of BC recurrence were estimated using a validated algorithm for administrative claims data. Multivariable Fine-Gray competing risk models estimated adjusted subdistribution hazards ratios and 95% confidence intervals for associations with BC recurrence risk.

In two experiments, we manipulated meter-syntax alignment while sentences with relative clause structures were either read as text (experiment 1, n = 40) or listened to as speech (experiment 2, n = 40). In experiment 2, we also measured the stability with which participants could tap in time with the metrical accents in the sentences they were comprehending. In addition to making more mistakes, sensorimotor synchronization was disrupted when syntactic cues clashed with the metrical context. We suggest that this reflects a tight coordination of top-down linguistic knowledge with the sensorimotor system to optimize comprehension.
Among the various causes for lower gastrointestinal bleeding, ectopic varices constitute a small chunk. Though rare, these can pose a diagnostic challenge with recurrent bleed leading to multiple admission and blood transfusions.

A 41-year-old male presented to our department with multiple episodes of melena. On further evaluation with CT angiography, a diagnosis of extrahepatic portal vein obstruction with moderate splenomegaly and ectopic jejunal varix was made. He underwent splenectomy with resection of involved jejunal segment with side to side anastomosis.

The diagnosis of ectopic varices remains elusive in a large number of cases in view of the varied etiology. Various newer endoscopic and imaging modalities can play a diagnostic as well as therapeutic role but this also further complicates the management as there is a lack of substantial guidelines directing the treatment protocol. As a result, we have to resort to a case by case approach for the optimal management in these cases.

Human tails are rare congenital malformations that describe protrusions commonly located around the midline of the lumbosacral region covered by skin, representing an embryonic remnant. Human tails are broadly classified into true and pseudo-tails. The real ones represent the persistence of a remnant of the embryonic tail formed between the fourth and eighth week of gestation.

A male infant aged 42 days presented to our institution with a tail-like structure that originated from the anal mucocutaneos junction. The parents did not bring the baby at birth because of the social stigma and ignorance. On examination, there was a tail-like structure that is about 22 cm in length and 1.5 cm in diameter originated from the mucocutaneous junction at the anus at the left side. There was no bony element on palpation. The lower vertebral column examination showed no abnormalities, and this was confirmed with an x-ray. The patient was operated on with simple excision of the tail and primary closure of the wound at the base. He was discharged on the same day well and good. The tail was taken for microscopic histopathology.

Angioleiomyoma is a rare benign soft tissue tumor of smooth muscle getting its origin from the muscular layer of vessel walls. The localized type of leiomyoma occurs on the skin, deep soft tissues, in the genitourinary, gastrointestinal, and respiratory tracts.

We report a case of a 16-year-old patient who presented with an intra-articular leiomyoma developed in the anterior cruciate ligament and treated with arthroscopy.

Angioleiomyoma can occur anywhere in the body and is most often seen in the extremities, particularly the lower limbs, but is very rare in the knee joint. Angioleiomyoma should be included in the differential diagnosis of nodular lesions mimicking loose body, such as lipomas, inclusion cysts, ganglion, pigmented villonodular synovitis (PVNS), fibroma, nodular synovitis, hemangioma, synovial sarcoma, myopericytoma, leiomyosarcomas, glomus tumor.

Angioleiomyoma related to the cruciate ligaments of the knee should be considered in the differential diagnosis of a painful knee especially when associated with a decreased of range of motion of knee.
