Operative vaginal birth (OVB) is an important skill for obstetricians. It is the most common emergency intervention in obstetrics and requires a high degree of skill. While there is a lot of data available on technical and non-technical factors affecting the outcome of OVBs little work has been done to investigate the factors that make obstetricians feel confident and competent when performing such a procedure. The objective of this study was therefore to identify the common factors that affect confidence and competence of obstetricians in operative vaginal births (OVB). Our hypothesis was that a qualitative research method would provide a rich approach where themes would be developed that the participants themselves identify as important.

Despite the potential usefulness, no automatic detector is available for brain metastases on contrast-enhanced CT (CECT). The study aims to develop and investigate deep learning-based detectors for brain metastases detection on CECT.

The study included 127 CECTs from 127 patients (65.5 years±11.1; 87 men). link2 The ground-truth annotation was performed semi-automatically by applying connected-component analysis to the binarized dataset by three radiologists. link2 Single-shot detector (SSD) algorithms, with and without a feature-fusion module, were developed and trained using 97 scans. The performance was evaluated at the detector's 50 % confidence threshold with the remaining 30 scans using sensitivity, positive-predictive value (PPV), and the false-positive rate per scan (FPR).

Feature-fused SSD achieved an overall sensitivity of 88.1 % (95 % confidence interval [CI] [85.2 %,88.6 %]; 214/243) and PPV of 36.0 % (95 % CI [33.7 %,37.1 %]; 233/648), with 13.8 FPR (95 % CI [12.7,15.0]). Lesions < 3 mm had a sensitivity of 23.1 % (95 % CI [21.2 %,40.0 %]; 3/13), with 0.2 FPR (95 % CI [0.23,0.65]). Lesions measuring 3-6 mm had a sensitivity of 80.0 % (95 % CI [76.0 %,79.8 %]); 60/75) with 5.8 FPR (95 % CI [5.0,6.2]). Lesions > 6 mm had a sensitivity of 97.4 % (95 % CI [94.1 %,97.4 %]); 151/155) with 7.9 FPR (95 % CI [7.2,8.5]). Feature-fused SSD had a significantly higher overall sensitivity (p = 0.03, t = 2.75) or sensitivity for lesions < 3 mm (p = 0.002, t = 4.49) than baseline SSD, while the overall PPV was similar (p = 0.96, t = -0.02).

The PHB production cost at two annual production capacities ($12.5/kg for 2,500 t PHB/year and $7.8/kg for 25,000 t PHB/year) was not cost-competitive to current PHB production processes when the revenues derived from co-products were not considered. link3 Sensitivity analysis projected the economic viability of a biorefinery concept that could achieve a minimum selling price of $1.1/kg PHB similar to polypropylene. The annual PHB production capacity and the identification of marketable end-uses with respective market prices for the co-products CPE and PI were crucial in attaining process profitability. Greenhouse gas emissions (ca. CRCD2 0.64 kg CO2-eq/kg PHB) and abiotic depletion potential (61.7 MJ/kg PHB) were lower than polypropylene. Results of this study provided a practical way for nitrogen removal from low C/N wastewater in cold region.Dopamine is a neuromodulator and neurotransmitter responsible for a number of physiological processes. Dysfunctions of the dopamine metabolism and signalling are associated with neurological and psychiatric diseases. Here we report that in primary co-culture of neurons and astrocytes dopamine-induces calcium signal in astrocytes and suppress spontaneous synchronous calcium oscillations (SSCO) in neurons. Effect of dopamine on SSCO in neurons was dependent on calcium signal in astrocytes and could be modified by inhibition of dopamine-induced calcium signal or by stimulation of astrocytic calcium rise with ATP. Ability of dopamine to suppress SSCO in neurons was independent on D1- or D2- like receptors but dependent on GABA and alpha-adrenoreceptors. Inhibitor of monoaminoxidase bifemelane blocked effect of dopamine on astrocytes but also inhibited the effect dopamine on SSCO in neurons. These findings suggest that dopamine-induced calcium signal may stimulate release of neuromodulators such as GABA and adrenaline and thus suppress spontaneous calcium oscillations in neurons.The temporal dynamics of calcium signaling are critical regulators of chondrocyte homeostasis and chondrogenesis. Calcium oscillations regulate differentiation and anabolic processes in chondrocytes and their precursors. link3 Attempts to control chondrocyte calcium signaling have been achieved through mechanical perturbations and synthetic ion channel modulators. However, such stimuli can lack both local and global specificity and precision when evoking calcium signals. Synthetic signaling platforms can more precisely and selectively activate calcium signaling, enabling improved dissection of the roles of intracellular calcium ([Ca2+]i) in chondrocyte behavior. One such platform is hM3Dq, a chemogenetic DREADD (Designer Receptors Exclusively Activated by Designer Drugs) that activates calcium signaling via the Gαq-PLCβ-IP3-ER pathway upon administration of clozapine N-oxide (CNO). We previously described the first-use of hM3Dq to precisely mediate targeted, synthetic calcium signals in chondrocyte-like ATDC5 cells. Here, we generated stably expressing hM3Dq-ATDC5 cells to investigate the dynamics of Gαq-GPCR calcium signaling in depth. CNO drove robust calcium responses in a temperature- and concentration-dependent (1 pM-100 μM) manner and elicited elevated levels of oscillatory calcium signaling above 10 nM. hM3Dq-mediated calcium oscillations in ATDC5 cells were reliant on ER calcium stores for both initiation and sustenance, and the downregulation and recovery dynamics of hM3Dq after CNO stimulation align with traditionally reported GPCR recycling kinetics. This study successfully generated a stable hM3Dq cell line to precisely drive Gαq-GPCR-mediated and ER-dependent oscillatory calcium signaling in ATDC5 cells and established a novel tool to elucidate the role that GPCR-mediated calcium signaling plays in chondrocyte biology, cartilage pathology, and cartilage tissue engineering.Mitochondrial activity warrants energy supply to oxidative myofibres to sustain endurance workload.
