In conclusion, hyponatremia with AC should be recognized during the preoperative and postoperative periods and may require treatment with hypertonic saline in addition to fluid restriction.Ectopic thyroid most commonly presents in the midline and is typically associated with the absence of an orthotopic thyroid. Less commonly, ectopic thyroid can present in the lateral neck, typically with a coexisting orthotopic thyroid and abnormal pathology in either the ectopic or orthotopic thyroid tissue. The most common cause of neurological symptoms in patients with systemic malignant tumors is central nervous system (CNS) metastases, and CNS metastases are one of the important causes of morbidity and mortality in these patients. The most common metastatic tumors to the CNS are lung, breast, malignant melanoma, genitourinary, and gastrointestinal tumors. We aimed to analyze our data on patients with CNS metastases in our department, which belongs to a large archive in the field of neuropathology.

The data of patients who had CNS metastases between January 2015 and August 2021 in our department were reviewed retrospectively. The patients were grouped in terms of demographic data, location, histopathological diagnosis, and primary origin characteristics, and their frequency and immunohistochemical staining characteristics were investigated.

There were 256 patients with CNS metastases in our study. The mean age was found to be 56.12. Of the patients,30.5% were female and 69.5% were male. A 74-year-old man with a history of diabetes mellitus presented with four days of fever, chills, dyspnea on exertion, and dark brown urine. A physical exam was notable for scleral icterus. Laboratory findings were significant for lactate dehydrogenase (LDH) of 518, total bilirubin of 7.4, and direct bilirubin of 6.2. Imaging, including abdominal ultrasound, CT abdomen and pelvis, magnetic resonance cholangiopancreatography (MRCP), and hepatobiliary iminodiacetic acid (HIDA) scans, demonstrated acute acalculous cholecystitis. On further history, the patient confirmed a recent hiking trip in Virginia. Further evaluation, including peripheral smear and polymerase chain reaction (PCR), was consistent with Babesia microti infection. Babesiosis is common in the Northeastern and Midwestern United States, and symptoms can range from asymptomatic infection to nonspecific malaise and fever to severe end-organ dysfunction. Diagnosis is via peripheral smear or PCR, which can be confirmed via serology. The combination of clindamycin and quinine or atovaquone and azithromycin are the cornerstones of pharmacotherapy. Acute acalculous cholecystitis is a very uncommon presentation of babesiosis. Babesia infection must be considered in the differential in a patient with nonspecific symptoms living in an endemic area.Maxillofacial defects and their rehabilitation are a major concern in this socially productive era. Social Pediatrics focuses on targeting and mitigating the effects of the social determinants of health on a child's well-being and development. Negative health outcomes have been seen in children who have faced poverty, food insecurity, inadequate housing, and traumatic events. In particular, children who come from socioeconomically disadvantaged households are more likely to develop behavioural problems. The purpose of this study is to explore the experiences of caregivers for children with a behaviour-related disorder. This includes children affected by attention, academic, and social issues (e.g. attention-deficit hyperactivity disorder, autism spectrum disorder). This study will aim to understand the strengths, barriers, and social limitations to accessing and receiving care for children with behavioural disorders. Methods A qualitative descriptive design was used to conduct three focus groups. Of the 64 caregivers contacted, a total of 13 participants agreed to be in the study. All focus groups were aniatricians who spend time to make a human connection with their patients. Barriers included physician turnover, long wait times for referrals, and a lack of knowledge of available services. Caregivers who were young mothers felt an added layer of judgement when accessing the necessary care for their children. This study is important as it contributes to our knowledge on the role Social Pediatrics can play in the care of children with behaviour-related disorders.A congenital tracheo-esophageal fistula of the H-type is a rare variant. A delay in the diagnosis may have a deleterious effect on the lung because of recurrent infections. Surgery is the cornerstone of management. Self-expandable metallic stents (SEMS) do not have a role in the management of these fistulae. We report a case of a missed diagnosis of a congenital H-type fistula managed as an acquired tracheo-esophageal fistula with two attempts at conservative management with a tracheal self-expandable metallic stent. The difficulties and disadvantages of using self-expandable metallic stents for the management of benign tracheo-esophageal fistulae are also discussed.

The Drug Resistance Index (DRI) is a useful tool that combines antibiotic consumption and bacterial resistance into a single measure. In this study, we used the DRI to assess the impact of introducing a new antibiotic restriction form on antibiotic effectiveness for the treatment of Gram-negative infections in the intensive care unit (ICU).

This was an observational studyto assess and evaluate the antibiotic susceptibility of Gram-negative bacteria and antibiotic prescribing rates for the antibiotics indicated for Gram-negative bacteria following the introduction ofa new antibiotic restriction form. The study was conducted from 2015 to 2017 at King Abdulaziz Medical City, a tertiary care facility in Jeddah, Saudi Arabia.Changes in antibiotic effectiveness before and after the introduction of the form were evaluated by calculatinge effectiveness of antibiotics over time. The results of our study are significant in the way that it highlights the importance of having an effective antibiotic stewardship program in healthcare settings and regular feedback of antibiotic consumption data to the stakeholders to keep the antibiotic prescriptions in check, thereby ensuring their sustained effectiveness.A 57-year-old female presented with L4-L5 and L5-S1 mobile spondylolisthesis and associated stenosis with radiculopathy who failed conservative treatment. This patient underwent lateral lumbar interbody fusion (LLIF) of L4-L5 and L5-S1, and posterior spinal fusion (PSF) with instrumentation. LLIF is a minimally invasive procedure to treat degenerative diseases of the lumbar spine. LLIF at the L5-S1 vertebral level is a relative contraindication secondary to increased risk of injury to the lumbar plexus and access issues at this level during the approach. With the help of imaging, careful preoperative planning can make this a feasible procedure in select patients.The skin is an extremely rare site of metastasis from papillary thyroid carcinoma (PTC) and is linked to underlying disseminated malignancy, which reflects a dismal prognosis. We present the case of a 70-years-old Saudi female who presented at our clinic with an eight-month history of two painful and itchy skin nodules over the scalp and the medial aspect of the right arm. She had a history of total thyroidectomy for PTC 30 years prior. Computed tomography-positron emission tomography showed multiple fluorodeoxyglucose avid lung and skeletal metastases. This case highlights the fact that skin nodules in a patient with a history of PTC should be assessed carefully with a high suspicion of skin metastasis to avoid any delay in treatment.
