The present study assessed 1) within-breath modulation of sympathetic baroreflex sensitivity (BRS), and 2) the effect of acute intermittent hypercapnic hypoxia (IHH) on within-breath sympathetic BRS and respiratory-sympathetic entrainment. Seventeen men (24±4 years) underwent an 8-10-minute spontaneously breathing baseline while collecting continuous measures of blood pressure (BP), heart rate, MSNA, ventilation, and end-tidal gases. A subset of 12 participants subsequently underwent a 40-minute IHH exposure composed of 40 consecutive 1-minute breathing cycles; 40 seconds of hypercapnic hypoxia and 20 seconds of normoxia. Data were compared between inspiration vs. expiration and low vs. high lung volume (calculated from the integral of spirometry-derived flow). Sympathetic BRS was determined by the slope of the weighted linear regression between diastolic BP and MSNA burst incidence. Respiratory-sympathetic entrainment was quantified as percent of MSNA bursts during each respiratory epoch relative to the total burst count. Sympathetic BRS was similar between inspiration and expiration (-3.9±2.0 vs. -3.6±1.8 bursts/100heartbeats/mmHg; p=0.61) but greater during low vs. high lung volumes (-4.6±2.3 vs. -2.1±1.6 bursts/100heartbeats/mmHg; p0.05). These findings provide novel insight towards the mechanisms controlling within-breath modulation of sympathetic outflow in humans.Background Most studies that have evaluated the impact of infection-control measures (ICM) reported a decrease in latent tuberculosis (TB) and not in TB. The objective of this study was to evaluate the impact of ICM on TB incidence among Health Care Workers (HCW's). Methods We conducted a retrospective record review study in a general, tertiary care, university-affiliated hospital. All TB case reports among HCWs in the hospital from 2005 to 2018 were reviewed. The TB incidence was measured before and after 2012 to evaluate the impact of ICM implemented. Findings In total, there were 53 TB cases. The number of TB cases before and after the implementation of ICM was 42 (incidence 100.0 cases/100,000 HCWs/year) and 11 (incidence 26.2 cases/100,000 HCWs/year), respectively (p less then .0001). Conclusions/Application to Practice TB incidence among HCWs reduced significantly after the implementation of ICM. The establishment of ICM, such as written TB infection control plan, monitoring, screening, training, and education, can reduce TB incidence.Effective and specifically targeted social and therapeutic responses for antisocial personality disorders and psychopathy are scarce. Some authors maintain that this scarcity should be overcome by revising current syndrome-based classifications of these conditions and devising better biocognitive classifications of antisocial individuals. The inspiration for the latter classifications has been embedded in the Research Domain Criteria (RDoC) approach. RDoC-type approaches to psychiatric research aim at transforming diagnosis, provide valid measures of disorders, aid clinical practice, and improve health outcomes by integrating the data on the genetic, neural, cognitive, and affective systems underlying psychiatric conditions. In the first part of the article, we discuss the benefits of such approaches compared with the dominant syndrome-based approaches and review recent attempts at building biocognitive classifications of antisocial individuals. Other researchers, however, have objected that biocognitive approaches in psychiatry are committed to an untenable form of explanatory reductionism. Explanatory reductionism is the view that psychological disorders can be exclusively categorized and explained in terms of their biological causes. In the second part of the article, we argue that RDoC-like approaches need not be associated with explanatory reductionism. Moreover, we argue how this is the case for a specific biocognitive approach to classifying antisocial individuals.We present the results of the first-ever DNA barcoding study of odonates from the Maltese Islands. In total, ten morphologically identified species were collected during a two-week long expedition in 2018. Eighty cytochrome c oxidase subunit I (COI) barcodes were obtained from the collected specimens. Intra- and interspecific distances ranged from 0.00% to 2.24% and 0.48% to 17.62%, respectively. Successful species identification based on ascribing a single morphological species to a single Barcode Index Number (BIN) was achieved for eight species (80%). Although traditional statistics classes may advise against certain practices (e.g., multiple comparisons, small sample sizes, violating normality), they may fail to cover others (e.g., outlier detection and violating linearity). More common, perhaps, is that researchers may fail to remember them. In this article, rather than rehashing old warnings and diatribes against this practice or that, I instead advocate a general statistical-analysis strategy. This graphic-based eight-step strategy promises to resolve the majority of statistical traps researchers may fall into-without having to remember large lists of problematic statistical practices. Although the gastrointestinal system accounts for a few numbers of sudden deaths, bleeding through ruptured varices represent a life-threatening condition. The role of forensic pathologist is vital in dealing with sudden deaths. Here, we report a case of a 46-year-old man who died suddenly following the rupture of oesophageal varices.Full-thickness burns pose a major challenge for clinicians to handle because of their restricted self-healing ability. Even though several approaches have been implemented for repairing these burnt skin tissue defects, all of them had unsatisfactory outcomes. Moreover, during recent years, skin tissue engineering techniques have emerged as a promising approach to improve skin tissue regeneration and overcome the shortcomings of the traditional approaches. Although previous literatures report the wound healing effects of the squalene oil, in the current study, for the first time, we developed a squalene-loaded emulgel-based scaffold as a novel approach for potential skin regeneration. This squalene-loaded agar-based emulgel scaffold was fabricated by using physical cross-linking technique using lecithin as an emulsifier. Characterization studies such as X-ray diffraction, Fourier-transform infrared spectroscopy, and field emission scanning electron microscopy revealed the amorphous nature, chemical interactions, and cross-linked capabilities of the developed emulgel scaffold. The squalene-loaded emulgel scaffold showed excellent wound contraction when compared with the agar gel and negative control. In case of the histopathology and recent immunohistochemistry findings, it was clearly evidenced that squalene-loaded emulgel promoted faster rate of the revascularization and macrophage polarization in order to enhance the burn wound healing. Moreover, the findings also revealed that the incorporation of squalene oil into the formulation enhances collagen deposition and accelerates the burnt skin tissue regeneration process. Finally, we conclude that the squalene-loaded emulgel scaffold could be an effective formulation used in the treatment of the burnt skin tissue defects.Skin burn is a major health problem in the community and seeking new and suitable treatment is suggested. In this regard, traditional remedies were consider in many countries. Regarding clinical application of herbal medicine in the healing of burn wounds, the present study was designed to evaluate the effect of Gundelia (Gundelia tournefortii L) extract with milk-cream on the healing of second-degree burn in a rat model. Thirty-six male Wistar rats (220 ± 30 g) were divided into 3 groups (n = 12), after establishment of second-degree burn group1, were left without any intervention; group 2, were treated topically with silver sulfadiazine; and group 3, were treated with Gundelia tournefortii L extract composite with milk-cream once a day for 21 days. Macroscopically and histological examinations were conducted on 7, 14, and 21 days of therapy. Data analyses were done using 1-way analysis of variance and post hoc Tukey tests. Macroscopically, evaluation of wounds' sizes on the 14th and 21st days indicated that the wound surface was reduced significantly (P less then .001) in group 3 compared with groups 1 and 2. Histological findings also showed that burn healing was significantly improved in group 3 compared with the other groups. Gundelia tournefortii L extract composite with milk-cream has an effective role on healing of second-degree burn in rat skin and it could a complementary and/or alternative medicine in wound healing.Through a presented case scenario, this article describes the assessment and management of the commercial sexual exploitation of children (CSEC), which includes sex trafficking, as relevant for the school nurse. CSEC is a major public health problem globally, nationally, and locally, even infiltrating school hallways. We analyzed tissues from 141 ice-associated seals collected in the Alaskan Bering and Chukchi Seas during 2006-15 for internal helminth parasites and compared results with past studies. Specimens were collected from ringed seals (Pusa hispida), bearded seals (Erignathus barbatus), spotted seals (Phoca largha), and ribbon seals (Histriophoca fasciata).Helminths were present in 94% (133/141) of the seals sampled. Nematodes were most prevalent in bearded (97%, 72/74) and spotted seals (93%, 13/14). Cestodes were most prevalent in bearded seals (82%, 61/74) and absent in ribbon seals, trematodes were only found in bearded (64%, 47/74) and ringed seals (5%, 2/44), and acanthocephalans were mostly found in ringed (61%, 27/44) and spotted seals (64%, 9/14). Although no helminths were new to the Bering-Chukchi Seas region, this study found a previously unreported host record for the lungworm Parafilaroides (Filaroides) gymnurus in a ribbon seal.
