To provide high-quality healthcare, it is essential to understand values that guide the healthcare decisions of older adults. We investigated the types of values that culturally diverse older adults incorporate in medical decision making.

Focus groups were held with older adults who varied in cognitive status (mildly impaired versus those with normal cognition) and ethnicity (Hispanic and non-Hispanic). Investigators used a qualitative descriptive approach to analyze transcripts and identify themes.

Forty-nine individuals (49% with cognitive impairment; 51% Hispanic) participated. Participants expressed a wide range of values relating to individual factors, familial/cultural beliefs and expectations, balancing risks and benefits, receiving decisional support, and considering values other than their own. Participants emphasized that values are individual-specific, influenced by aging, and change throughout life course. Participants described barriers and facilitators that interfere with or promote value solicitation and incorporation during medical encounters.

Study findings highlight that in older adults with various health experiences, cognitive and physical health status, and sociocultural backgrounds, medical decisions are influenced by a variety of values.

Diabetic ketoacidosis (DKA) has increased in prevalence in the emergency department (ED) in recent years. The complications of DKA are life threatening and necessitate rapid identification and management. Pediatric complications include cerebral edema, venous thrombosis, acute kidney injury, and severe infections including necrotizing fasciitis and mucormycosis. Rhinocerebral mucormycosis carries a high mortality rate and requires early treatment with antifungals and surgical debridement.

A 16-year-old boy with no significant past medical history presented to the ED with new-onset DKA complicated by hypothermia, hyperosmolar hyperglycemic state, cerebral edema, and multifactorial shock. This study aimed to compare the therapeutic efficacy of resection (RES) and microwave ablation (MWA) for hepatocellular carcinoma (HCC) within the Milan criteria.

Between 2011 and 2019, 426 HCC patients within the Milan criteria were treated at our institution (RES n=291; MWA n=135). We compared overall survival (OS), disease-free survival (DFS), complications, and hospital stay in these patients using propensity score matching (PSM) and determined the prognostic factors using multivariate Cox analysis.

Following 11 matching using PSM, 121 patients were matched in each group. The 1-, 3-, and 5-year OS rates were 98.3%, 84.7%, and 69.6% for the MWA group and 96.5%, 81.8%, and 78.1% for the RES group (p=0.667). The corresponding DFS rates for the MWA and RES groups were 81.8%, 54.4%, and 42.3% and 85.4%, 67.8%, and 57.9%, respectively (p=0.174). The MWA group had less blood loss and shorter hospital stays (both p<0.001) than the RES group.

We hypothesise that the unusual PPS representation observed in SCZ is caused by an imbalance of excitation and inhibition (E/I) in recurrent synapses of unisensory neurons or an impairment of bottom-up and top-down connectivity between unisensory and multisensory neurons. These hypotheses were tested computationally by manipulating the effects of E/I imbalance, feedback weights and synaptic density in the network. Using simulations we explored the effects of such impairments in the PPS representation generated by the network and fitted the model to behavioural data. We found that increased excitation of sensory neurons could account for the smaller PPS observed in SCZ and H-SPQ, whereas a decrease of synaptic density caused the sharp definition of the PPS observed in SCZ. We propose a novel conceptual model of PPS representation in the SCZ spectrum that can account for alterations in self-world demarcation, failures in tactile discrimination and symptoms observed in patients.

When the strong occlusal pressure was applied on the maxillary first premolar by clenching, GBF in the attached gingiva on the buccal side decreased significantly compared with the resting GBF, with medians of 2.3mL/min/100g and 5.4mL/min/100g, respectively (P<0.05). After the release of the maximum clenching, GBF recovered immediately and transiently increased to a median of 2.4mL/min/100g, showing a significant difference to the resting GBF (P<0.05). In contrast, in the interdental papilla, no significant change in GBF was found by clenching.

YouTube was searched using the keyword "orthodontic retainers" from Google Trends. From the first 120 results, 97 videos were selected to be analyzed. Videos were rated for quality by video information and quality index (VIQI) and the global quality scale (GQS). Videos were classified as low- and high-content groups using a 10-point score considering several retainer-related topics. Mann-Whitney U tests, chi-square tests, and Pearson correlation coefficients were used for statistical evaluations.

Most YouTube videos were uploaded by dentists/specialists (62.9%). We classified 12 videos as high content and 85 as low content. Instructions on the use of the orthodontic retainer was the most commonly covered topic (42.3%), followed by the effect on oral hygiene (38.1%), definition (37.1%), procedure of preparing orthodontic retainers (33.0%), and procedure of positioning (30.9%). A lower percentage of videos (6.2%) mentioned soft tissue irritation. The high-content video group had a significantly higher median value of GQS score and VIQI (P<0.001). The correlation between GQS and VIQI was strong (r=0.698; P<0.01).

This randomized clinical trial aimed to compare the skeletal and dental effects of miniscrew-anchored maxillary protraction (MAMP) using hybrid hyrax (HH) and conventional hyrax (CH) expanders in growing patients with Class III malocclusion.

This was a randomized, parallel, controlled trial. Forty growing patients with Class III malocclusion and maxillary deficiency (Wits appraisal of less than -1mm) were randomized into 2 groups. Patients were recruited at the Orthodontic Clinic of Bauru Dental School, University of São Paulo, Brazil. The HH group was composed of patients with Class III malocclusions in the late mixed or early permanent dentition treated with a HH expander with 2 miniscrews in the maxilla and 2 miniscrews in the anterior region of the mandible. Class III elastics were used from the maxillary first molars to the mandibular miniscrews placed between permanent canines and first premolars. The CH group was composed of patients treated with a similar protocol except for the use of a CH expander in the maxilla.
