Composite pheochromocytomas with a non-pheochromocytoma component stemming from an embryological origin other than the neural crest comprise a unique entity. We present a patient diagnosed with a composite pheochromocytoma with spindle cell sarcoma, which is, to our knowledge, the first case reported in the current literature. A 45-year-old female patient with elevated blood pressure had undergone adrenalectomy for clinically and radiologically diagnosed pheochromocytoma. Macroscopic examination of the specimen showed an encapsulated mass containing two nodules. One tumor was composed of large polygonal neoplastic cells with round vesicular nuclei and granular cytoplasm forming the characteristic Zellballen pattern consistent with pheochromocytoma, while the other was composed of spindle cells with hyperchromatic nuclei and prominent nucleoli, thus resembling a sarcoma. The patient was discharged on the fourth post-operative day without any complications. Even though for both tumors surgical resection is the main treatment modality, our case report aims to shed light on and discuss the etiology and management of a rarely presented composite pheochromocytoma and spindle cell sarcoma.Embryos are diagnosed as mosaic if their chromosomal copy number falls between euploid and aneuploid. The purpose of this study was to investigate the impact of mosaicism on global gene expression. This study included 42 blastocysts that underwent preimplantation genetic testing for aneuploidy (PGT-A) and were donated for IRB approved research. Fourteen blastocysts were diagnosed as mosaic with Next-generation Sequencing (NGS). Three NGS diagnosed euploid embryos, and 25 aneuploid embryos (9 NGS, 14 array Comparative Genomic Hybridization, 2 Single Nucleotide Polymorphism array) were used as comparisons. RNA-sequencing was performed on all of the blastocysts. Differentially expressed genes (DEGs) were calculated using DESeq2/3.5 (R Bioconductor Package) with p  less then  0.05 considered significantly differentially expressed. Pathway analysis was performed on mosaic embryos using EnrichR with p  less then  0.05 considered significant. With euploid embryo gene expression used as a control, 12 of 14 mosaic embryos had fewer DEGs compared to aneuploid embryos involving the same chromosome. On principal component analysis (PCA), mosaic embryos mapped separately from aneuploid embryos. Pathways involving cell proliferation, differentiation, and apoptosis were the most disrupted within mosaic embryos. Mosaic embryos have decreased disruption of global gene expression compared to aneuploid embryos. This study was limited by the small sample size, lack of replicate samples for each mosaic abnormality, and use of multiple different PGT-A platforms for the diagnosis of aneuploid embryos.The amplitude of the coronavirus disease 2019 (COVID-19) pandemic motivated global efforts to find therapeutics that avert severe forms of this illness. After their structures have been confirmed by several techniques, such as 1H NMR, 13C NMR, FT-IR, UV-Vis, and MALDI TOF analyses, their luminescence behavior was investigated in ethanol and PBS (physiological medium, pH ~ 7.4) solutions. Emission spectra of both complexes are dominated by 3MLCT states at room temperature in ethanolic solution, but at 77 K the Ir-q exhibits the 3LC as the dominant emission state. The Ir-q complex shows one of the highest emission quantum yields, 11.5%, for a red emitter based on iridium(III) complexes in aerated PBS solution, with color coordinates (x;y) of (0.712;0.286). Moreover, both complexes display high potential to be used as a biological marker with excitation wavelengths above 400 nm, high water solubility (Ir-p 1838 μmol L-1, Ir-q 7601 μmol L-1), and distinct emission wavelengths from the biological autofluorescence. Their cytotoxicity was analyzed in CHO-k1 cells by MTT assays, and the IC50 was estimated as being higher than 131 μmol L-1 for Ir-p, and higher than 116 μmol L-1 for Ir-q. Concentrations above 70% of viability were used to perform cell imaging by confocal and fluorescence microscopies and the results suggest that the complexes were internalized by the cell membrane and they are staining the cytoplasm region.This systematic review and meta-analysis assessed the antidiabetic effect of pharmaconutrients as an add-on in type 2 diabetes mellitus patients by pooling data from currently available randomized controlled trials (RCTs). To compare the incidence, timing, and microbiologic factors associated with late spinal infection (onset ≥ 6months after index operation) in pediatric versus adult spinal deformity patients who underwent instrumented posterior spinal fusion (PSF).

We retrospectively queried our institutional database for pediatric (aged ≤ 21years) and adult patients who underwent instrumented PSF from 2000 to 2015. Inclusion criteria were > 12-month follow-up, spinal arthrodesis spanning 4 or more levels, and idiopathic or degenerative spinal deformity. We included 1260 patients (755 pediatric, 505 adult). Incidence, timing, and microbiologic and operative parameters of late spinal infections were compared using chi-squared and Fisher exact tests. Alpha = 0.05.

Late spinal infection occurred in 28 (3.7%) pediatric and 2 (0.39%) adult patients (p = 0.009). Mean onset of infection was 4.2years (range 0.7-12) in pediatric patients and 4.0years (range 0.7-7.3) in adults (p = 0.93). Pediatric patients underwent arthrodesis spanning more levels (mean ± standard deviation, 10 ± 2.0) compared with adults (8.4 ± 3.3) (p < 0.001). Adults experienced greater intraoperative blood loss (2085 ± 1491mL) compared with pediatric patients (796 ± 452mL) (p < 0.001). Culture samples yielded positive growth in 11 pediatric and 2 adult cases. Propionibacterium and coagulase-negative staphylococci were the most commonly detected microorganisms in both cohorts.

Late spinal infections were significantly more common in pediatric patients than in adults after instrumented PSF for spinal deformity. Skin and indolent microorganisms were the primary identifiable causative bacteria in both cohorts.

In 2018, a regional health authority in British Columbia (BC) initiated a multi-year project to support planning and response to extreme heat. Climate projections indicate that temperatures in the southern interior of BC will continue to increase, with concomitant negative impacts on human health. Successful climate change adaptation must include cross-sectoral action, inclusive of the health sector, to plan for and respond to climate-related events, including extreme heat.

The objective of this project was to support the development and implementation of a Heat Alert and Response System (HARS) in a small, rural community. The health authority facilitated collaboration among provincial and local governments, community organizations, and First Nations partners to assess community assets, draft a plan for extreme heat, and prepare for a community-supported response during heat events.

Stakeholders expressed the importance of utilizing existing partnerships and community resources, such as physical and pronforming similar HARS planning processes in other rural BC communities.Photodegradation of organic pollutants is the most suitable and cheaper technique to counter decontamination issues. Among the metal-based nanoparticles, TiO2 is considered to be the most effective heterogeneous photocatalyst for the photodegradation of organic pollutants. However, the large band gap and the high electron-hole pair recombination rate limit its practical applications. Herein, an approach was introduced to minimize the mentioned limitations by preparing CdTiO2 and ZnCdTiO2 nanocomposites by co-precipitation method. The as-synthesized TiO2, CdTiO2, and ZnCdTiO2 were characterized by scanning electron microscopy, transmission electron microscopy (TEM), energy dispersive X-ray, X-ray diffraction (XRD), Brunauer-Emmett-Teller (BET), and UV-visible spectrophotometry. Morphological analysis revealed that TiO2 are mostly agglomerated with different shapes and sizes and the nanocomposites are mostly in dispersed form. The components of the nanocomposites are strongly intercalated in the ternary nanocomposite as confirmed from TEM analysis. XRD analysis confirmed the anatase TiO2 while the UV-visible analysis showed the shifting toward higher wavelength. The band gap energy of TiO2 (2.65) decreased to 2.6 and 2.56 eV for CdTiO2 and ZnCdTiO2, respectively. BET analysis has shown a 47.2 m2/g surface area for the ternary ZnCdTiO2 nanocomposite. The photodegradation results revealed that TiO2, CdTiO2, and ZnCdTiO2 degraded about 74%, 86%, and 97.61% methylene blue dye, respectively, within 2 h. Maximum photodegradation is achieved in the basic medium and the ternary ZnCdTiO2 nanocomposite degraded 98% dye at pH 10.
