All rights reserved.OBJECTIVES To identify predictors of poor overall survival (OS) among penile squamous-cell carcinoma (pSCC) patients with clinical inguinal lymphadenopathy (cN+) in order to define the best candidate for neoadjuvant chemotherapy (NAC). MATERIALS AND METHODS Using an international, multicenter database of 924 pSCC patients, we identified 334 cN+ men with available clinical and follow-up data. Lymph node involvement was defined either by the presence of palpable inguinal node disease or by preoperative CT assessment. 18F-FDG-PET/CT-scan was performed based on clinical judgment of the treating physician. Regression-tree analysis generated a risk stratification tool for prediction of 24-month overall mortality (OM). Kaplan-Meier explored the OS benefit related to the use of NAC according to the regression-tree stratified sub-groups. RESULTS Of all, 120 (35.9%), 152 (45.5%), and 62 (18.6%) harbored cN1, cN2, and cN3 disease. 18F-FDG-PET/CT was performed in 48 (14.4%) patients, and 16 (4.8%) had inguinal & pelvic nodal PET detection. Median OS was 107 months, with 24-month OS of 66%. At regression-tree analysis (area under the curve = 70%), patients with cN3 and cN2 with PET/CT-detected inguinal & pelvic nodal activity had the higher risk of 24-month OM (>50%). NAC was associated with improved 24-month OS rates (54% vs. 33%) only in this sub-group of patients (p=0.002), and it was also confirmed after multivariable adjustment (HR 0.28, 95% CI 0.13-0.62, p=0.002). CONCLUSION pSCC patients with cN3 or cN2 and inguinal & pelvic 18F-FDG-PET/CT-scan detected disease exhibited higher 24-month OM rates according to our regression-tree model. Neoadjuvant chemotherapy was associated with improved OS only in these subgroups of patients. All rights reserved.Intracoronary application of nicorandil can effectively reduce the myocardial no-reflow (MNR) after percutaneous coronary intervention (PCI). We sought to investigate the mechanisms of nicorandil in preventing MNR, besides that of dilating the coronary microvasculature. A total of 60 patients undergoing PCI were enrolled and randomly divided into a nicorandil group and a control group. Before PCI, 2 mg of nicorandil or an equal volume of normal saline was injected into the coronary artery. Blood samples were collected before, 24 hours and 1 week after PCI and inflammatory cytokines were tested. In the control group, the expression of pro-inflammatory cytokines was significantly increased, while the anti-inflammatory cytokines were decreased 24 hours after PCI. In contrast, these changes were reversed in the nicorandil group, indicating that nicorandil regulated the inflammatory response induced by PCI. Then, proteomic analysis was performed to further elucidate the potential mechanisms. A total of 53 differentially expressed proteins (DEPs) were found 24 hours after PCI in the control group, and the changes of these relevant genes were reversed in the nicorandil group. read more These DEPs were significantly enriched in the inflammatory pathways. Although knowledge on the peculiarities of testing and assessing fate and effects of nanomaterials in the environment strongly increased in the last years, uncertainties how to perform a reliable and robust environmental risk assessment for nanomaterials still remain. These uncertainties are of special relevance in a regulatory context, challenging both industry and regulators. This paper presents current challenges in regulatory hazard and exposure assessment under REACH as well as classification of nanomaterials and makes proposals to address them. Still, the nano-specific considerations made here are expected to be also be valid for environmental risk assessment approaches in other regulations of chemical safety. Inter alia, these proposals include a way forward to account for exposure concentrations in aquatic toxicity test systems, a discussion how to account availability of dissolving nanomaterials in aquatic test systems and a pragmatic proposal to deduce effect data for soil organisms. Furthermore, it specifies how to potentially deal with nanoforms under CLP and outlines the needs for proper exposure assessments of nanomaterials from a regulatory perspective. All rights reserved.BACKGROUND Treatment decisions for metastatic spine disease are complex and depend on prognosis. Four prognostic systems in use are the Oswestry Risk Index, modified Bauer score (MBS), van der Linden score and New England Spinal Metastasis Score (NESMS). We aimed to determine the performance of these scoring systems in a New Zealand cohort of patients and develop a prognostic score specific to this demographic. METHODS A retrospective review of a patient cohort from 2009 to 2016 was undertaken. Scores and individual scoring items were evaluated with univariate and multivariate analyses. Significant items were used to design a simple, population-specific objective scoring system, which was then tested. RESULTS A total of 106 patients receiving either surgery and radiotherapy (65) or radiotherapy alone (41) were included. Mean post-treatment survival time was 13.7 months. All scoring systems were significantly correlated with survival and had similar concordances. The MBS had the largest coefficient of determination (Cox and Snell's R2 = 0.18), followed by the NESMS (R2 = 0.14). On multivariate analysis, the lung cancer (MBS) and serum albumin (NESMS) items were significant. A modified Oswestry Risk Index primary tumour item and NESMS serum albumin outperformed the MBS (R2 = 0.20), providing the basis for a prognostic scoring tool specific to our demographic. CONCLUSION Based on serum albumin and primary tumour type, we propose the 'Metastatic Spine Risk Index' as a simple and objective tool, specific to our population for predicting survival, which can be used in conjunction with other clinical information when considering treatment options. © 2020 Royal Australasian College of Surgeons.Radiotherapy has been optimised over the last decades not only through technological advances, but also through the translation of biological knowledge into clinical treatment schedules. All rights reserved.BACKGROUND Right-sided Bochdalek hernias are uncommon congenital diaphragmatic hernias that can be challenging to manage because they can contain bowel, omentum, kidney or liver. METHODS We describe our experience at a single centre and integrate this with all reported cases in the literature to evaluate how this rare problem has been managed and when a minimally invasive approach is appropriate. RESULTS A total of 31 patients were identified, four patients from our institution and 27 case reports from the literature. A minimally invasive approach was utilized in 15 of 31 patients (including two of four at our institution) and was more common if the operation was performed in the elective setting (10/12 versus 4/19). If a bowel resection was required, an open approach was more common (9/17 versus 1/14). The commonest method of repair was a primary suture repair (18/31), and the mesh was used in 13 cases, particularly in the elective setting (9/12 versus 4/19). CONCLUSION Right-sided Bochdalek hernias are an uncommon and challenging problem. These hernias can contain bowel, kidney and even liver but can still be successfully repaired using minimally invasive techniques, especially in the elective setting and when an emergency bowel resection is not required. © 2020 Royal Australasian College of Surgeons.AIMS It is well known that tight glycaemic control reduces all-cause mortality and the development of microvascular complications in patients with type 1 diabetes mellitus (T1D), but that effective glycaemic control is difficult to achieve in different age groups. Currently, the state of glycaemic control across the lifespan in patients with T1D in New Zealand is not known. Thus, the aim of this study was to determine the differences in glycaemic control with age, gender, rurality and ethnicity in patients with T1D in the Waikato region of New Zealand. METHODS Retrospective review of clinical records of all patients with T1D on the Waikato Regional Diabetes Database in December 2017 (n = 1303). Glycaemic control was determined by the most recent HbA1c in the last two years. RESULTS Median (25%,75%) HbA1c was 67 (59,81) mmol/mol (8.3%) and highest in those aged 15-29 years.
