This case demonstrates a rare variant of idiopathic PPFE with progressive platythorax disproportionate to the extent of upper-lobe fibroelastosis.The etiology of Sjögren's syndrome (SS) remains unclear and is associated with many other autoimmune diseases. In particular, symptoms of SS are not apparent when steroids are already being administered for other autoimmune diseases. This report documents a case of autoimmune hepatitis with SS, which was diagnosed on the basis of the worsening of unilateral pleural effusion after the discontinuation of steroids as well as the manifestation of symptoms, such as dry mouth. Adrenal insufficiency due to the discontinuation of steroids was assumed to be the cause of the worsening, rather than infection stress, because no indicators of infectious diseases were observed. The diagnosis of SS was confirmed via lip biopsy examination and anti-SS antibody positivity. Re-administration of steroids rather than antibiotics drastically reduced the pleural effusion and improved the dry mouth symptom. SS with pleural effusion in a case of autoimmune disease was reported to show both unilateral and bilateral pleural effusion predominantly containing lymphocytes. SS with pleural effusion may be more common than expected and should be differentiated from traditional SS. Moreover, biopsy examination should be considered if necessary because the condition might remain latent when steroids are administered.Pleural mesothelioma is a disease with a very poor prognosis. Here, we report a mesothelioma patient who survived for 5 years and a half. As a result of the autopsy, the tumor was diagnosed as a myxoid variant, which is internationally proposed as a histological subtype of epithelioid mesothelioma with a relatively favorable prognosis. Since patients with this disease are expected to survive for a long period even without treatment, careful determination of the therapeutic approach is considered necessary. This report is considered to be the first of a myxoid variant epithelioid pleural mesothelioma in Japan.
The gold standard for reconstruction is the microvascular free flaps, whereas pedicled flaps are generally employed as a rescue procedure. Pectoralis major myocutaneous flap (PMMC) flap being a reliable flap is associated with high rate of complication. So, the purpose of this study was to check and assess the predictability of PMMC Flap and its clinical outcome in 168 Indian patients.

We conducted a retrospective study of PMMC flap which was harvested in 168 Indian oral cancer patients in 3 years time span. Patients were kept on follow up for 1-3 years. Rate of complication were documented for flap necrosis and wound dehiscence; and patient related data like neck bulk acceptance and range of motion were recorded. Gender comparison of complications were also documented.

The most common complication was wound dehiscence in 11 (6.5%) patients. Partial flap necrosis was observed in 3 (1.8%) female patients. There was no case of total flap necrosis. Questionare method was used for patients to record neck bulk acceptance. Range of motion was evaluated during follow up period. Follow up period of all the patients was 1-3 years.

Clinical trials had concluded a significant relationship between implant failure and bruxism. A three-dimensional (3D) finite element analysis (FEA) was done to evaluate the stress distribution in straight and angled abutments around titanium and carbon fiber-reinforced polyetheretherketone (CFR-PEEK) implant with 2 different prosthetic crowns under parafunctional loading.

Twelve 3D models of bone block were created representing the maxillary right premolar area with osseointegrated implants. The models were divided in two group; CFR-PEEK implant (n=6) and group titanium implant (n=6).Each group was subdivided based on implants with three different abutments (straight, 15°, 25° angled abutments) and having two different prosthetic crowns porcelain fused to metal (PFM) and polyetheretherketone (PEEK). A vertical load of 1000N was applied to the central fossa and an oblique load of 500N (30°) was applied to the buccal incline of the palatal cusp. The von Mises stresses and principal stresses were analyzed using ANSYS software.

CFR-PEEK and titanium implants produced similar stress in bone under vertical and oblique loading. Straight abutment showed better results than 15° and 25° angled abutments in all the groups. PEEK crown produced lesser stress than PFM crown under vertical and oblique loading.

The study concluded that straight abutment along with PEEK crown could be given in patients with bruxism to reduce the stress concentration in bone, thus preventing possible implant failure. Titanium and CFR-PEEK implants with straight abutments if given, then it should be provided with an occlusal splint.

Genotypic diversity and polymorphism of Streptococcus mutans remain as a topic of interest, as this provides a satisfactory explanation for the contrasting caries status in people harboring Sterptococcus mutans. This systematic review aimed to analyze and appraise Genomic diversity of Streptococcus mutans and its relationship in Early Childhood Caries.

A literature search was conducted until August 2020 using PubMed, Cochrane central, SIGLE using search keywords and MeSH headings include "Early Childhood caries", "Streptococcus mutans" and "Genotypic diversity". Apart from the aforementioned electric search, manual search was also done. The retrieved papers were assessed by three review authors and the disagreements were discussed and the final decision was made by consensus. Qualities of the included studies were assessed and data were extracted for analysis.

A total of 4081 papers were retrieved for screening after the removal of duplicate articles. Among the retrieved studies, 4074 papers did not satisfy the study inclusion criteria. Finally, those 7 papers which fulfilled all the inclusion criteria were selected and reviewed systematically.

Clinical trials have not demonstrated significantly increased infection risk with ustekinumab. We report a case of disseminated histoplasmosis in the setting of ustekinumab and methotrexate following a hike in the Catskill Mountains, a region not commonly associated with Histoplasma encapsulatum. To our knowledge, this is the first reported case of newly acquired histoplasmosis complicating treatment with ustekinumab.A 61-year old lady with poorly-controlled type 2 diabetes mellitus was diagnosed with rhino-orbital-cerebral mucormycosis following presentation with sinusitis, ophthalmoplegia, proptosis and facial numbness. She was treated successfully with aggressive surgical intervention including orbital exenteration, accompanied by anti-fungal therapy with liposomal amphotericin B and posaconazole, followed by isavuconazole as salvage therapy. We discuss the challenges around optimising antifungal therapy of this lethal infection in the context of hepatic and renal toxicity.
