The decision to prescribe osteoporosis medications for a patient on the basis of fracture risk may depend on which risk calculator is used. Pentylenetetrazol antagonist Differences in the calculator methods contribute to the discrepancy between them.
Worldwide, low back pain (LBP) is the leading cause of disability and affects 16% of the Australian population. Pain that lasts more than 12 weeks and is presumed lumbar musculoskeletal in origin is deemed non-specific chronic LBP. Managing LBP requires a multidisciplinary approach.

The aim of this article is to provide evidence for incorporating exercise training into the multidisciplinary treatment plans of patients with non‑specific chronic LBP.

There is mounting evidence in support of the integration of exercise training for the multidisciplinary treatment of non specific chronic LBP. Clinically meaningful reductions in pain intensity may be achieved with Pilates, aerobic, stabilisation/motor control and resistance exercise training. Clinically meaningful reductions in disability may be attained with resistance, stabilisation/motor control, water-based, Pilates and yoga exercise. Resistance and aerobic exercise can also improve mental health in this susceptible population group. The evidence suggests it is reasonable to consider including exercise-based services in the care team to enable patients with non-specific chronic LBP to transition to self-management.

Idiopathic rotational abnormalities of the lower limb have been studied extensively in children through to adulthood, but there remains concern among parents and the community whether a normal variant is pathological.

The aim of this article is to draw attention to deformities in the transverse plane and to highlight the variation in normal over time, and according to sex.

Rotational malalignment of the lower limb can be a rare cause of pain in the hip, knee, ankle and foot. Understanding common presentation and physical examination findings is the first step in diagnosing and, ultimately, managing symptomatic rotational abnormalities in children and adolescents.

Transient vision loss may indicate underlying disease of the eye, optic nerve, orbit, brain or heart. Detailed history-taking followed by a complete ocular and neurological examination is therefore a crucial part of any consultation.

It is important to determine whether a patient with transient vision loss can be reassured or requires urgent referral for further investigation. This review examines monocular and binocular transient vision loss and provides a structured approach to the examination of a patient with transient vision loss. Theaim of this article is to provide clinicians with confidence when encountering these patients.

Transient vision loss can imply serious underlying pathology; therefore, accurate history-taking and astute observation are paramount. link2 This review discusses the differential diagnosis of monocular and binocular transient vision loss and the relevant localising features of each.

Corticosteroids are adrenal steroid hormones responsible for immune homeostasis, blood pressure regulation, energy and appetite regulation, and suppression of inflammation. Oral corticosteroids ('steroids') are commonly used in children for a number of medical conditions, including asthma, croup, inflammatory bowel disease and nephrotic syndrome. There has been some concern about potential side effectsof oral, topical or inhaled steroids, including reduction in growth, weight gain, behavioural changes and immunosuppression resulting ininfection.

While many recent studies have assessed individual risks, this article aims to provide an overview of steroid use in common paediatric presentations, highlighting current management and risks associated with recurrent doses of steroids in the paediatric population.

Short courses of oral steroids (less than two weeks) in children are very unlikely to cause long-term side effects in children. Children requiring courses more than two weeks' duration warrant specialist referral and a weaning plan to reduce adrenal suppression and insufficiency.

Gilles de la Tourette syndrome (GTS), characterised by motor and vocal tics, has a prevalence of approximately 1% inschool-aged children. Commonly encountered comorbidities of GTS include attention deficit hyperactivity disorder (ADHD) and obsessive-compulsive behaviour/disorder (OCB/OCD). Genetic factors play an important part in the aetiology of GTS, and family members may exhibit tics or related disorders such as ADHD, OCB or OCD.

The aim of this article is to present a summary of the current evidence to assist the assessment and management of GTS in primary care.

A comprehensive assessment should include exploration of not only tics butalso associated features and comorbidities. The stigmatising and impairing nature of tics can have a significant impact on the quality of life ofthe young person and their parents/carers, as well as on family functioning. Management includes education and explanation, behavioural treatments and(sometimes) medication.

Attention deficit hyperactivity disorder(ADHD) is the most common neurodevelopmental disorder and is estimated to affect approximately 10% ofchildren and 5% of adults. It is associated with impaired functioning. Recognition and appropriate treatment can make a substantial difference.

The aim of this article is to provide a framework for recognising ADHD and the ways that it can present at different stages of the lifespan.

General practitioners have a vital role inidentifying people who may have undiagnosed ADHD. It is important totake a history targeted at eliciting problems in everyday functioning. People who are intellectually able may have developed coping strategies that mask their ADHD; in those with other psychiatric diagnoses, the underlying ADHD may be overlooked. Timely and effective treatment can make an enormous difference to the individual and to their family and peers and can reduce the likelihood of subsequent development of secondary mental health and social issues.

In Australia, preschoolers are being identified and diagnosed as autistic.

The aim of this article is to describe the different paths preschool children and their families can take from identification of developmental or behavioural concerns to ongoing support, intervention and healthcare.

There are many ways in which general practitioners, working alongside other professionals and with relevant services, can assist each child and family.

The health and wellbeing of Autistic people are well below those of their non-Autistic counterparts. With studies showing a reduced life expectancy and poor physical and mental health, it is important that healthcare professionals strive towards improving the lives of Autistic people by recognising those who are undiagnosed and/or misdiagnosed and providing meaningful support.

The aim of this article is to empower general practitioners (GPs) to feel comfortable in recognising potentially undiagnosed Autistic adults. The authors aim to further the understanding of the Autistic experience, including frequently co-occurring physical and mental health conditions, and to give GPs some tools tomake the experience of accessing healthcare less problematic for Autisticpeople.

The healthcare experiences of Autistic adults will be greatly improved with aknowledgeable, understanding and accepting GP as the cornerstone of their care.

While large publicly available datasets of chest radiographs and free-text radiology reports exist, only limited numerical edema severity labels can be extracted from radiology reports. link3 This is a significant challenge in learning such models for image classification. To take advantage of the rich information present in the radiology reports, we develop a neural network model that is trained on both images and free-text to assess pulmonary edema severity from chest radiographs at inference time. Our experimental results suggest that the joint image-text representation learning improves the performance of pulmonary edema assessment compared to a supervised model trained on images only. We also show the use of the text for explaining the image classification by the joint model. To the best of our knowledge, our approach is the first to leverage free-text radiology reports for improving the image model performance in this application.
