Introduction Dengue fever (DF) is the most common arbovirus-related infection worldwide. Most of the dengue infections are asymptomatic. The clinical presentation of DF varies from mild febrile illness to dengue hemorrhagic fever (DHF) and dengue shock syndrome (DSS). Both DHF and DSS are severe forms of illness and carry higher rates of mortality. Various clinical parameters are associated with severe DF. The objective of this study was to determine any relation between elevated alanine aminotransferase (ALT) levels at presentation and development of severe DF. Methods This cross-sectional study was carried out at a large tertiary care hospital in Rawalpindi, Pakistan. Over a period of six months, 65 dengue patients were enrolled and their ALT levels were recorded at presentation. learn more All the patients were managed as per guidelines in a similar way, and clinical course was followed for the development of severe forms of DF, such as DHF and DSS. Results Out of total 65 patients, 45 (69.2%) were males and 20 (30.8%) were females. Classical DF was present in 18 (27.7%) patients, whereas 47 (72.3%) patients developed DHF or DSS. Patients with DF had a mean ALT level of 131.67 (±244.48) U/L at presentation, whereas patients with DHF and DSS had a mean ALT level of 228.15 (±467.88) U/L at presentation. The Mann-Whitney U test was applied to compare differences between ALT levels of both groups, and p value was found to be 0.018 ( less then 0.05). Thus, it was concluded that there was a statistical difference between ALT levels of patients with simple DF and patients with DHF or DSS. Conclusion Higher serum levels of ALT at presentation can give a clue about possibility of progression to severe forms of DF (DHF and DSS). Therefore, patients with higher ALT at presentation should be prioritized and monitored more rigorously.The premasseteric branch of the facial artery is a variable posterior branch that is closely associated with the anterior border of the masseter muscle. Further workup revealed a mass in the upper vagina on imaging studies, and the patient eventually underwent a biopsy, which confirmed the diagnosis of malignant melanoma of the vagina on pathological examination.Globally, gastroesophageal reflux disease (GERD) has a potentially high prevalence, with a wide rate of variability across different populations due to inconsistency in the risk factors. Hence, a cross-sectional study was conducted using a self-administered, structured questionnaire at Al-Iskan Primary Health Care (PHC) Center to investigate the prevalence rate and associated factors in Makkah Almukarramah, the west of Saudi Arabia. The study included 339 participants. The mean age for participants was 39.5 ± 15.5 years, and the range was from 18 to 84 years. Male participants were 247 (72.9%) and females were 92 (27.1%). Participants were diagnosed with GERD on achieving a GERD questionnaire score of ≥8. In our sample, 59 (17.4%) were diagnosed with asymptomatic GERD. The represented logistic regression shows that family history of GERD, marital status (divorce), smoking, physical activity frequency, tomatoes/tomato-based foods, salty foods, acidic drinks, frequent use of analgesics, and nonsteroidal anti-inflammatory drugs (NSAIDs) consumption shows statistical significance and association (P less then 0.05) with increased risk of symptomatic GERD. Conclusively, the results show that GERD is prevalent in Makkah with the presence of modifiable risk factors which can significantly affect the ascendency of the disease.Blunt chest trauma (BCT) has become increasingly more prevalent in recent years. As a result, the incidence of blunt cardiac injury (BCI), or cardiac or myocardial contusion, has also increased. The sequelae of BCI often are undiagnosed due to variability in the clinical presentation. This case highlights a transient right bundle branch block (RBBB) following a motor vehicle accident (MVA), resulting in BCI. Right-sided cardiac injuries predominate BCI owing to the anterior location of the right ventricle within the thoracic cage; however, the pathophysiologic mechanisms underlying the electrocardiographic manifestations are vaguely understood. In this case, a 66-year-old female sustained a BCI resulting in a transient RBBB. The patient fully recovered following a three-day hospitalization with complete recovery of normal cardiac conduction.Background Short-term medical missions (STMMs) are a highly debated and largely understudied form of international volunteer work. With growing dedication to health care abroad, research evaluating their impact is crucial to ensure continued interventions are effective in improving medical care. STMM care varies in length, frequency, size, location, services offered, and country of origin and destination. This makes systematic evaluation of STMMs difficult. In addition, the transient nature of patient visits makes trending STMM's impact on long-term health outcomes complex. Despite intermittent availability, primary care missions offering pharmaceutical supplies have the unique opportunity to provide continued care to the community via free prescription supplies each visit. Given the challenges with measuring long-term outcomes in this population, it is unknown if these donated medications have any impact on patient health outcomes. As medication noncompliance is known to hinder health outcomes, our study chose to evaluate patient medication habits to see if these prescription supplies were being utilized appropriately.
