3 injections (up to 1 year). The development of new technologies in medicine lead to creating an optimal treatment methods of cystic liver echinococcosis, based on minimally invasive technologies, such as percutaneous puncture-aspiration echinococcectomy (PAIR) under ultrasound (US) guidance. Aim of this study is to analyze and evaluate the efficiency of PAIR treatment. The study material included a retrospective analysis of 33 patients who underwent PAIR from December 2017 to December 2019. Of them, 25 (75%) patients with primary CLE, 8 (24%) patients with recurrent cystic liver echinococcosis. Cystic liver echinococcosis was staged according to WHO classification by US images. All patients pre- and postoperatively received therapy with Albendazole. The mean duration of surgery was 41 (30-70) min. Hospital stay times were ranged from 2 and 16 days (mean 4,8 days) in all patients. The conversion to laparotomy during the procedure was not necessary. Operation was finished in 3 cases due to developing anaphylactic shock during operation. There was no fatal outcome in any cases. One patient developed bilio-cystic communication, resolved spontaneously without additional treatment. The use of minimally invasive PAIR intervention followed by Albendazole therapy has a positive effect on reducing the patient's hospital stay.On October 2, 2020, by Novo Nordisk initiative, an expert council was held in Tbilisi, Georgia, dedicated to the problems of introducing innovative insulin therapy on the example of insulin degludek into the clinical practice. The council chaired by prof. R. Kurashvili included seven leading endocrinology experts in Georgia and a specialist in the field of pharmaco-economics from Russian Federation Prof. A. Kulikov. During the expert council, important scientific, clinical and economic emphases were made on the benefits of using insulin degludec associated with its unique structure and mechanism of action. Special emphasis was placed on the difficulty in managing children and adolescents with type 1 diabetes. check details The meeting provided compelling evidence that insulin degludec reduces the incidence of hypoglycemia and diabetic ketoacidosis, thereby contributing to overall health and the prevention of vascular complications in children and adolescents with diabetes. The members of the council resolved to petition the Georgian Ministry of Health to include insulin degludek in the state program for children and adolescents with diabetes. The purpose of this meta-analysis was to investigate the influence of cardiorespiratory fitness (CF) levels on dementia risk and dementia mortality. MEDLINE and EMBASE databases were used to search for eligible studies from January 1990 to September 2019. To be included, the study was required to be a prospective cohort study that provided CF measurements and indicated relative risk and confidence intervals for the associations between CF and dementia risk and mortality. A total of six studies were selected for this meta-analysis. Low-level CF was associated with nearly three times greater risk of dementia (2.93, 95% confidence interval [1.31, 6.57]; p less then .05) compared with a high-level CF. Enhanced CF levels decreased the risk of dementia, and an increase of one metabolic equivalent of task in the CF level reduced the risk of dementia and dementia mortality. link2 Maintaining more than 12 metabolic equivalents of task of CF level was required to substantially decrease dementia risk and dementia mortality. The objective of this study was to understand the experiences of people living with dementia and their informal carers' taking part together (in dyads) in Tai Chi classes and the aspects influencing their adherence. Dyads' experiences of taking part in Tai Chi classes for 20 weeks within the TACIT Trial were explored through class observations (n = 22 dyads), home-interviews (n = 15 dyads), and feedback. Data were inductively coded following thematic analysis. Tai Chi classes designed for people with dementia and their informal carers were enjoyable and its movements, easy to learn. Facilitators of participants' adherence were the socializing component and their enjoyment of the classes, whereas unexpected health problems were the main barrier. Finding the optimal level of challenge in the class setting might be crucial for people with dementia to feel satisfied with their progression over sessions and enable their continued participation.Momentary feelings of pleasure and purpose can be sources of intrinsic motivation, but momentary purpose is rarely studied. Activities, contexts, and feelings of retired/semiretired adults (n = 67, aged 50-78 years) were captured using ecological momentary assessment. Participants provided 2,065 valid responses to six daily smartphone surveys for 7 days. Physical activity was measured by waist-worn ActiGraph accelerometer. Pleasure (measured by affective happiness) and purpose outcomes were regressed on activities, context, and potential confounding variables. Interactions between activities and contexts were explored. Participants were highly active 98.5% met physical activity guidelines. Sedentary activities were negatively associated with sense of purpose, especially when indoors. However, social sedentary activities were positively associated with feelings of happiness. Active, social outdoor activities were positively associated with both outcomes. Less sedentary participants experienced greater happiness and purpose during all their activities. Context matters active, social, and outdoor activities seem to be more appealing to older adults.
The World Health Organization has released the first global public health guidelines on physical activity and sedentary behavior for people living with disability. This paper presents the guidelines, related processes, and evidence, and elaborates upon how the guidelines can support inclusive policy, practice, and research.
Methods were consistent with the World Health Organization protocols for developing guidelines. Systematic reviews of the evidence on physical activity for health for people living with disability were appraised, along with a consideration of the evidence used to inform the general 2020 World Health Organization guidelines.
Evidence supported the development of recommendations for people living with disability, stressing that there are no major risks to engaging in physical activity appropriate to an individual's current activity level, health status, and physical function, and that the health benefits accrued generally outweigh the risks. They also emphasize the benefits of limiting sedentary behavior.
The guidelines mark a positive step forward for disability inclusion, but considerable effort is needed to advance the agenda. This paper highlights key considerations for the implementation of the new recommendations for people living with disability, in line with the human rights agenda underpinning the Global Action Plan on Physical Activity 2018-2030 and allied policies.

The COVID-19 pandemic has transformed cancer care with the rapid expansion of telemedicine, but given the limited use of telemedicine in oncology, concerns have been raised about the quality of care being delivered. We assessed the patient experience with telemedicine in routine radiation oncology practice to determine satisfaction, quality of care, and opportunities for optimization.
Patients seen within a multistate comprehensive cancer center for prepandemic office visits and intrapandemic telemedicine visits in December 2019 through June 2020 who completed patient experience questionnaires were evaluated. Patient satisfaction between office and telemedicine consultations were compared, patient visit-type preferences were assessed, and factors associated with an office visit preference were determined.
In total, 1,077 patients were assessed (office visit, n=726; telemedicine, n=351). The telemedicine-consult survey response rate was 40%. No significant differences were seen in satisfaction scores betgy practice, and found high patient satisfaction and confidence in their care. Optimization of telemedicine in oncology should be a priority, specifically access to audiovisual capabilities that can improve patient-oncologist communication.

Oral mucositis is a complication of cancer therapy, causing severe pain that affects oral functioning, nutrition, and quality of life, as well as therapy nonadherence or dose-limiting toxicity. Anecdotal experience has suggested that methylene blue (MB) oral rinse may be an effective and safe treatment of this oral pain.
To evaluate the efficacy and safety of MB oral rinse for the treatment of oral pain due to mucositis in patients with cancer, we retrospectively evaluated patients who experienced refractory pain despite conventional therapy.
We identified 281 patients who received MB oral rinse. Most were receiving treatment for leukemia (n=85; 30.3%) and head and neck squamous cell carcinoma (n=84; 29.9%). The most common treatments were radiation therapy alone (n=108; 38.4%) and chemoradiation (n=86; 30.6%). Median duration of symptoms was 14 days. Mean (SD) numeric rating scale pain scores were 7.7 (1.83; median, 8) before MB oral rinse and 2.51 (2.76; median, 2) after MB oral rinse (P<.0001). Most patients achieved pain control within the first 3 doses. The effectiveness of MB oral rinse was independent of patient age, sex, cancer type, cancer stage, MB dilution, and pain duration or baseline pain scores. The lowest response to treatment was reported in individuals with esophageal mucositis. Few patients experienced adverse effects of MB oral rinse (n=13; 4.6%); 10 had a transient burning sensation, 2 had transient blue discoloration of the teeth and mouth, and 1 had increased pain.
MB oral rinse is an effective and safe treatment for refractory pain from oral mucositis related to cancer treatment.

Successful retention strategies require consideration of social and academic institutional systems with attention to student integration in a program. This systematic review explores implemented retention strategies in nursing programs worldwide and provides guidance for nursing programs and researchers considering the retention question. Joanna Briggs Institute scoping review methods informed this review. CINAHL, ERIC, PsychINFO, and MEDLINE, databases were searched from January 1998 to December 2019. Data was extracted from 112 full text papers and dissertations. Papers were of varying quality and inconsistently evaluated, usually lacking theoretical grounding. Student participants in strategies were preselected by racial minority status or through various markers of academic performance. Retention strategies described in the literature are single program and multifactorial, with mentorship, study skills, literacy and language approaches, and tutoring the most common components.
