R-CHOP with or without radiotherapy is the standard treatment for limited-stage diffuse large B-cell lymphoma (DLBCL). To prevent overtreatment, we assessed whether four cycles of CHOP plus six applications of rituximab was adequate with negative interim PET/CT and the role of consolidation radiotherapy specifically for patients with Waldeyer's ring DLBCL. One hundred and twenty-nine patients with limited-stage DLBCL were enrolled in this open-label, nonrandomized, single-arm, phase 2 clinical trial (NCT01804127).

All patients were initially treated with 4 cycles of R-CHOP and underwent interim PET/CT. Patients with negative PET/CT (Deauville scores 1-2) received 2 additional cycles of rituximab monotherapy, unless they had any risk factors (primary mediastinal large B-cell lymphoma, extranodal primary or bulky disease). Otherwise, patients received another 2 cycles of R-CHOP. Patients with partial response on interim PET/CT received another 4 cycles of R-CHOP. No radiotherapy was conducted in Waldeyer's itted in patients with Waldeyer's ring DLBCL without compromising the efficacy. These results need to be confirmed in a randomized study.

Coxiella burnetii, the etiologic agent of Q fever, is mainly responsible for endocardite. But there are only a few cases of Coxiella burnetii-caused wound infection have been published, because the pathogen is very difficult to isolate using conventional culture methods.

A 76-year-old man, underwent endovascular repair of ruptured left iliac aneurysm plus abdominal aortic aneurysm under general anesthesia in 2018. Left iliac fossa mass resection was performed in 2020. After operation, the wound in the left iliac fossa was repeatedly ruptured and not healing. We used the wound tissue to perform the Metagenomics next-generation sequencing (mNGS), Coxiella burnetii was detected. Fasudil mouse Sanger sequencing and serologic verification of Coxiella burnetii all showed positive results.

This study proved that mNGS was an effective method to detect clinically unexplained infections, and showed the ability of pathogen identification with high sensitivity and accuracy.

Our aim was to assess SCT acceptability and utility with a survey and an institutional prospective evaluation of students' scores.With a user's online survey, we collected the opinions and satisfaction data of all graduate students and teachers involved in the SCT setting. We performed a prospective analysis comparing the scores obtained with SCT to those obtained with the national standard evaluation modality.General opinions about SCT were mostly negative. Students tended to express more negative opinions and perceptions. There was a lower proportion of negative responses in the teachers' satisfaction survey. The proportion of neutral responses was higher for teachers. There was a higher proportion of positive positions towards all questions among teachers. PCC scores significantly increased each year, but SCT scores increased only between the first and second tests. PCC scores were found significantly higher than SCT scores for the second and third tests. Medical students' and teachers' global opinion on SCT was negative. At the beginning SCT scores were found quite similar to PCC scores. There was a higher progression for PCC scores through time.Extracellular vesicles (EVs), spherical biological vesicles, mainly contain nucleic acids, proteins, lipids and metabolites for biological information transfer between cells. In this review, the mechanisms underlying MP biogenesis, factors that influence MP production, properties of MP membranes, size, composition and isolation methods of MPs are discussed. Additionally, the applications and mechanisms of action of MPs, as well as the main hurdles for their applications in cancer management, are introduced.
Most diagnosed oral cancer cases in Saudi Arabia are in the Jazan region. A common type of smokeless tobacco "Shammah" is prevalent in this region. This study aimed to gain an in-depth understanding of the possible psychosocial determinants of Shammah consumption among adult Shammah users in Jazan region.

A qualitative study was conducted by means of one-on-one interviews among thirty adult Shammah users. Participants were recruited by means of a purposive sampling technique. Data were collected using a semi-structured interview guide utilizing face-to-face and phone-call interviews. Thematic analysis with hybrid approach was used to analyze the dataset.

Twenty-four sub-codes within four overarching themes were generated. Participants revealed uncertainty related to Shammah composition, how to quit knowledge and Shammah prevention/cessation programs. Shammah use identified as a normal phenomenon in society. Its use was frequently reported in participants' close network but most users faced family and pemmah and Khat.
Shammah use was associated with uncertainty about Shammah composition and quitting knowledge, social acceptability, influence from family/friends, a range of positive and negative attitudinal beliefs toward its use and high quitting efficacy beliefs. Future interventions targeting Shammah should address the acknowledged triggers and barriers in the present study including the dual use of Shammah and Khat.Aim We assessed the effectiveness of an AHP-led rehabilitation service for patients with Covid-19 respiratory failure post-discharge from Cavan Hospital. Methods Of the 140 patients discharged from Cavan hospital with Covid-19, from March to June 2020, 3.6% required MV or HFNC. Using BTS guidelines a pathway was established. Patients underwent a 6 week post discharge telephone assessment. Of the 12 eligible patients only four agreed to participate. Assessments included mMRC dyspnoea score, Nijmegen questionnaire and HADS score. Patients completed an 8 week program including aerobic and endurance training with interval training at 10 stations. Focussed strength exercise was added as required. Results All three patients who completed the program had a real (> 20%) improvement in 6MWT, mMRCC dyspnoea score (1.0) and HADS. No change was seen in O2 saturation. All patients improved on IMSTS (68%, 11% and 41%). Conclusion An AHP led clinic is a safe and cost effective means of providing pulmonary rehabilitation for patients following Covid-19 pneumonitis.
Nucleosome positioning is the precise determination of the location of nucleosomes on DNA sequence. With the continuous advancement of biotechnology and computer technology, biological data is showing explosive growth. It is of practical significance to develop an efficient nucleosome positioning algorithm. Indeed, convolutional neural networks (CNN) can capture local features in DNA sequences, but ignore the order of bases. While the bidirectional recurrent neural network can make up for CNN's shortcomings in this regard and extract the long-term dependent features of DNA sequence.

In this work, we use word vectors to represent DNA sequences and propose three new deep learning models for nucleosome positioning, and the integrative model NP_CBiR reaches a better prediction performance. The overall accuracies of NP_CBiR on H. sapiens, C. elegans, and D. melanogaster datasets are 86.18%, 89.39%, and 85.55% respectively.

Benefited by different network structures, NP_CBiR can effectively extract local features and bases order features of DNA sequences, thus can be considered as a complementary tool for nucleosome positioning.

Obesity in pregnancy and related early-life factors place the offspring at the highest risk of being overweight. Despite convincing evidence on these associations, there is an unmet public health need to identify "high-risk" offspring by predicting very early deviations in weight gain patterns as a subclinical stage towards overweight. However, data and methods for individual risk prediction are lacking. We aimed to identify those infants exposed to obesity in pregnancy at ages 3 months, 1 year, and 2 years who likely will follow a higher-than-normal body mass index (BMI) growth trajectory towards manifest overweight by developing an early-risk quantification system.

Following the coronavirus disease 2019 (COVID-19) pandemic, the health authorities recommended the implementation of strict social distancing and complete lockdown regulations to reduce disease spread. The pharmacists quickly adopted telemedicine (telepharmacy) as a solution against this crisis, but awareness about this technology is lacking. Therefore, the purpose of this research was to explore the patients' perspectives and preferences regarding telepharmacy instead of traditional in-person visits.

An electronic questionnaire was designed and sent to 313 patients who were eligible for the study (from March to April 2021). The questionnaire used five-point Likert scales to inquireabout motivations for adopting telepharmacy and in-person visits,their perceived advantages and disadvantages,and the declining factorsof telepharmacy. Finally, the results were descriptively analyzed using SPSS 22.

Of all 313 respondents, a total of 241 (77%) preferred appointments via telepharmacy while 72 (23%) preferred iy during crises such as the current COVID-19 pandemic. Telepharmacy can be applied as an important means and a crucial service to lessen the load on healthcare organizations and expand drug supply shelters in pharmacies. However, there are still substantial hurdles to overcome in order to successfully implement the telemedicine platform as part of mainstream practice.
