In order to obtain a comprehensive understanding of the status and potential risks of inorganic toxic elements in soil resulting from mining activities, Cu, Pb, Cr, Zn, Ni, As, and Cd were selected to evaluate a total of 42 soil samples collected from Gannan mining areas in northwestern China. The concentrations of As and Cd were much higher than their respective background values, while the concentrations of the other elements fluctuated around their background values. Results of combined multivariate statistical analyses and the distribution patterns of the individual pollutants imply that the toxic elements were originated from different sources even for one element in different sampling locations. The pollution index values indicated that As and Cd have a moderate to high pollution levels. The geo-accumulation indexes (Igeo) indicated that Cu, Pb, Cr, Zn, and Ni are likely of geologic origin, while As and Cd have been significantly affected by anthropogenic activities. Potential ecological risk indexes further showed that soils from mining areas within the study area pose a high potential ecological risk, and As and Cd were major risk contributors. Based on the calculated Hazard Index, the ingestion of soil particles appeared to be the main exposure route resulting in a higher risk, followed by dermal contact. The potential health risks of children and adults for As were greater than the safe level. The carcinogenic risk associated with As for local residents was also higher than the accepted levels, indicating a serious health risk to local residents. These results suggest that proper management strategies and various remediation practices should be implemented in the Gannan mining area in northwestern China.
Intraoperative nerve dysfunction has been difficult to investigate because of its rarity and unpredictable occurrence. The diagnostic test attributes of nerve function monitors have not been clearly defined. This proof-of-concept study aimed to assess the feasibility of using brachial plexus blockade (BPB) in awake patients as an experimental model for nerve dysfunction to characterize the diagnostic test attributes of somatosensory evoked potentials (SSEPs).

We obtained baseline SSEPs and neurologic function in patients and subsequently placed BPBs (experimental model) to generate progressive states of nerve dysfunction. We monitored SSEP changes (index test) and neurologic symptoms (reference standard) simultaneously during the onset of BPB to determine the temporal relationships and diagnostic test attributes of SSEPs.

Brachial plexus blockade produced differential motor and sensory dysfunction that allowed simultaneous clinical and neurophysiologic assessment. One hundred and fifty-seven pairs of multiple data points from 14 patients were included for final analysis. The onset of abnormal SSEP signals almost always preceded the onset of neurologic symptoms. The sensitivities and specificities of SSEP to detect the impairment of power (motor rating score ≤ 4/5), cold sensation, and two-point discrimination were 100% and 67%, 99% and 55%, and 100% and 46%, respectively.

This study found that BPB can produce sufficient differential nerve dysfunction to allow adequate evaluation of the diagnostic test attributes of SSEPs as a nerve monitor. The results of this study may stimulate further work on refining intraoperative nerve dysfunction models and diagnostic nerve function monitors.

With a potential penetration risk, the breeding and leakage of copepods are drawing more and more attention in recent years. The current study provided a thermal treatment method to control copepods and their eggs. Results showed that (1) the immediate death rates of P. tunguidus and H. falxus after heated to 34-40 °C for 5 min are positively correlated to the treatment temperatures (P  less then  0.01), and all individuals of the both species were eliminated after heated at 40 °C for 5 min; (2) overall hatching rates of P. tunguidus eggs were negatively correlated with treatment temperatures (P  less then  0.01) between 39-45 °C, with zero percent hatched after treatment at 45 °C for 5 min; (3) hatching rates of H. GSK2656157 cell line falxus were negatively correlated with treatment temperatures (P  less then  0.01) between 37-41 °C, with no nauplii hatched when treated at 41 °C for 5 min; (4) paraffin section histological examination indicated that thermal treatment caused severe damage to internal organs and egg structure. Finally, based on the experimental data, the application of the thermal treatment method was discussed in ozonation combined with biological activated carbon (O3/BAC) processing of drink water treatment.Non-obstructive azoospermia (NOA) is one of the leading causes of male factor infertility, which results from impaired spermatogenesis. Currently, the sole feasible therapeutic option for men with NOA to father their biologic children is sperm retrieval by testicular sperm extraction (TESE) approaches followed by an intracytoplasmic sperm injection program. Nevertheless, the rate of sperm retrieval from NOA men following TESE has remained as low as 50%, leading to a significant number of unsuccessful TESE operations. Given that TESE is associated with multiple side effects, the prediction of TESE outcome preoperatively can abolish unnecessary operations and thereby prevent NOA patients from sustaining adverse side effects. As the process of spermatogenesis is under the regulation of hormones, the hormonal profile of serum and/or seminal plasma may contain useful information about spermatogenesis status and can potentially predict the chance of sperm retrieval from NOA patients. A large body of literature is available on the predictive capability of different serum and seminal plasma hormones such as FSH, LH, testosterone, inhibin B, AMH, estradiol, prolactin, and leptin in a stand-alone basis or combinational fashion with respect to the TESE outcome. The present review aimed to evaluate the potential of these hormonal markers as noninvasive predictors of sperm retrieval in men with NOA.In this paper we examine determinants of prepaid modes of health care financing in a worldwide cross-country perspective. We use three different indicators to capture the role of prepaid modes in health care financing (i) the share of total prepaid financing as percent of total current health expenditures, (ii) the share of voluntary prepaid financing as percent of total prepaid financing, and (iii) the share of compulsory health insurance as percent of total compulsory prepaid financing. In the econometric analysis, we refer to a panel data set comprising 154 countries and covering the time period 2000-2015. We apply a static as well as a dynamic panel data model. We find that the current structure of prepaid financing is significantly determined by its different forms in the past. The significant influence of GDP per capita, governmental revenues, the agricultural value added, development assistance for health, degree of urbanization and regulatory quality varies depending on the financing structure we look at. The share of the elderly and the education level are only of minor importance for explaining the variation in a country's share of prepaid health care financing. The importance of the mentioned variables as determinants for prepaid health care financing also varies depending on the countries' socio-economic development. From our analysis we conclude that more detailed information on indicators which reflect the distribution of individual characteristics (such as income, family size and structure and health risks) within a country's population would be needed to gain deeper insight into the decisive determinants for prepaid health care financing.
The risk of malignancy associated with sequential disease-modifying therapies (DMTs) for patients with multiple sclerosis (MS) is uncertain. The aim of this study was to analyze the risk of cancer in patients with MS treated with azathioprine (AZA) and the influence of sequential DMTs on the risk.

We retrospectively enrolled a cohort of AZA-treated MS patients followed in two Italian centers from 1987 to 2019. The ratio between observed and expected cancers in the Italian general population was calculated as standardized incidence ratio (SIR). Associations between AZA and DMTs and cancer were estimated by Cox proportional hazards model.

We identified 500 AZA-treated MS patients, followed for a median time of 9.7 (0.1-45.7) years 61.8% of them were treated with DMTs. We found 22 cases of cancer (4.4%). The SIR was 1.14 (95% CI 0.98-1.29), not significantly increased in comparison with the general population. However, the risk was significantly higher in the quintiles of age 32-45, SIR 1.21 (95% CI 1.21-1.42), and 46-51, SIR 1.11 (95% CI 1.11-1.32) than in older cases. Age at AZA treatment onset was the only covariate significantly related to cancer incidence (HR = 1.049, 95% CI 1.007-1.093). The exposure to other DMTs did not modify the risk.

Retrospective equivalence (combined non-inferiority and non-superiority design) study with a before-and-after design on a tertiary ICU in which the SDD frequency was reduced from q.i.d. to t.i.d. All patients with ICU admissions ≥72h and with ≥2 surveillance cultures collected on different dates were included in this study. We compared successful decontamination of Gram-negative bacteria (GNB). Furthermore, time to decontamination, ICU-acquired GNB bacteraemia and 28-day mortality were compared between the two groups. In total 1958 ICU admissions (1236 q.i.d., 722 t.i.d). Decontamination was achieved during the first week of admission in 77% and 76% of patients receiving SDD q.i.d and t.i.d., respectively. Successful decontamination within 14 days (without consecutive acquisition of Gram-negative bacteria) was achieved in 69.3% of the admissions with q.
