Trastuzumab emtansine (T-DM1) is an antibody-drug conjugate of trastuzumab [a monoclonal antibody against human epidermal growth factor receptor 2 (HER2)] and DM1 (an inhibitor of tubulin polymerisation). It was initially approved in the European Union for the treatment of adult patients with HER2-positive unresectable locally advanced or metastatic breast cancer (BC) who had previously received trastuzumab and taxanes. On 18 December 2019, a variation of the marketing authorisation was approved extending this use to the adjuvant therapy of adult patients with HER2-positive early BC who have residual invasive disease in the breast and/or lymph nodes after neoadjuvant taxane-based and HER2-targeted therapy. A phase III randomised, multicentre, open-label trial compared T-DM1 with trastuzumab as adjuvant therapy in patients with HER2-positive early BC who had received preoperative chemotherapy and HER2-targeted therapy followed by surgery, with a finding of invasive residual disease in the breast and/or axillary lymph nodes. The study met its primary endpoint by showing an increased 3-year invasive disease-free survival rate in the T-DM1 arm (88.3%) compared with the trastuzumab arm (77.0%), with an unstratified hazard ratio of 0.50 (95% confidence interval 0.39-0.64). There was a higher incidence of hepatotoxicity (37.3% versus 10.6%), thrombocytopenia (28.5% versus 2.4%), peripheral neuropathy (32.3% versus 16.9%), haemorrhage (29.2% versus 9.6%) and pulmonary toxicity (2.8% versus 0.8%) in the T-DM1 arm compared with the control arm. The aim of this manuscript was to summarise the scientific review of the application leading to regulatory approval of this additional indication in the European Union.Precision medicine is now pivotal to design patients' specific treatment strategies with the aim of prolonging progression and overall survival. In this regard, invasive tumor tissue testing has so far been the golden standard for making cancer diagnosis, but has limitations. Cell-free tumor DNA (ctDNA), a form of liquid biopsy, is a noninvasive biomarker that can be isolated from patients' blood and other biofluids. An increasing body of evidence has demonstrated clinical utility of plasma ctDNA profiling to select patients for genomic-driven therapies. Analyses of mutations in plasma ctDNA have shown high accuracy and more rapid identification of mutations, allowing matching patients for specific therapies with equivalent clinical efficacy to that of the tissue profiling. In the clinical setting, ctDNA has been recently implemented to select patients with specific genomic alterations to targeted treatments, and a few molecular tests have been approved for use in non-small-cell lung, prostate, ovarian, and breast cancers. However, standardization of ctDNA collection, storage, and analysis methods would be critical to facilitate the wide adoption of ctDNA technology in routine clinical practice. This review summarizes how we can exploit ctDNA analysis to treat cancer patients, and explains how the results should be interpreted. In addition, we focus on how ctDNA could be used in the future as a marker of minimal residual disease to guide adjuvant therapy, as an immuno-oncology biomarker in patients treated with immune checkpoint blockade drugs, and as an early cancer detection marker to screen the asymptomatic population.Single-vehicle crashes are more fatality-concentrated and have posed increasing challenges in traffic safety, which is of great research necessity. Tremendous previous studies have conducted relevant analysis with econometric modeling approaches, whereas the ability of non-parametric methods to predict crash severity is still smattering of knowledge. Consequently, the main objective of this paper is to conduct single-vehicle crash severity prediction with different tree-based and non-parameter models. An alternate aim is to identify the intrinsic mechanism of how contributing factors determine single-vehicle crash severity. By virtue of Grid-Search method, this paper conducted fine-tuning of different models to obtain the best performances based on five crash severity sub-datasets. For model evaluation, the accuracy indicators were calculated in training, validation and test sets, respectively. Besides, feature importance extraction was undertaken based on the results of model comparison. The finding indicated that these models didn't exhibit a huge performance difference for crash severity prediction in the same severity level; however, the performances of the models did vary among different datasets, with an average training accuracy of 99.27 %, 96.4 %, 86.98 %, 86.84 %, 71.76 % in fatal injury, severe injury, visible injury, complaint of pain, PDO crash datasets, respectively. Additionally, it was found that in each severity dataset, the indicator urban freeways is a determinant factor that leads to the occurrence of crashes while rural freeways is more related to more severe crashes (i.e., fatal and severe crashes). This paper can provide valuable information for model selection and tuning in accident severity prediction. Future research could consider the influences that temporal instability of contributing features has on the model performances.
Although traffic accidents cause considerable economic losses and injuries to individuals, families, and communities, little is known about the impact of meteorological factors on the incidence of traffic accident injuries (TAIs). Therefore, a time-series study was conducted to explore the effect of meteorological variables on TAIs in Dalian, Northern China.

Poisson generalized linear models (PGLM) combined with distributed lag nonlinear models (DLNM) were used to estimate the association between daily TAIs and ambient temperature in Dalian, China, 2015-2017. The injury data collected by Dalian national injury surveillance hospitals, and meteorological data were extracted and accumulated from the National Meteorological Information Center. Both high temperatures (RR = 1.198, 95%CI:1.017-1.411) and low temperatures (RR = 1.017, 95%CI:1.001-1.035) increased the risk of TAIs. The cumulative lag effect would last until after the 7th day. While the 40-59 years subgroup seemed to be more vulnerable in high temperature environments, those who are more than 60 years showed higher TAIs in low temperatures for both single-day and cumulative TAI risks.

Many authors agree on the relevant role of EVs in correct communication between the mother and the embryo, as a fundamental system for the pregnancy to reach term and embryonic development to occur correctly. This review analyzes current information on known EVs, their main functions, and their role in implantation and embryonic development in domestic animals.Pre-slaughter fasting duration affect blood parameters related to stress and metabolism, decrease live and carcass weights and impact beef quality of cattle. In three experiments, 1100 steers and heifers, finished on feedlot or on pasture, were evaluated to assess the influence of the site and duration of fasting before slaughter on physiological, carcass and meat quality traits. Cattle were allocated to one of two fasting duration - long (23-29 h) or short (2-6 h) - and to one of two sites of fasting- farm or abattoir. All animals had access to water ad libitum until slaughter, except during transportation. Cattle were assigned to two (long fasting on abattoir/normal lairage time, short fasting on farm/minimal lairage time) or three (long fasting on abattoir/normal lairage time, short fasting on farm/minimal lairage time, long fasting on farm/minimal lairage time) treatments. Seventeen slaughters were considered in a randomized complete block design. Linsitinib There was no effect of the site of fasting on any variable. Cattle with short fasting duration had higher carcass weights and water consumption than those under long fasting duration. Haematocrit, globulin, total protein, and lactate dehydrogenase at the time of slaughter increased with fasting duration. The 'chapeau de gendarme' (CDG) sign, characterised by a symmetric down-turned mouth, has been considered as a semiological hallmark of focal epileptic seizures with cingulate or insular involvement. We report three cases in which the CDG sign featured in habitual seizures.

The epileptogenic zones of these three patients were localized in the lateral prefrontal cortex by video-stereoelectroencephalography monitoring. A sulcal resection has led to a seizure-free outcome in each patient.

Ictal stereoelectroencephalography demonstrated that ictal discharges arising from the lateral prefrontal cortices were immediately followed by low-voltage fast activity, concomitantly involving the anterior insulo-cingulate cortices, and preceding the onset of the CDG signs. Limited sulcal resection and seizure freedom after surgery confirmed the restricted distributions of the epileptogenic zones in the lateral prefrontal cortex.

Our observations led us to speculate that the epileptic discharges could enter the emotional insulo-cingulate cortical system through the prefrontal-cingulate inhibitory projections (feedforward pattern) -arising from the supragranular layers of the lateral prefrontal cortex-and proceed to the deeper layers of the anterior-middle cingulate cortex, trigger the co-occurrence of gamma bands, and evoke a set of exaggerated behaviours, which is often accompanied by the unique facial sign.
