The nonmotor manifestations of Parkinson's disease often begin years before the motor symptoms. Autopsy studies, including both Parkinson's disease patients and matched controls, demonstrated that α-synuclein aggregates in Parkinson's disease patients can be found in both the substantia nigra and the enteric nervous system. Therefore, it has been hypothesized that the pathological process that leads eventually to Parkinson's disease might initially take place in the enteric nervous system years before the appearance of motor features. The gut microbiome plays essential roles in the development and maintenance of different body systems. Dysbiosis of the normal gut microbiome is thought to be associated with pathophysiologic changes not only in the gastrointestinal system itself but also in the enteric and central nervous systems. These changes are thought to ultimately cause loss of dopaminergic neurons via various mechanisms including the release of neurotoxins into the systemic circulation, decreased production of neuroprotective factors, and triggering inflammatory and autoimmune responses. In this review, we review the gut microbiome changes in Parkinson's disease and discuss the mechanisms by which gut microbiome dysbiosis may be a contributing factor to the pathophysiology of Parkinson's disease. © 2020 International Parkinson and Movement Disorder Society.Activity of acid sphingomyelinase has been implicated in a number of diseases like acute lung injury, sepsis or metastasis of melanoma cells. Here, we present a sphingomyelinase FRET probe based on FAM/BODIPY dyes for real time monitoring of acid sphingomyelinase. The probe gives rise to a tremendous increase in fluorescence of the fluorescein FRET donor upon cleavage and we show that this is to a significant part due to cleavage-associated phase transition, suggesting a more systematic consideration of such effects in future probes. KGaA, Weinheim.In order to create a healthy, comfortable, productive, and energy-efficient indoor environment, the computational fluid dynamics (CFD)-based adjoint method with an area-constrained topology method can be used to inversely design the optimal number, size, location, and shape of air supply inlets and air supply parameters. However, this method is not very mature, and the distribution of retained inlets is always scattered. To solve that problem, this investigation introduced a filter method that smooths the intermediate results during the inverse design process. Using a three-dimensional, non-isothermal, asymmetrical office with pre-set air supply inlets as an example, this study verified the performance of the proposed filter-based topology method. The verified method was then used to solve a multi-objective design problem and design an optimal indoor environment for a room. The results indicate that the proposed method was able to find the optimal number, location, and shape of air supply inlets and the optimal air supply temperature, velocity, and angle that led to a thermally comfortable, healthy, productive, and energy-efficient indoor environment. Finally, this investigation installed the optimal inlets in an environmental chamber to mimic the room. The measured air temperature, velocity, and mean age of air in several typical locations in the environmental chamber matched the CFD simulation results very closely. © 2020 John Wiley & Sons A/S. The objective of this review article is to recognize the severity of complications associated with malnutrition in patients with cirrhosis and ways to overcome these obstacles. © 2020 American Society for Parenteral and Enteral Nutrition.During the early ontogeny of fish larvae, the accurate development of the visual system plays a key role, because it is involved in locating food, orientation, selection of favorable habitat, and evasion of predators. The structure of the eye of the fish is typical of vertebrates, with some modifications related to the aquatic environment. In the present work, we describe the development of the larval eye of Engraulis anchoita for the first time. Larvae were collected at the Permanent Station of Environmental Studies (EPEA) in coastal waters of the Southwestern Atlantic Ocean during research cruises in 2015 and 2016. We describe the histology of the retina layers, determine the beginning of the functionality of the eye, and discuss a possible synchronization with the development of the digestive tract. This study provides information about the biology of E. anchoita, the most abundant fish species in the southwestern Atlantic Ocean. Also, recent studies have shown responses of the retina and other tissues to the increase in environmental acidity. Therefore, results of this study are also discussed with respect to the possible effect of acidification on the larvae of this species. The continuity of the time series developed at the EPEA will allow monitoring the effect of long-term environmental and biological variables on the early ontogeny of anchovy in the context of climate change. The high commercial fishing potential of E. anchoita due to its high abundance, as well as its essential role in the trophic web of other commercially valuable fishing resources of Argentina, reinforce the need to continue deepening knowledge about this species. At hatching, the retina is formed by only few layers from which the other layers differentiates during ontogeny. It is quite rare to find a mass only on the cardiac surface. Today we report a patient with a cardiac mass grown on the cardiac surface and also had a coronary aneurysm combined a coronary fistula, pathology examination showed that the mass was not a tumor but an aneurysm with thrombosis. This is the first time that a primary thrombus discovered on the surface of the heart. © 2020 Wiley Periodicals, Inc.BACKGROUND AND AIM All human beings have the need to feel connected with others. However, researchers have found that for those aged 80+, loneliness markedly increases and that such loneliness is often linked to life changes or the loss of a close relative. The loss of a life partner is considered to have a greater impact on men's identify, social engagement and management of life. The aim of this study was to deepen understanding of older (80+) men's life after the loss of their life partners. THEORETICAL FRAMEWORK Caring science and Eriksson's theory of caritative caring form the theoretical base for the study and its interpretative framework. The central concepts are suffering of life and desire of life. INFORMANTS, METHODOLOGY AND METHODS A total of five men aged 81-91 gave thereby consent for participation in the study. Emanating from hermeneutical methodology, qualitative deep interviews with a narrative approach were used to collect data. Thereafter, a six-step qualitative thematic analysis in accordance with Braun and Clarke was used to analyse the text. INTERPRETATION AND CONCLUSION Four themes emerged reconciliation with one's lived life, living in the present with uncertainty about the future, a reorientation in life including new living conditions and living with the hope of finding a new life partner and feeling loved. We found that loss can lead to suffering in life. The life can be filled with loneliness, emptiness, life weariness and an absence of desire, hope and longing. Yet there is a vitality in suffering that enables to find new meaning contexts and desire for life. Here, the fundamental choice underpinning the movement between life suffering and desire for life is comprised of reconciliation with one's lived life, living in the present, daily habits and routines, being seen, participation, love and thoughtfulness. © 2020 The Authors. Scandinavian Journal of Caring Sciences published by John Wiley & Sons Ltd on behalf of Nordic College of Caring Science.BACKGROUND AND AIMS The optimal valve and prosthesis in patients with aortic valve infective endocarditis with annular abscess is controversial. If extensive annular defects occur after debridement, standard techniques are difficult; more complex aortic root replacement (ARR) or aortic valve translocation technique are inevitable. The Solo Smart stentless bioprosthesis is specially designed for supra-annular implantation without annular stitches. METHODS Nineteen patients with active aortic valve infective endocarditis underwent aortic valve replacement in the past 3 years. Of these, we performed aortic valve replacement using the Solo Smart valve in four patients with extensive annular destruction and complex aortic root pathologies requiring reconstruction. RESULTS Although more than two-thirds of the annular structure was missing after radical debridement of infected tissues, supra-annular aortic valve replacement with the Solo Smart valve could be performed successfully in all four patients. All patients are doing well without prosthetic valve dysfunction and/or recurrent infection. CONCLUSION The supra-annular aortic valve replacement using the Solo Smart valve is considered to be a useful alternative to standard aortic valve replacement in patients complicated with extensive annular defect. It is a simple and technically less demanding alternative to ARR or aortic valve translocation technique. © 2020 Wiley Periodicals, Inc.OBJECTIVE Improved treatment outcome in family-based treatment (FBT) for anorexia nervosa (AN) is predicted by weight gain occurring early in the course of treatment (i.e., about 4 lbs by week 4). Although prior work suggests that early weight gain in higher levels of care (e.g., partial hospitalization programs [PHP]) predicts weight restoration at discharge, no study has examined the specific rate of gain within FBT-informed PHP programs that best predicts treatment response. METHOD This study examined rate of weight gain in pounds and percent expected body weight (EBW) that predicts positive outcome in 70 patients (M age = 15.49 years, SD = 2.56) with AN who were enrolled in a family-based PHP. RESULTS Receiver operator characteristic analyses demonstrated that changes in %EBW during weeks 2-5 were more useful than changes in weight in predicting positive outcome. Gaining at least 8.9 pounds or over 8% of EBW in the first 4 weeks of treatment significantly predicted positive outcome. DISCUSSION Findings suggest that positive outcome in an FBT-informed PHP is predicted by rapid weight gain in the initial weeks of treatment.
