Purpose The purposes of the study were to i) verify the variations in training load and the subsequent effects on swimming performance parameters, biochemical parameters, and autonomic nervous activity during a water polo season; ii) investigate the sensitivity of physiological markers in tracking training load and performance variations, and iii) verify the overreaching prevalence. Method The training load of 20 female water polo players was monitored (using the session rating of perceived exertion method [sRPE], training monotony, and strain), and the lactate minimum speed (LMS), repeated sprint ability (RSA), plasma hormone and glutamine concentration, salivary immunoglobulin A (SIgA), and heart rate variability (lnRMSSD) were evaluated during the season. Result The training load parameters were higher in the competitive cycle (p ≤ 0.002). The LMS improved only in the general cycle from baseline (p = .015), while the RSA best time improved in the general (p = .002) and specific cycles (p = .012) from baseline and deteriorated in the competitive from general cycle (p = .008). The SIgA secretion rate presented a reduction only in the specific cycle from baseline (p = .032), while the lnRMSSD increased in the general (p = .038) and competitive (p less then .001) cycles from baseline. Five athletes were diagnosed as overreaching state. Conclusion Therefore, the physiological markers (i.e., plasma hormone and glutamine concentration, SIgA, and lnRMSSD) showed little sensitivity to detect changes in training load and swimming performance. The higher training loads applied in the competitive cycle seem to limit swimming performance gains.Purpose The extra-time (ET) period of soccer is competed during fixture congested schedules with often limited recovery time between matches. The aim of this study was to assess muscle damage recovery following 90- and 120-min (i.e., incorporation of ET) of simulated soccer match-play. Methods Twelve semiprofessional soccer players completed 90 and 120-min treadmill-based soccer-specific exercise in a counterbalanced order. Creatine kinase (CK), creatinine, urea, aspartate aminotransferase, perceived muscle soreness, pain pressure threshold, reactive strength index, countermovement jump height, and isokinetic strength assessments of eccentric knee flexors at 60, 180 and 270 deg‧s-1 were taken at baseline and immediately-, 24, 48 and 72-hr post-exercise to assess recovery. Results No significant between-trial interactions except for CK were found. Pairwise comparisons detected a 53% increase in CK at 24-hr (455 ± 29 μ∙L-1) following 120-min of simulated match-play vs. the corresponding post 90-min time-point (299 ± 29 μ∙L-1; p less then .01). The 120-min trial caused a 58% higher CK response at 72-hr (244 ± 25 μ∙L-1) vs. post 90-min comparisons (154 ± 29 μ∙L-1; p = .02). No interaction effects were detected for any other recovery variables. Creatine kinase and perceived muscle soreness remained elevated up to 72-hr in both trials (p less then .01). Conclusions These data indicate that 120 min of simulated soccer match-play delays the time-course of CK recovery up to 72-hr post-match. However, 120 min of simulated soccer has no additional impact on functional recovery and perceived muscle soreness vs. 90 min. Recovery should be investigated following 90- and 120-min of actual match-play.Trial registration The study was pre-registered on the Open Science Framework (DOI 10.17605/OSF.IO/VGU6T Date 10/06/2019).Acute lymphoblastic leukemia (ALL) is a challenging disease with a growing genetic landscape, even though there is substantial gap between developed and non-developed countries when it comes to availability of such new technologies. This manuscript reports a 5-year retrospective cohort of newly diagnosed ALL patients and their genetic findings and outcomes. An expanded genetic evaluation by using FISH and RT-PCR was implemented, aiming to identify Ph-like alterations. Patients were treated according to our local protocol, which allocated patients according to age and Philadelphia-chromosome status. A total of 104 patients was included, with median age of 37.5 years. Philadelphia chromosome was detected in 33 cases of B-lineage. Among 45 Ph-negative B-lineage, after excluding KMT2A or TCF3-PBX1 cases, we identified 9 cases with Ph-like fusion. Ph-positive and Ph-like patients had higher initial WBC (p = 0.06). Out of 104 cases, two cases did not start chemotherapy and an early death rate of 10.8% was found. Allogeneic transplantation was performed in 18 cases, being ten performed in first CR. Three-year overall survival (OS) and 3-year event-free survival were 42.8% and 30.8%, respectively. For patients treated with a pediatric regimen, 3-year OS was 52.5%. Extramedullary disease (HR 0.42) and platelet counts (HR 0.9) were independently associated with OS. We still face excessive non-relapse mortality that compromises our results. Measures to decrease early infection are desirable.Fires occur in most terrestrial ecosystems where they drive changes in the traits, composition, and diversity of fungal communities. Fires range from rare, stand-replacing wildfires to frequent, prescribed fires used to mimic natural fire regimes. Fire regime factors, including burn severity, fire intensity, and timing, vary widely and likely determine how fungi respond to fires. Despite the importance of fungi to post-fire plant communities and ecosystem functioning, attempts to identify common fungal responses and their major drivers are lacking. This synthesis addresses this knowledge gap and ranges from fire adaptations of specific fungi to succession and assembly fungal communities as they respond to spatially heterogenous burning within the landscape. Fires impact fungi directly and indirectly through their effects on fungal survival, substrate and habitat modifications, changes in environmental conditions, and/or physiological responses of the hosts with which fungi interact. Some specific pyrophilous, or "fire-loving," fungi often appear after fire. Our synthesis explores whether such taxa can be considered cosmopolitan, and whether they are truly fire-adapted or simply opportunists adapted to rapidly occupy substrates and habitats made available by fires. We also discuss the possible inoculum sources of post-fire fungi and explore existing conceptual models and ecological frameworks that may be useful in generalizing fungal fire responses. We conclude with identifying research gaps and areas that may best transform the current knowledge and understanding of fungal responses to fire.Tracheal bronchus (TB) occurs in 0.1-3% of the population as an accessory bronchus that originates in the trachea, typically supplying the right upper lobe. The presence of a TB can pose unique airway challenges, particularly during procedures that require lung isolation. Here, we describe a case of TB with difficult lung isolation. Careful examination with fiberoptic bronchoscopy permitted double lumen tube positioning without obstruction of the TB. A second case is presented where the presence of TB did not affect anesthetic management. Implications of TB for airway management and strategies for successful one-lung ventilation are discussed.
Understanding the potential inhalation toxicity of poorly characterized aerosols is challenging both because aerosols may contain numerous chemicals and because it is difficult to predict which chemicals may present significant inhalation toxicity concerns at the observed levels. We have developed a novel systematic procedure to address these challenges through non-targeted chemical analysis by two-dimensional gas chromatography-time-of-flight mass spectrometry (GC × GC-TOFMS) and assessment of the results using publicly available toxicity data to prioritize the tentatively identified detected chemicals according to potential inhalation toxicity.
The procedure involves non-targeted chemical analysis of aerosol samples utilizing GC × GC-TOFMS, which is selected because it is an effective technique for detecting chemicals in complex samples and assigning tentative identities according to the mass spectra. For data evaluation, existing toxicity data (e.g. from the U.S. Environmental Protection Agency CompToxicology endpoints, we have developed an effective approach to elucidate the potential inhalation toxicity of aerosols.An inactivated poliovirus vaccine candidate using Sabin strains (sIPV) grown on the PER.C6® cell line was assessed in infants after demonstrated immunogenicity and safety in adults. The study recruited 300 infants who were randomized (1111) to receive one of 3 dose levels of sIPV or a conventional IPV based on Salk strains (cIPV). Poliovirus-neutralizing antibodies were measured before the first dose and 28 days after the third dose. Reactogenicity was assessed for 7 days and unsolicited adverse events (AEs) for 28 days after each vaccination. Serious AEs (SAEs) were recorded throughout the study. Solicited AEs were mostly mild to moderate. None of the SAEs reported in the study were judged vaccine related, including one fatal SAE due to aspiration of vomitus that occurred 26 days after the third dose of low-dose sIPV. After 3 sIPV vaccinations and across all dose levels, seroconversion (SC) rates were at least 92% against Sabin poliovirus types and at least 80% against Salk types, with a dose-response in neutralizing antibody geometric mean titers (GMTs) observed across the 3 sIPV groups. Compared to cIPV, the 3 sIPV groups displayed similar or higher SC rates and GMTs against the 3 Sabin types but showed a lower response against Salk types 1 and 2; this was most visible for Salk type 1. While the PER.C6® cell line-based sIPV showed an acceptable safety profile and immunogenicity in infants, lower seroprotection against type 1 warrants optimization of dose level and additional clinical evaluation.The main objective of the present meta-analysis was to analyze associations between security of attachment to parents and self-esteem. Studies were included if they assessed bivariate associations between self-esteem and attachment security with mothers and/or fathers, or with both parents in general. A systematic search in the electronic databases PSYCINFO, Web of Science, Google Scholar, and PSYNDEX identified 202 studies with 81,485 participants that were included in this multilevel meta-analysis. Criteria from the Mixed Methods Appraisal Tool were used for assessing the quality of the individual studies. Most studies assessed security with verbal measures (190 studies), and the mean age of participants was 16.5 years. We found a moderate positive concurrent correlation of attachment security to parents with self-esteem (r = .34; 95% confidence interval [CI], .33-.36), with associations being stronger when assessing attachment to parents in general (r = .37; CI, .35 to .40) rather than to mothers (r = .33; CI, .31 to .35) or fathers (r = .32; CI, .30 to .34) in particular. Cross-lagged effects indicate that higher initial attachment security predicts an increase in self-esteem over time (r = .19; CI, .09 to .28), while initial self-esteem predicts change in security (r = .08; CI, .02 to .14). Correlations of attachment security with self-esteem were weaker in older participants and stronger in studies with validated attachment measures. As most of the included studies have been conducted with adolescents and young adults, knowledge about associations of secure attachment to parents and self-esteem in the first years of life is still limited. Nonetheless, it is concluded that the available results support suggestions of attachment theory on the role of secure attachment for self-esteem, although causal effects could only be tested in experimental studies.
