The results suggest that olfaction is not a sensitive marker of anorexia nervosa diagnosis, but olfactory sensitivity may be a useful indicator of anorexia nervosa severity.Hemophagocytic lymphohistiocytosis (HLH) is a syndrome of excessive immune system activation driven mainly by high levels of interferon gamma. The clinical presentation of HLH can have considerable overlap with other inflammatory conditions. We present a cohort of patients with therapy refractory HLH referred to our center who were found to have a simultaneous presentation of complement-mediated thrombotic microangiopathy (TMA). Lapatinib Twenty-three patients had therapy refractory HLH (13 primary, 4 EVB-HLH, 6 HLH without known trigger). Sixteen (69.6%) met high-risk TMA criteria. Renal failure requiring renal replacement therapy, severe hypertension, serositis, and gastrointestinal bleeding were documented only in patients with HLH who had concomitant complement-mediated TMA. Patients with HLH and without TMA required ventilator support mainly due to CNS symptoms, while those with HLH and TMA had respiratory failure predominantly associated with pulmonary hypertension, a known presentation of pulmonary TMA. Ten patients received eculizumab for complement-mediated TMA management while being treated for HLH. All patients who received the complement blocker eculizumab in addition to the interferon gamma blocker emapalumab had complete resolution of their TMA and survived. Our observations suggest co-activation of both interferon and complement pathways as a potential culprit in the evolution of thrombotic microangiopathy in patients with inflammatory disorders like refractory HLH and may offer novel therapeutic approaches for these critically ill patients. TMA should be considered in children with HLH and multi-organ failure, as an early institution of a brief course of complement blocking therapy in addition to HLH-targeted therapy may improve clinical outcomes in these patients.Background and aims Linked color imaging (LCI) improved the visibility of gastric cancer and colorectal flat lesions. This study aimed to investigate the usefulness of LCI in detecting superficial esophageal squamous cell carcinomas (SESCC). Methods We enrolled 37 consecutive SESCC patients (46 SESCCs) diagnosed using LCI and blue laser imaging bright mode (BLI-BRT) and treated in Hiroshima University Hospital between April 2018 and November 2018. Eight professional endoscopists compared images obtained on non-magnifying BLI-BRT and LCI versus conventional white light imaging (WLI). Identification and boundary diagnosis of SESCC with LCI and BLI-BRT were compared with WLI. Changes in lesion visibility were clarified. Interobserver agreement was assessed. Clinicopathological features of lesion that influence visibility with LCI were assessed. Results In LCI, 37% (17/46) of cases had improved visibility and 63% (29/46) had unchanged visibility (interobserver agreement = 0.74). Among cases with multiple lugol voiding lesions (LVLs), ΔE between the lesion and background mucosa was significantly higher in LCI than in WLI (20.8 ± 7.9 vs 9.2 ± 6.1, P less then 0.05). No significant differences were found in tumor size, morphological type, color, depth, and smoking or drinking history. However, multiple LVLs were significantly higher among cases with improved versus unchanged visibility. On BLI-BRT, 39% (18/46) of cases had improved visibility and 61% (28/46) had unchanged visibility (interobserver agreement = 0.60). Conclusion Almost the same as BLI-BRT, LCI improves SESCC visibility compared with WLI. This is useful for cases with multiple LVLs. In cases without background coloration (BGC), LCI may make SESCC more visible than BLI-BRT.Nonlinear effects on the electrophoresis of a soft particle, consisting of a rigid hydrophobic core coated with a diffuse polymer layer (PEL) suspended in an electrolyte medium, are studied. The impact of the ion partitioning effect arising due to the Born energy difference between the PEL and the electrolyte is approximated based on the equilibrium Boltzmann equation, with which the ion distribution and hence, the charge density is modified. The equations describing the electrokinetic transport comprising the Darcy-Brinkman extended Navier-Stokes equations which includes the ion partitioning effect coupled with the modified Nernst-Planck equations and Poisson equations for electric field are solved numerically. The present numerical model for the soft particle compares well with the existing theoretical solutions and experimental results in the limiting cases. A deviation from existing simplified models based on the Boltzmann distribution of ions occurs when the Debye layer polarization, relaxation and the electroosmosis induced by the PEL immobile charge become significant. The hydrophobicity of the inner core strongly influences the nonlinear electrokinetic effects by modifying the Debye layer, electroosmotic flow in the PEL and surface conduction. The results indicate that the ion partitioning can significantly increase the electrophoretic mobility of the soft particle by attenuating the shielding effect. When the Debye layer is in the order of the particle size the hydrophobicity of the core surface and the ion partitioning effect manifest the surface conduction, which implies that the Boltzmann distribution of ions is no longer valid. The core hydrophobicity and ion partitioning effect have influence on the condensation of the PEL immobile charge, which creates a significant impact on the mobility.The question of whether some cases of interstitial cystitis may have an infectious etiology has been debated for some time. As part of the MAPP (Multidisciplinary Approach to the Study of Chronic Pelvic Pain) Research Network, we examined urine specimens from interstitial cystitis patients who provided specimens over time and also reported various symptoms at the time of urine collection. We first performed next-generation sequencing to look for the presence of viruses in urines, and detected two human polyomaviruses that are known to be excreted into urine, BKPyV and JCPyV. We were especially interested in BKPyV because it is a known cause of another bladder disease, hemorrhagic cystitis, in bone marrow transplant recipients. Further analysis of individual samples indicates a trend toward higher excretion of polyomaviruses in patients experiencing increased symptoms.Seneca Valley virus (SVV) is an emerging global picornavirus that causes porcine idiopathic vesicular disease. We characterized the genome and conducted evolutionary and recombination analyses of four newly identified SVV strains which were CH-GDZS-2019, CH-GDMZ-2019, CH-GDHZ01-2019, and CH-GDHZ02-2019. Sequence alignment and phylogenetic analysis showed that strains circulating in swine herds in China were genetically diverse and complex. Recombination analyses indicated that strain CH-GDZS-2019 was derived from strains USA-IA44662-2015-P1 and USA-GBI29-2015, which were both isolated in the USA in 2015, while CH-GDMZ-2019 was derived from the Chinese field strains 1-2018-BH-China and CH-GDQC-2017. Our results provided important insights into the molecular characterization of the SVV strains co-circulating in Guangdong Province in China in 2019 and demonstrated the importance of additional SVV surveillance in China.There is a strong link between habitat loss, fragmentation of natural vegetation, and reduction of biodiversity in aquatic systems. The objective of this study was to evaluate the influence of different landscape scales on the community of Ephemeroptera, Plecoptera, and Trichoptera in Atlantic Forest rivers. We sought to understand how aquatic communities are affected by different habitat structures and anthropogenic changes in different spatial scales (in radii of 30, 50, 100, 150, and 200 m). Seven points were established where eight sample campaigns were carried out between 2015 and 2016. The analyses indicate the types of land use, with forest cover being of greatest expressiveness, as determinants in the structure and diversity of the Ephemeroptera, Plecoptera, and Trichoptera community. When evaluating the effect of the scales, for the basin evaluated, it was detected that the 30 and 50 m buffers were the most representative. These buffers suggest that the configuration and composition of landscape, especially the forest cover class, have a strong influence on the composition of the aquatic entomofauna in the region. The results confirm the importance of studies at local scales regarding the relation of the landscape for Ephemeroptera, Plecoptera, and Trichoptera biodiversity in view of the particular relationship of these orders with the riparian forest for its various functions. It is concluded that riparian forests in the 30 and 50 m scales should not be modified; otherwise, aquatic biodiversity will be compromised.Purpose To evaluate the development and rate of growth in macular atrophy after intravitreal injections of aflibercept (IVAs) for neovascular age-related macular degeneration (AMD) over a 2-year period. Study design Retrospective, interventional, consecutive case series. Methods This study included 94 eyes of 92 patients with treatment-naïve AMD involving the foveal center treated with IVAs at 3 university hospitals in Japan. The patients underwent IVAs bimonthly after 3 initial monthly doses in the first year. The protocol was converted to a treat-and-extend regimen in the second year. The incidence and growth rate of macular atrophy were quantified based on hypoautofluorescence detected by fundus autofluorescence images. Additionally, possible background factors related to the development and rate of growth of macular atrophy were investigated. Results Of 94 eyes, 39 (41.5%) had typical AMD and 55 (58.5%) had polypoidal choroidal vasculopathy. Ten eyes (10.6%) had macular atrophy at the baseline. Of the remaining 84 eyes, 14 (16.7%) had developed new macular atrophy at 2 years, the square root of the growth rate of atrophy was 0.52 mm/year. In multivariate analyses, a poorer best-corrected visual acuity (P = 0.01) and the presence of intraretinal fluid (P = 0.04) at baseline were found to be the independent predictors for the development of macular atrophy. No factors were found that were significantly related to the growth rate of the macular atrophy. Conclusions Our study determined the incidence and rate of growth of macular atrophy after IVAs for neovascular AMD in clinical settings. Eyes with vision reduction and intraretinal fluid at the baseline develop macular atrophy more frequently after IVAs for neovascular AMD.
