The purpose of this study was to evaluate treatment strategies and factors influencing overall survival (OS) and disease-free survival (DFS) in resectable, non-small cell lung cancer (NSCLC) with mediastinal (N2) lymph node metastasis.

All patients undergoing surgery for NSCLC with N2 disease between 2006 and 2016 were included. Treatment approaches included surgery only, neoadjuvant therapy followed by surgery, surgery followed by adjuvant therapy, and neoadjuvant therapy followed by surgery and adjuvant therapy (triple therapy). Patient clinical and pathologic data were retrospectively collected.

A total of 281 patients were included in the study. In total, 209 patients had neoadjuvant therapy, 47.4% of which went on to received additional adjuvant therapy. The pathologic complete response rate was 12.9%. The treatment strategy which included triple therapy was isolated as a significant contributor to improved OS and DFS. Nodal downstaging (N0) after induction therapy conferred an OS benefit (38.3% vs. 15.6%, p = .03). Patients with single-station N2 disease experienced higher DFS. Video-assisted thoracic surgery (VATS) lobectomy completion rates were higher at the end of the study period compared to the beginning (p < .001).

Because of the strong consumer driver towards more natural or higher sustainability cosmetic products, silk fibroin was evaluated to help develop a formulation with natural and effective ingredients for personal care. In order to exploit the physical properties of silk fibroin, it was evaluated to maximize the surfactant properties of other commercial ingredients to lower surface tension and build up viscosity. A synergistic effect was seen between silk fibroin and capryl glucoside, a sugar surfactant which exhibited a natural and effective co-surfactant system. This system demonstrated better surface tension properties than sodium laureth sulphate (SLES), cocamidopropyl betaine (CAPB), rhamnolipids and sophorolipids, which led to greater foamability and cleansing properties. This system proved to also be compatible with polysaccharide viscosity modifiers to enhance the viscosity of the system. The present study comprises a systematic exploration of natural formulation development of silk proteins and otheratural product such as a cleanser or shampoo while still having the same or greater effectiveness as synthetic surfactants and ingredients typically used in cosmetic formulations.
Silk fibroin in combination with capryl glucoside outperforms other commercial surfactants that are commonly used in the industry because of its surface-active behaviour and synergy. This system is then enhanced further with polysaccharide rheological modifiers, carrageen and xanthan gum to help build up viscosity. The complex mixture of silk fibroin, sugar surfactant and biopolymer results in a formulation that is all natural, while still having high performance by achieving great foamability and enhanced sebum removal. The mixture can further be used to formulate a fully natural product such as a cleanser or shampoo while still having the same or greater effectiveness as synthetic surfactants and ingredients typically used in cosmetic formulations.
Atherosclerosis is recognized as a systemic low-grade inflammatory disease. Furthermore, the dysregulation of the inflammatory response and its timely resolution is a pivotal process in determining the clinical manifestations of cardiac and cerebral acute ischaemia following atherothrombosis.

This narrative review is based on the material searched on PubMed up to October 2020. The search terms we used were as follows "atherosclerosis, inflammation, acute myocardial infarction and ischemic stroke" in combination with "biomarker, inflammatory cells and molecules, treatment."

The expected goal of addressing inflammation for the treatment of atherosclerosis and its acute ischaemic complications is reducing mortality and morbidity related to atherosclerotic cardiovascular disease, which are currently the first cause of death and disability worldwide. In this narrative review, we summarize the evidence about the main cellular and molecular mechanisms of inflammation in atherogenesis, atherothrombosis and acute ischaemic complications, with particular focus on the potential molecular targets for novel pharmacological treatments.

Although a large amount of evidence from animal models of atherothrombotic disease, and promising results of clinical trials, anti-inflammatory treatments against atherosclerosis are not yet recommended. A deepest understanding of pathophysiological mechanisms underlying the mechanisms driving resolution of the acute inflammation will probably allow to identify the optimal molecular target.
