Background/aims Adherence to medication and maintained virologic response (MVR) are related to the risk of adverse clinical outcomes. This study aimed to compare the efficacy of entecavir (ETV) and tenofovir disoproxil fumarate (TDF) in relation to the adverse clinical outcomes among chronic hepatitis B (CHB) patients stratified according to adherence to medication and MVR. Methods A total of 1794 treatment-naive CHB patients treated with ETV (n = 894) or TDF (n = 900) for > 1 year were identified. Results Adherence rates were significantly higher in the TDF than in the ETV (93.4% vs. 89.1%, respectively; P less then 0.001). The MVR of ETV and TDF were 64.5% and 71.7%, respectively (P = 0.001). The MVR of ETV and TDF in the good adherence group were 72.1% and 76.4%, respectively (P = 0.083); in the poor adherence group, the MVR of ETV and TDF were 63.0% and 54.0%, respectively (P = 0.384) Multivariate analysis showed that the risk of HCC and death or transplantation was similar between groups (HR 0.826, 95% CI 0.522-1.306; P = 0.413 and HR 0.636, 95% CI 0.258-1.569; P = 0.325, respectively) after adjusting for adherence to medication and MVR. In the 589 propensity-matched pairs of patients, risk of HCC and death or transplantation was similar between treatment groups after stratification according to adherence rates and MVR. Conclusions After adjustment for adherence and MVR, ETV, and TDF did not differ in terms of the risk of HCC and death or transplantation in all patients and propensity score-matched cohorts. Background and aims Information concerning deep radiological healing of perianal fistulas in Chinese patients with CD is limited. The present study aimed to establish the effectiveness of infliximab on CD-related perianal fistulas using magnetic resonance imaging (MRI) and identify predictors of deep radiological remission of fistulas. Methods We retrospectively reviewed patients with CD with draining perianal fistulas treated with infliximab and included only those who underwent clinical assessment and MRI before and after infliximab therapy. Results Among 178 patients who underwent repeated MRI and clinical assessment, 65.2% had complex fistulas. Post-infliximab therapy, 55.1% of patients with perianal fistulizing CD showed clinical remission and 26.4% presented a clinical response; 38.2% had deep radiological remission, and 34.3% had a partial response based on the Ng score; the Van Assche scores decreased obviously compared with baseline. Prolonged infliximab infusion (18 times) presented higher radiological remission rates in patients with CD with complex fistulas. Concomitant treatment with azathioprine increased the fistula healing rate compared with infliximab alone (50% vs. 36.9%, P less then 0.001). Younger age at diagnosis of CD, proctitis and requiring perianal surgery were identified as predictors of poor deep radiological remission of fistulas. Eight of ten patients who stopped infliximab and switched to an alternative agent retained a status of fistula healing in the first year of follow-up. Conclusions Infliximab induced deep radiological remission of perianal fistulas in Chinese patients with CD. Routine MRI should be used to monitor fistula healing. Patients with younger age at diagnosis of CD, proctitis, and/or requiring perianal surgery should receive combined therapy and careful monitoring.Factitious disorders (FD) like Munchausen syndrome are well known to most physicians, yet the corresponding ICD-10 diagnosis F68.1 remains severely under-assigned and often misdiagnosed. To approach this problem, we conducted a nationwide inquiry for Germany and Norway as well as a comparison between these two countries regarding the incidence of diagnosis of FD. The assignment rates of F68.1 in somatic hospitals from 2008 to 2016 were analyzed based on the Diagnosis Related Groups statistic from the German Federal Statistical Office and the data provided from the Norwegian Patient Registry. The Norwegian data also included information on individual patients whereas the German data only contained the total number of F68.1 assignment due to strict medical confidentiality laws. The incidence of the diagnosis of FD in Germany and Norway showed similar assignment rates with 3.71 and 3.18 per 100,000, respectively. The mean age was 39.4 years for German patients and 35.6 years for Norwegian patients. The gender distribution was almost equal for the individual patients' rate (49% female and 51% male). Furthermore, our results indicate that female patients with FD tend to demand healthcare services more frequently than male patients. Smaller studies focusing on the diagnosis of FD have significantly higher assignment rates compared to nationwide inquiries. Our results illustrate substantial differences between estimations of the incidence of FD and the need for further studies. Besides the many obstacles associated with diagnosis of FD, strict medical confidentiality laws prevent reliable and scientific investigations of this matter. Background The prevalence of smoking among patients undergoing bariatric surgery has been reported to be as high as 40%. The effect of smoking in the perioperative period has been extensively studied for various surgical procedures, but limited data are available for bariatric surgery. The objective of this study is to review the existing literature to assess (1) the impact of smoking on postoperative morbidity and mortality after bariatric surgery, (2) the relationship between smoking and weight loss after bariatric surgery, and (3) the efficacy of smoking cessation in the perioperative period among bariatric surgery patients. Methods A comprehensive search of electronic databases including MEDLINE, EMBASE and the Cochrane Library from 1946 to February 2020 was performed to identify relevant articles. Following an initial screen of 940 titles and abstracts, 540 full articles were reviewed. Results Forty-eight studies met criteria for analysis five structured interviews, three longitudinal studies, thirty-twonce among bariatric surgery patients in the perioperative period. The keto-enol tautomerisation in homophthalic anhydride (HA) is investigated in the ground (S0) and excited (S1) electronic states. The keto form with a dicarbonyl structure is found to be the most stable form in S0 and enol form with a monocarbonyl structure in S1 indicating an excited state intramolecular proton transfer (ESIPT) process. The computed results show consistency with the change in basis sets and methods of calculations. Apart from the two tautomers, transition states are also identified. The barrier to interconversion is found to reduce substantially in S1. Adavosertib order Internal reaction coordinate (IRC) calculations confirm the pathway of interconversion between the two forms in S0 and S1. The observed FT-IR spectra corroborate well with our computed spectra. The appearance of two strong lines around 1800 cm-1 confirms the lowest energy structure to be the keto tautomer with a dicarbonyl form in S0. Our computations corroborate well with the crystal structure data for an analogous molecule. Electron distribution in HOMO and LUMO indicate the excitation process as π → π* in nature. The qualitative chemical concepts like hardness and electrophilicity are calculated to estimate the stability of the tautomers. The energy and hardness profiles with the variation of IRC are opposite to each other, verifying the principle of maximum hardness. The present study aims to investigate the impact of a metal-tolerant bacterium on metal detoxification and rape seedling growth promotion under Cd stress. The results showed that the isolated bacterium Enterobacter sp. Zm-123 has capability to resist Cd (200 mg/L), produce IAA (26.67 mg/L) and siderophores (82.34%), and solubilize phosphate (137.5 mg/L), etc. Zm-123 inoculation significantly enhanced the fresh weight of rape seedlings from 9.47 to 19.98% and the root length from 10.42 to 57.05% compared with non-inoculation group under different concentrations of Cd (0, 0.5, 1, 3, 5 mg/L) (p less then 0.05). It also significantly increased the content of chlorophyll, soluble sugar, soluble protein, and proline (p less then 0.05) in rape seedlings. Moreover, a significant elevation in catalase (CAT) and peroxidase (POD) activities and a significant reduction in malondialdehyde (MDA), electrolyte leakage (EL), and Cd content in rape seedlings were detected owing to Zm-123 inoculation (p less then 0.05). The combined results imply that strain Zm-123 can alleviate the Cd phytotoxicity and promote the rape seedling growth by improving the physiological activity and antioxidant level, which can be potentially applied to protect plants from Cd toxicity. The development of societies has led information and communication technology (ICT) to play a gradually important role in people's lives, transforming the way societies and economies function. ICTs are often associated with the path to reducing CO2 emissions; however, do they lead to that path? Or are they themselves a growing source of energy consumption and emissions? The current study estimates the effect of ICT, trade, economic growth, financial development, and energy consumption on carbon emissions in South and Southeast Asian (SSEA) region for the period of 1990-2014. Moreover, the study also tried to validate the environmental Kuznets curve (EKC) hypothesis between GDP per capita and CO2 emissions. Cluster analysis was used to identify two groups (potential and advanced countries) based on their social development score. The long-run connection between the variables was examined and the long-run elasticities of ICT, financial development, energy consumption, trade, and economic growth with respect to e the integration of energy efficiency into user routines. Due to the increasing use of standby mode and Wi-Fi assistive devices, the rapid implementation of legislation regulating these technologies to make them more efficient is recommended.Exposure to contaminants is one of the main threats to all living organisms. In this context, bats have been used to indicate environmental contaminants in urban and agricultural environments, since they are extremely sensitive to changes in the ecosystem and easily accumulate waste in their body tissues. Among bats, Sturnira lilium, is a frugivorous species widely distributed and abundant in Brazil that uses an extensive variety of habitats and shelters. In this study, we aimed to evaluate the oxidative state of S. lilium individuals in agricultural and urban areas in southern Brazil. Individuals were sampled in agricultural and urban areas from November 2017 to March 2018 through the mist-net method. Parameters of the superoxide dismutase (SOD) and catalase (CAT) enzyme activity, non-protein thiols (NPSH), and lipid peroxidation (TBARS) were determined based on liver tissue. A total of 46 individuals were captured, 29 of them in urban areas and 17 in agricultural areas. We found that S. lilium individuals from agricultural areas showed a significant increase in TBARS, NPSH, and SOD activity, compared to individuals from urban areas. The activity of the antioxidant enzyme CAT did not differ. The present findings suggest that the species S. lilium, which are widely distributed and abundant in Brazil in urban and agricultural areas, can usefully be employed in biomonitoring programs. Further studies are to be encouraged to refine our knowledge on the potential DNA damage caused by environmental contamination, as well as identify potential contaminants to bats.
