In this study, we first discovered that ZEA stimulated 3T3-L1 adipocyte browning by increasing the expression of specific markers (Cd137, Tbx1, Sirt1, Cidea, Ucp1, Tmem26, and Cited1), thereby reducing lipid accumulation. Besides, ZEA promoted mitochondrial biogenesis by increasing the expression of PRDM16, UCP1, NRF2, PGC-1α, and SIRT1. Moreover, the uncoupled oxygen consumption rate (OCR) of protons leaked in 3T3-L1 adipocytes was rapidly increased by ZEA treatment, which improved mitochondrial respiration and energy metabolism. The purposes of this study were to determine the rate of failure of the hip joint after acetabulum fracture and to identify risk factors.
Acetabulum fractures treated over 17 years at a level-1 trauma center were reviewed. Patient, injury, and treatment factors were assessed regarding possible association with failure of the hip joint end-stage arthrosis and/or total hip arthroplasty (THA).
Seventy percent were treated with primary open reduction and internal fixation (ORIF). Seventy-two (12.5%) of 575 fractures underwent THA; 64 were after initial ORIF. The mean follow-up was 80 months, and the median time to THA was 14 months (range 10-200 months). Age, body mass index, fracture type, marginal impaction, and hip dislocations were independent predictors of hip joint failure. The mean injured age of THA patients was 53 versus 43 (P < 0.001). T-type fractures were most likely to fail (21% within 2 years, 45% within 10 years, P = 0.001). Other injury features marginal impaction and posterior hip dislocation were associated with failure with odds ratios 2.79 and 1.73, respectively (P < 0.001).

Arthroscopic rotator cuff repair (RCR) is associated with substantial postoperative pain. Oral narcotic agents are the preferred analgesic postoperatively. However, these agents are associated with several side effects and a potential for abuse. This study evaluates the efficacy of ketorolac as an adjunctive agent for postoperative pain control after arthroscopic RCR.
Adult patients undergoing arthroscopic RCR were prospectively enrolled and randomized to one of two groups. The control received our institution's standard-of-care pain protocol, including oxycodone-acetaminophen 5 to 325 mg on discharge. The ketorolac group received the standard-of-care protocol, intravenous ketorolac at the completion of the procedure, and oral ketorolac on discharge. Pain and functional outcome scores and narcotic utilization were recorded three times per day for the first 5 days after surgery. Repeat magnetic resonance imaging was done at least 6 months postoperatively.
In our study, 39 patients were included for final arthroscopic RCR.

To compare early goal-directed therapy (EGDT) 'With' and 'Without' intermittent superior vena cava oxygen saturation (ScvO2) monitoring in pediatric septic shock.

Patients not responding to fluid resuscitation (up to 40 ml/kg) were randomized to EGDT 'With' (n=59) and 'Without' (n=61) ScvO2 groups. Resuscitation was guided by ScvO2 monitoring at 1-hour, 3-hour, and later on six-hourly in the '' ScvO2 group and by clinical variables in the 'Without' ScvO2 group.
Primary outcome was all-cause 28-day mortality. Secondary outcomes were time to and proportion of patient achieved therapeutic endpoints (at 6 hours and PICU stay), need for organ supports, new organ dysfunction (at 24 hours and PICU stay), and length of PICU and hospital stay.
The study was stopped after interim analysis due to lower mortality in the intervention group. There was significantly lower all-cause 28-day mortality in EDGT with ScvO2 than without ScvO2 group [37.3% vs. 57.5%, adjusted hazard ratio 0.57, 95%CI 0.33 to 0.97, P=0.04]. Therapeutic endpoints were achieved early in 'With' ScvO2 group [mean (SD) 3.6 (1.6) vs. 4.2 (1.6) h, P=0.03]. Organ dysfunction by sequential organ assessment score during PICU stay was lower in with ScvO2 group [median (IQR) 5 (2,11) vs. 8 (3,13); P=0.03]. There was no significant difference in other secondary outcomes.

VDD was classified as 25(OH)D less then 20 ng/mL. The prevalence of VDD was 76.8% (n=327), and sun-exposure, male sex, and calcium and vitamin D supplementations during infancy were important determinants. Routine supplementation with vitamin D in infancy is likely to reduce the occurrence of VDD in children.Durations of any event, such as duration of hospitalization, has an annoying feature of highly skewed distribution and incomplete values due to dropouts and limited follow-up. The usual methods of statistical analysis are, therefore, not applicable. The method of survival analysis is a nonparametric method and is designed to overcome these problems. Survival is a generic term and is used for any time-to-event data. The entire survival pattern at different points in time is studied by the Kaplan-Meier method under certain conditions. Log-rank method is used to compare survival pattern in two or more groups. Hazard is the rate of occurrence of an event per unit of time and studied by Cox method. The concept of survival and all these methods of survival analysis are briefly discussed in this short note in a nonmathematical format for medical audience.
In India, till recently, breastfeeding women have been excluded from the coronavirus disease (COVID-19) vaccination program, rendering a significant population of the country, including frontline workers, ineligible to derive the benefits of the COVID-19 vaccine rollout.
The objective of this recommendation is production of an evidence-based document to guide the pediatricians to give advice to breastfeeding mothers regarding the safety of COVID-19 vaccines in lactating women.
Formulation of key question was done under the chairmanship of president of the IAP. It was followed by review of literature regarding efficacy and safety of COVID-19 vaccines in breastfeeding women. The recommendations of other international and national professional bodies were also deliberated in detail. The available data was discussed in the ACVIP focused WhatsApp group. Opinion of all members was taken and the final document was prepared after achieving consensus.
The IAP/ACVIP recommends the administration of COVID-19 vaccines to all breastfeeding women.
