Under the name of uncommon bladder cancers are gathered rare histological entities which represent less than 5% of bladder tumors. There is not a clear and consensual therapeutic management for these entities.
To review a single-institution 10-year experience with rare form of bladder cancers detailing the diagnosis, treatment, and patient outcome.
We performed a retrospective review of 27 medical records of rare bladder cancer form treated at our center between February 2006 and February 2015. The clinicopathologic features are reported with emphasis on treatment and survival.
Mean patient age was 65.5 ± 20 yr and 70% of patients were males. Smoking background was found in 16 cases, chronic bladder irritation factors were found in 12 cases, and past urinary tract infection was found in 11 cases. The main symptom was total hematuria (93%) causing an anemia in 16 cases. The two mean histological forms were epidermoid carcinoma (37%) and adenocarcinoma (22%). 26% of patients were found to have extended cystectomy concerning the conclusions could be drawn but elements suggest this may be the treatment of choice. The highly aggressive nature of those lesions justifies an aggressive and fast therapy when feasible which gives the best outcomes.
To examine the impact of endoscopy setting (hospital-based vs. office-based) on sedation/analgesia administration and to provide nationwide data on monitoring practices among Greek gastroenterologists in real-world settings.
. A web-based survey regarding sedation/analgesia rates and monitoring practices during endoscopy either in a hospital-based or in an office-based setting was disseminated to the members of the Hellenic Society of Gastroenterology and Professional Association of Gastroenterologists. Participants were asked to complete a questionnaire, which consisted of 35 items, stratified into 4 sections demographics, preprocedure (informed consent, initial patient evaluation), intraprocedure (monitoring practices, sedative agents' administration rate), and postprocedure practices (recovery).
211 individuals responded (response rate 40.3%). Propofol use was significantly higher in the private hospital compared to the public hospital and the office-based setting for esophagogastroduodenoscopy (EGD)n both for EGD and for colonoscopy. Greek endoscopists are adequately trained in airway management techniques.
The private hospital-based setting is associated with higher propofol sedation administration both for EGD and for colonoscopy. Greek endoscopists are adequately trained in airway management techniques. The current study is aimed at establishing links between brain network examination and neural plasticity studies measured by optical neuroimaging. Sixteen healthy subjects were recruited from the University of Macau to test the Granger Prediction Estimation (GPE) method to investigate brain network connectivity during figurative language comprehension. The method is aimed at mapping significant causal relationships across language brain networks, captured by functional near-infrared spectroscopy measurements (fNIRS) (i) definition of regions of interest (ROIs) based on significant channels extracted from spatial activation maps; (ii) inspection of significant causal relationships in temporal resolution, exploring the experimental task agreement; and (iii) early identification of stronger causal relationships that guide neuromodulation intervention, targeting impaired connectivity pathways. GSK2126458 in vitro Our results propose top-down mechanisms responsible for perceptive-attention engagement in the left anterior frontal cortex and bottom-up mechanism in the right hemispheres during the semantic integration of figurative language. Moreover, the interhemispheric directional flow suggests a right hemisphere engagement in decoding unfamiliar literal sentences and fine-grained integration guided by the left hemisphere to reduce ambiguity in meaningless words. Finally, bottom-up mechanisms seem activated by logographic-semantic processing in literal meanings and memory storage centres in meaningless comprehension. To sum up, our main findings reveal that the Granger Prediction Estimation (GPE) integrated strategy proposes an effective link between assessment and intervention, capable of enhancing the efficiency of the treatment in language disorders and reducing the neuromodulation side effects.Chemotherapy-induced peripheral neuropathy (CIPN) is a dose-limiting side effect caused by chemotherapy drugs, and its existence seriously affects the quality of life of patients. We first established an oxaliplatin-induced peripheral neuropathy (OIPN) model and then measured and evaluated mechanical hyperalgesia, thermal nociception, cold allodynia, and intraepidermal nerve fiber (IENF) density to determine Siwei Jianbu Decoction's role in preventing OIPN. Then, we conducted a systematic pharmacological study that revealed important roles for the MAPK signaling pathway and proinflammatory immune pathway and confirmed these roles by western blot, immunofluorescence, and qPCR. The data show that Siwei Jianbu Decoction can effectively prevent oxaliplatin-induced neuroinflammation by inhibiting an increase in NF-κB expression via downregulation of p-ERK1/2 and p-p38. The present study showed that SWJB may be beneficial in preventing oxaliplatin-induced peripheral neuropathy.Sound conditioning (SC) is defined as "toughening" to lower levels of sound over time, which reduces a subsequent noise-induced threshold shift. Although the protective effect of SC in mammals is generally understood, the exact mechanisms involved have not yet been elucidated. link2 To confirm the protective effect of SC against noise exposure (NE) and the stress-related signaling pathway of its rescue, we observed target molecule changes caused by SC of low frequency prior to NE as well as histology analysis in vivo and verified the suggested mechanisms in SGNs in vitro. Further, we investigated the potential role of Hsp70 and Bmi1 in SC by targeting SOD1 and SOD2 which are regulated by the FoxO1 signaling pathway based on mitochondrial function and reactive oxygen species (ROS) levels. Finally, we sought to identify the possible molecular mechanisms associated with the beneficial effects of SC against noise-induced trauma. Data from the rat model were evaluated by western blot, immunofluorescence, and RT-PCR. link3 The results revealed that SC upregulated Hsp70, Bmi1, FoxO1, SOD1, and SOD2 expression in spiral ganglion neurons (SGNs). Moreover, the auditory brainstem responses (ABRs) and electron microscopy revealed that SC could protect against acute acoustic trauma (AAT) based on a significant reduction of hearing impairment and visible reduction in outer hair cell loss as well as ultrastructural changes in OHCs and SGNs. Collectively, these results suggested that the contribution of Bmi1 toward decreased sensitivity to noise-induced trauma following SC was triggered by Hsp70 induction and associated with enhancement of the antioxidant system and decreased mitochondrial superoxide accumulation. This contribution of Bmi1 was achieved by direct targeting of SOD1 and SOD2, which was regulated by FoxO1. Therefore, the Hsp70/Bmi1-FoxO1-SOD signaling pathway might contribute to the protective effect of SC against AAT in a rat model. In order to track the desired path under unknown parameters and environmental disturbances, an adaptive backstepping sliding mode control algorithm with a neural estimator is proposed for underactuated ships considering both ship-bank interaction effect and shift angle. Using the features of radial basis function neural network, which can approximate arbitrary function, the unknown parameters of the ship model and environmental disturbances are estimated. The trajectory tracking errors include stabilizing sway and surge velocities errors. Based on the Lyapunov stability theory, the tracking error will converge to zero and the system is asymptotically stable. The controlled trajectory is contractive and asymptotically tends to the desired position and attitude. The results show that compared with the basic sliding mode control algorithm, the overshoot of the adaptive backstepping sliding mode control with neural estimator is smaller and the regulation time of the system is shorter. The ship can adjust itself and quickly reach its desired position under disturbances. This shows that the designed RBF neural network observer can track both the mild level 3 sea state and the bad level 5 sea state, although the wave disturbance has relatively fast time-varying disturbance. The algorithm has good tracking performance and can realize the accurate estimation of wave disturbance, especially in bad sea conditions.Perceptual organization is an important part of visual and auditory information processing. In the case of visual occlusion, whether the loss of information in images could be recovered and thus perceptually closed affects object recognition. In particular, many elderly subjects have defects in object recognition ability, which may be closely related to the abnormalities of perceptual functions. This phenomenon even can be observed in the early stage of dementia. Therefore, studying the neural mechanism of perceptual closure and its relationship with sensory and cognitive processing is important for understanding how the human brain recognizes objects, inspiring the development of neuromorphic intelligent algorithms of object recognition. In this study, a new experiment was designed to explore the realistic process of perceptual closure under occlusion and intact conditions of faces and building. The analysis of the differences in ERP components P1, N1, and Ncl indicated that the subjective awareness of perceptual closure mainly occurs in Ncl, but incomplete information has been processed and showed different manners compared to complete stimuli in N170 for facial materials. Although occluded, faces, but not buildings, still maintain the specificity of perceptual processing. The Ncl by faces and buildings did not show significant differences in both amplitude and latency, suggesting a "completing" process regardless of categorical features. In recent years, environmental awareness has increased considerably, and in order to decrease endangerments such as air and water pollution, and also global warming, green procurement should be employed. Therefore, in the assessment of suppliers, their environmental performance should be taken into consideration along with other criteria for supplier selection. Raising awareness of sustainability in production and conservation and protection of the environment is very important both for the whole environment and for the company itself by increasing its competitive advantage. And, one of the steps to achieve this is for the companies to try to select green suppliers. So, the purpose of this study is to raise awareness and tackle the need for green supplier selection and, using multiple-criteria decision-making models, to elaborate a case study regarding this. A survey was conducted in a manufacturing firm. The data were analysed, and fuzzy MCDM (multicriteria decision-making) methods and artificial neural networks were implemented.
