Urologists may hesitate to offer transurethral resection of the prostate (TURP) because of increased morbidity in elderly patients.

We aimed to compare data on postoperative outcomes of elderly men undergoing bipolar transurethral resection of the prostate (B-TURP) as compared to thulium laser vapoenucleation of the prostate (ThuVEP).

We retrospectively reviewed data of all patients aged ≥ 75years who underwent benign prostatic hyperplasia surgery. Differences between interventions were estimated using propensity scores (PS) to adjust for different patients characteristics.

Between 2017 and 2020, 275 men were included in the analysis. Propensity score retrieved 65 patients in each group. Median age was 78 (4) years in B-TURP group and 78 (6) in ThuVEP group. Median prostate volume was 63 (35) ml and 54 (24) ml in B-TURP group and ThuVEP group, respectively. Only American Society of Anesthesiologists score was significantly higher in ThuVEP group (p = 0.006). Median operation time, catheterization time, and hospital stay were similar in both groups (55min, 2 and 3days). Overall, 84.6% of men had no complications, with no significant differences between the groups (p = 0.234). Only one patient in B-TURP group experienced a Clavien grade IIIb complication. By 1year, there were a statistically significant differences in International Prostate Symptom Score (p = 0.000) in favor of B-TURP group and in maximal urine flow rate (p = 0.000) in favor of ThuVEP group.

In our everyday lives, we need to interact constantly and appropriately with others. Not only is ToM involved in understanding others' mental states (other-oriented mental inferences), but it also helps to keep our own mental states (self-oriented mental inferences). In this study, we designed a false-belief task using event-related potential (ERP) measures to investigate the dynamic differences between the two types of mental inferences. In the false-belief task, participants were prompted with a cue to attribute a belief to either themselves (self-oriented) or another person (other-oriented). Results showed that other-oriented false-belief attribution elicited a larger late positive component (LPC; 320~440 ms post-cue onset) than the one elicited by self-oriented attribution at bilateral parieto-occipital electrodes, and also a larger late negative component (LNC; 380~500 ms post-cue onset) at frontal-central electrodes. In addition, the difference in amplitude of LPC between the two types of false-belief attribution was positively correlated with self-reported autistic traits measured by the Autism Spectrum Quotient (AQ). Based on prior literature, we propose that the LPC and LNC reflect the processes of self-other distinction and conflict control, respectively. Moreover, the difficulty in distinguishing other from the self is related to one's degree of autistic traits.
Microelectrode recordings (MERs) are a significant clinical indicator for sweet spots identification of implanted electrodes during deep brain stimulation of the subthalamic nucleus (STN) surgery. As 1D MERs signals have the unboundedness, large-range, large-amount and time-dependent characteristics, the purpose of this study is to propose an automatic and precise identification method of sweet spots from MERs, reducing the time-consuming and labor-intensive human annotations.

We propose an automatic identification method of sweet spots from MERs for electrodes implantation in STN-DBS. To better imitate the surgeons' observation and obtain more intuitive contextual information, we first employ the 2D Gramian angular summation field (GASF) images generated from MERs data to perform the sweet spots determination for electrodes implantation. Then, we introduce the convolutional block attention module into convolutional neural network (CNN) to identify the 2D GASF images of sweet spots for electrodes implantaed networks model. Our method can assist neurosurgeons in automatically detecting the most likely locations of sweet spots for electrodes implantation, which can provide an important indicator for target selection while it reduces the localization error of the target during STN-DBS surgery.Ketamine in sub-anaesthetic doses has analgesic properties and an opioid-sparing effect. Intrathecal (i.t.) delivery of analgesics bypasses systemic metabolism and delivers the analgesic agent adjacent to the target receptors in the spinal cord and so small doses are required to achieve effective pain relief. In order to relieve intractable cancer-related pain, sustained-release ketamine formulations are required in combination with a strong opioid because frequent i.t. injection is not practical. In this study, ketamine or ketamine-loaded porous silicon (pSi) were encapsulated into poly(lactic-co-glycolic acid) (PLGA) microparticles by a novel supercritical carbon dioxide (scCO2) method, thereby avoiding the use of organic solvent. Multiple parameters including theoretical drug loading (DL), presence of pSi, size of scCO2 vessel, PLGA type, and use of co-solvent were investigated with a view to obtaining high DL and a sustained-release for an extended period. The most important finding was that the use of a large scCO2 vessel (60 mL) resulted in a much higher encapsulation efficiency (EE) compared with a small vessel (12 mL). In addition, pre-loading ketamine into pSi slightly improved the level of drug incorporation (i.e. EE and DL). Salicylic acid and methyl salicylate are among the widely used topical salicylates namely for keratolytic and anti-inflammatory actions, respectively. The current review summarises both passive and active strategies, including emerging technologies employed to enhance skin permeation of these two salicylate compounds. The formulation design of topical salicylic acid targets the drug retention in and on the skin based on the different indications including keratolytic, antibacterial and photoprotective actions, while the investigations of topical delivery strategies for methyl salicylate are limited. The pharmacokinetics and metabolisms of both salicylate compounds are discussed. The current overview and future perspectives of the topical delivery strategies are also highlighted for translational considerations of formulation designs.
Disorders of glucose metabolism are a serious acromegaly comorbidity and may be differently impacted by medical treatments of acromegaly. In this retrospective longitudinal multicenter study, we investigated the outcome of glucose metabolism and its predictors in patients treated with Pasireotide LAR (PAS-LAR) alone or in combination with Pegvisomant (PAS-LAR + Peg-V).

Acromegaly patients treated continously with PAS-LAR or PAS-LAR + Peg-V for at least 6 months.

Forty patients (25 females, 15 males) were enrolled. At last visit, 27/40 patients (67.5%) reached biochemical control of acromegaly. Overall, glucose metabolism improved in 3 (all in PAS-LAR + Peg-V; 7.5%), worsened in 26 (65%) and remained unchanged in 11 patients (27.5%). Glucose metabolism worsened in 25 patients (73.5%) treated with PAS-LAR and in a single patient (16.7%) treated with PAS-LAR + Peg-V (p < 0.001). Among patients treated with Pas-LAR alone, GH at baseline was higher in those with worsening of glucose metabolism (p = 0.04) as compared to those with stable glucose status. A significantly higher reduction of HbA1c was observed in patients treated with PAS-LAR + Peg-V, as compared with those treated with PAS-LAR alone (p = 0.005).

Attention toward natural compounds effectiveness and safety, supporting the development of a new nutraceutical science. In the alternative remedies for the treatment of prostatic diseases, medicinal herbs, in the form of herb parts or extracts, are getting attention due to their positive effects on prostatic diseases. At present, there is no available detailed literature review about the efficacy of medicinal herbs in the treatment of CP/CPPS. This review aimed to explore the useful medicinal herbs in the treatment of CP/CPPS from different perspectives and their possible mechanism of action in managing CP/CPPS.
