net.Exosome and biomimetic nanoparticles have great potential to develop into a wide-scale therapeutic platform within the regenerative medicine industry. Exosomes, a subgroup of EVs whose diameter ranges from 30-100 nm, have recently gained traction as an innovative approach for the treatment of various diseases, including heart disease. Their beneficial factors and regenerative properties can be contrasted between various cell types. Various biomimetic nanoparticles have also emerged as a unique platform in regenerative medicine. Biomimetic nanoparticles are a drug delivery platform that have the ability to contain both biological and fabricated components to improve therapeutic efficiency and targeting. The novelty of these platforms holds promise for future clinical translation upon further investigation. In order for both exosome therapeutics and biomimetic nanoparticles to translate to large-scale clinical treatment, numerous factors must first be considered and improved. Standardization of different protocols, from exosome isolation to storage conditions, must be optimized to ensure batches are pure. Standardization is also important to ensure there is no variability in this process across studies, thus making it easier to interpret data across different disease models and treatments. Expansion of clinical trials incorporating both biomimetic nanoparticles and exosomes will require a standardization of fabrication and isolation techniques, as well as stricter regulations to ensure reproducibility across various studies and disease models. This review will summarize current research on exosome therapeutics and the application of biomimetic nanoparticles in cardiac regenerative medicine, as well as applications for exosome expansion and delivery on a larger and more clinical scale. In injury, tissue damage, or tumor formation, they are activated and leave their niche to migrate to the site of injury, where they release a plethora of growth factors, cytokines, and other bioactive molecules. With the accumulation of data about the interaction between MSCs and tumor cells, the dualistic role of MSCs remains unclear. However, a large number of studies have demonstrated the natural anti-tumor properties inherent in MSCs, so this is the basis for intensive research for new methods using MSCs as a tool to suppress cancer cells development. This review focuses specifically on advanced approaches in modifying MSCs to become a powerful, precision-targeted tool for killing cancer cells, but not normal healthy cells. Suppression of tumor growth by MSCs can be accomplished by inducing apoptosis or cell cycle arrest, suppressing tumor angiogenesis, or blocking mechanisms mediating metastasis. In addition, the chemosensitivity of cancer cells may be increased so that the dose of the chemotherapeutic agent used could be significantly reduced. One of the best ways to improve these values in patients with psychiatric disorders is to prefer long-acting injectable (LAI) drugs to oral AP. OBJECTIVE The objective of this study is to evaluate adherence, persistence and switching of antipsychotics and compare in real life long-acting with oral formulations. MATERIALS AND METHODS This pharmacological, observational, retrospective, non-interventional study involved all patients of the ASL of Pescara treated in the front-line with AP in the period between January 2011 and February 2019. Adherence was measured using the ratio between received daily dose and prescribed daily dose. Persistence to treatment with antipsychotics was calculated as the daily difference between start and end of treatment. Eliglustat order RESULTS We examined 840 patients treated with Aripiprazole, 130 patients treated with Paliperidone and 925 patients treated with Risperidone. Adherence was significantly better in long-acting formulations with values of 0.89 (Aripiprazole) and 0.82 (Paliperidone and Risperidone) than in oral formulations with values of 0.78, 0.70 and 0.58, respectively. Persistence curves over three years of therapy have not shown a statistically significant difference (p = 0.3314). Persistence curves based on formulation have not shown a statistically significant difference. 7% of patients treated with Aripiprazole, 12% of patients treated with Risperidone and 28% of patients treated with Paliperidone switched therapy. CONCLUSIONS In all drugs of the present study, adherence values were better in LAI than in OA, whereas no statistically significant difference was found in persistence values. Some symptoms of diabetes, such as painful peripheral neuropathy and nocturia, or associated other sleep disorders, such as sleep breathing disorders or sleep movement disorders, may influence the sleep quality and quantity. Both sleep disorders and diabetes may lead to cognitive impairment. The risk of development of cognitive impairment in the diabetic patients may be related to vascular and non-vascular and other factors, such as hypoglycemia, hyperglycemia, central insulin resistance, amyloid and tau deposits and other causes. Numerous sleep disorders, e.g., sleep apnea, restless legs syndrome, insomnia and poor sleep quality most likely also associated with cognitive impairment. Adequate functioning of the system of clearance of the brain from toxic substances, such as amyloid β, i.e. glymphatic system, is related to undisturbed sleep and prevents cognitive impairment. In case of coexistence, sleep disturbances and diabetes either independently lead to and/or mutually aggravate cognitive impairment.
