Whether there are racial differences in the efficacy/safety of hypnotics has not been sufficiently investigated. We aimed to evaluate the efficacy/safety of lemborexant 5 mg and lemborexant 10 mg vs placebo once daily in a subset of Japanese patients with insomnia and to compare the results with those of non-Japanese patients.
This subanalysis reports the results of the first 6 months (period 1, placebo-controlled) of SUNRISE 2, a 12-month, global, randomized, double-blind, phase 3 study. Changes in patient-reported sleep onset latency, patient-reported sleep efficiency, and patient-reported wake after sleep onset with lemborexant 5 mg or lemborexant 10 mg vs placebo were evaluated. Treatment-emergent adverse events were evaluated for safety.
In total, 949 patients were randomized (Japanese, n = 161; non-Japanese, n = 788). Groups were balanced at baseline except for the male/female ratio (P = .0002) and body mass index (P < .0001) in the Japanese vs non-Japanese subgroups. Overall, the efficacy and ; Identifier 2015-001463-39.

To evaluate the utility of a contact-free device in screening for obstructive sleep apnea.
Three hundred fifty-nine participants (mean age 46 ± 13 years, body mass index 26.1 ± 4.2 kg/m², 67.7% male) underwent overnight monitoring using a contact-free device, the OrbSense, and polysomnography (PSG) in the sleep laboratory simultaneously. The OrbSense recordings were analyzed automatically, and PSG was scored based on recommended guidelines.
The respiratory event index from the OrbSense was lower than the apnea-hypopnea index (AHI) from PSG (25.5 ± 20.7 vs 27.0 ± 25.2 events/h; P = .007) and was significantly correlated with AHI (Pearson coefficient, 0.92; P < .0001). Bland-Altman analysis showed a mean difference of 1.5 events/h, and the limit of agreement was -18.6 to 21.5 events/h. Use of the OrbSense resulted in larger underestimates of AHI and lower negative predictive values at higher AHI values (especially when AHI ≥ 30 events/h). This research letter presents a cluster of severe medical conditions that were delayed in diagnosis due to postponed presentation to healthcare facilities during the COVID-19 pandemic. Ewing sarcoma, severe hemolytic anemia, endocarditis requiring surgery, and septic hip requiring surgery are some examples of cases we encountered with delayed diagnoses. This led to the appearance of a rather low burden of disease in the pediatric population during the pandemic, and pediatric hospitals and clinics experienced a very low volume of activity. Given the low burden of COVID-19 in children, and the well-defined separation between infected and non-infected areas within the hospitals, we should consider improving the guidelines and messages conveyed to the public regarding the importance of prompt medical assessment for other medical conditions, even during a pandemic, along with reassurance of the safety of entering medical facilities given the strict isolation procedures being observed. Conclusion Medical associations should reconsider the messages being sent to the public during future outbreaks, and encourage medical assessment.
A clinical and/or research fellowship abroad has become a prevalent choice among Israeli physicians. However, the influence of fellowship programs on the career path is unclear. We evaluated the role of physicians returning from fellowship in the organizational hierarchy and their professional and academic status.
This was a retrospective, descriptive, cross-sectional study of physicians who completed a survey after accomplishing a fellowship. The survey included questions about the physicians' attitudes toward the program, programs' details, and the physicians' current academic, professional, and administrative status. Information about scientific publications was also collected.
Of the 106 physicians receiving the questionnaire, 101 responded. The majority completed a two-year fellowship in North America. Forty percent participated in an integrated program (research and clinical), and 40% participated in clinical programs. Subjectively, the physicians attributed a significant value to the fellowship and positively recommend it. Most of the physicians held managerial positions, academic appointments, and had generated significant research.
The subjective perspective of all physicians participating in the study was that attending a fellowship program had a positive impact on their careers. Objectively, the accomplishment of a fellowship program empowered the studied physicians to become scholars, senior executives, and opinion leaders in their professional field.

Although modern cochlear implants (CIs) are approved for magnetic resonance imaging (MRIs) adverse events still occur with unacceptable frequency. Methods In this retrospective study, magnet displacement due to MRIs was analysed. Relevant factors e.g. symptoms during MRI, diagnostics, surgical intervention following the diagnosis and possible subsequent damage were assessed.
16 patients were enclosed. All patients complained about pain while the scan was conducted. Computed tomography (CT) scans of the temporal bone or X-rays of the skull were performed to confirm diagnosis. Artefacts on CT scans delayed immediate diagnosis in some cases.
Despite various studies demonstrating the range of adverse events related to CIs following MRI, little information is available on diagnosis and radiologic recognition of magnet dislocation. In patients complaining about pain following an MRI scan an X-ray of the head should be performed immediately. Most adverse events occur in radiological centres without expertise in cochlear implants.

X-ray of the skull is suggested to be used as the method of choice in imaging.Sediment microbial fuel cells (SMFCs) have served as an alternative technique to suppress phosphorus release from lake sediments to water bodies and thus mitigate eutrophication. However, the phosphorus regulation mechanism remains unclear. The purpose of this research was to understand the electrochemical influence of an SMFC on the phosphorus concentration in interstitial water. In this study, a lab-scale SMFC was applied to acetate-spiked sediments (ace+) and unspiked sediments (sed) with closed-circuit (CC)/open-circuit (OC) columns, and the circuitry was switched to investigate the relationship between electron transfer and phosphorus concentration. The dissolved total phosphorus (DTP) concentration in the sediment interstitial water in CC columns significantly decreased to below 0.1 mg/L, whereas the DTP in OC columns remained high for nine weeks. After switching the circuit, the DTP in OC→CC columns dropped but that in CC→OC columns increased within one week. PX478 At the end of the experimental period, the DTP concentrations in CC/sed, CC/ace+, OC/sed, and OC/ace+ columns were 0.10 ± 0.02, 0.03 ± 0.00, 0.82 ± 0.01, and 1.66 ± 0.12 mg/L, respectively. The respective estimated anode capacitances of those columns were 2.05 ± 0.49, 5.15 ± 0.14, 0.72 ± 0.19, and 0.71 ± 0.12 nF. We concluded that the phosphorus may have been electrochemically attracted and retained on the anode in the sediment because the adsorbed DTP contents and the increased anode capacitances were strongly correlated. Thus, SMFCs can be used for suppressing phosphorus release from eutrophic lake sediments.
To understand experiences with and measure the effect of mobile standing desks (MSD) on undergraduates' sedentary time (ST).

Graphite carbon nitride (g-C3N4), which has the advantages of simple preparation, safety, non-toxicity and chemical resistance, was expected to become a photocatalyst for solving environmental pollution. However, the performances of g-C3N4 still have some limitations that the electron hole recombination is fast and the powder is not easy to recover. In this study, zinc-doped graphite carbon nitride photocatalyst (Zn-g-C3N4) was mixed with polyacrylonitrile (PAN) to produce photocatalyst fibres by electrospinning. It not only solves the problem that the powder catalyst is difficult to recycle, but also effectively inhibits the recombination of photoelectron-hole pairs. Zn-g-C3N4/PAN has good photocatalytic activity for the simultaneous reduction of hexavalent chromium and degradation of pharmaceuticals. When organic pollutants are present, the reduction efficiency of hexavalent chromium was improved without affecting its own removal efficiency. The potential application value of Zn-g-C3N4/PAN catalytic fibre was further explored by simulating the complex actual water environment. The composite fibre can be easily reused and keep its superior photocatalytic performance. The mechanism of pharmaceuticals degradation was proposed, in which ∙ O 2 - is the most important active species, which leads to the oxidation of pharmaceuticals. Besides, the photoelectrons generated by the catalyst can reduce the toxic hexavalent chromium. The efficiency of Zn-g-C3N4 to remove pollutants is improved by PAN fibre as a carrier, which not only solves the problem of difficult recovery of powder catalysts, but also provides more active sites.
